1.A Case of Cutaneous Matestasis Originating from Thymic Carcinoma.

We report a case of cutaneous metastasis of thymic carcinoma in Korean. A 44-year-old female, who had been diagnosed as thymic carcinoma and treated with total thymectomy and post-operative radiation therapy, presented grouped erythematous papules on the left chest. CT scans revealed metastatic lesions in the left pleura and abdominal cavity. Histopathologic examination of skin biopsy showed extensive infiltration of malignant cells in dermis and subcutis. The morphology of infiltrating cells was similar to that of tumor cells from primary thymic carcinoma specimen. The results of immunohistochemical study for skin biopsy were consistent with the features of non-neuroendocrine thymic carcinoma. Histopathologic and immunohistopathologic similarity between cutaneous lesion and previous tumor suggested that malignant cells in the skin lesion could originate from progressing thymic carcinoma.

2.A Case of Glomeruloid Hemangioma in a Patient with Multicentric Castleman's Disease.

Glomeruloid hemangioma is a histologically distinctive cutaneous angioma which is rarely de-scribed in patients with POEMS (polyneuropathy, organomegaly, endocrinopathy, M-protein, and skin changes) syndrome and multicentric Castleman's disease. We report an additional case of glomeruloid hemangioma in a 30-year-old Korean woman with multicentric Castleman's disease showing features of POEMS syndrome. Histopathology revealed multiple dermal dilated vascular spaces composed of a conglomerate of capillaries, resulting in structures reminiscent of renal glomeruli. Periodic acid-Schiff-positive and diastase-resistant eosinophilic globules were found within the cytoplasm of vacuolated endothelial cells. The endothelial cells lining the capillary loops showed positive immunostaining for factor VIII-related antigen and CD31.

3.Two Cases of Pigmented Fungiform Papillae of the Tongue.

Pigmented fungiform papillae of the tongue(PFPT) is a rare benign pigmentary disorder of the tongue. In dark-skinned individuals, PFPT appears to be relatively common, whereas this entity is seldom reported in fair-skinned people and Asians. Although the relevance to systemic conditions has been suggested in several reported cases, the etiology and pathogenesis of this entity are not yet clearly defined. We report two cases of PFPT in young Korean females. Clinically both patients presented with asymptomatic pigmentary lesions confined to fungiform papillae of the tongue, which had been detected several months ago. Histopathologic findings revealed increased melanophages in the subepidermal area within the fungiform papillae.

4.A Case of Eosinophilic Pustular Folliculitis (Ofuji's Disease) in a Newborn.

We describe a case of eosinophilic pustular folliculitis in a 1-month-old Korean male infant. The patient was suffering from erythematous papules and pustules affecting the scalp, trunk, and extremities, detected at birth. The flare was accompanied by leukocytosis and eosinophila. Histopathology revealed folliculitis with a predominant eosinophilic infiltrate. Lesions showed partial response to topical steroid and disappeared in four weeks. Three months after the resolution of the lesions, he presented a similar clinical picture as the previous episode.

5.Correction of the Aging Upper Third of the Face Using by Transpalpebral Approach.

Aging changes on the upper third of the face include brow ptosis, glabellar furrowing and forehead transverse wrinkle caused by an imbalance of the forehead muscles. Surgical methods to correct these phenomena include forehead lift with a coronal incision, endoscopic access, and direct forehead skin excision. In response to a common need for cosmetic improvement in the brow-upper eyelid and a desire for minimal incisions for forehead rejuvenation, an approach through blepharoplasty incision had been developed. This approach has many advantages, such as fewer skin incision, less tissue mobilization and direct excision of the hyperactive depressor muscles. However, this approach is not suitable for those who have significant forehead wrinkle and ptosis. We used a combined subperiosteal approach for forehead lift and transection of the hyperactive corrugator and procerus muscles through blepharoplasty incision. This combined surgical technique is simple and effective to correct aging of the upper third of the face without the usual complication. We performed 10 cases of the transpalpebral approach for the correction of the upper third of the face from January 1997 to September 1998, following them up for from 3 to 6 months and obtained satisfactory results. We report our experiences with a review of the literature.

6.Two Cases of Plasma Cell Cheilitis Treated with Intralesional Injection of Corticosteroids.

Plasma cell cheilitis is a rare inflammatory disorder which shows a dense, band-like infiltrate of plasma cells in the upper dermis. Although the histological picture is shared with other diseases of the lips, plasma cell cheilitis is not associated with any known dermatoses. Some authors have shown the effectiveness of topical or intralesional corticosteroids; however, there have been many reports describing poor therapeutic response to topical steroids. We de-scribe two patients with plasma cell chielitis whose clinical condition responded rapidly to the intralesional injection of corticosteroids.

7.Acquired Partial Lipodystrophy (Barraquer-Simons Syndrome): Early Cosmetic Intervention with Autologous Fat.

Barraquer-Simons syndrome is a rare acquired lipodystrophy characterized by gradually symmetric subcutaneous fat loss in a craniocaudal distribution, often associated with hypocomplementemia and nephropathies. Facial cosmetic treatment in this disorder has not been fully described in the literature. We present a patient with Barraquer-Simons syndrome with emphasis on early cosmetic intervention with autologous fat grafting and its long-term efficacy. At the follow-up 37 months after the last fat grafting, preservation of the grafted fat was noted while lipodystrophy progressed in the trunk regions. Autologous fat grafting is suggested for the correction of facial dysmorphism in this type of lipodystrophy.

8.Gram-negative Folliculitis following Prolonged Use of Broad-spectrum Antibiotics.

Gram-negative folliculitis is an infection with Gram-negative bacteria that most often occurs as a complication of prolonged broad-spectrum antibiotic therapy in patients suffering from acne or rosacea. A 21-year-old male soldier with acne vulvaris had been treated with broad-spectrum antibiotics for two months because of severe pyoderma on the soles. While the plantar lesions improved significantly, prolonged use of the systemic antibiotics resulted in widespread follicular pustules on his face, scalp, and back of the neck. A skin biopsy showed features of superficial pustular folliculitis. Klebsiella species were identified from the pus culture. A diagnosis of gram-negative folliculitis was made. The lesions almost disappeared after 4 weeks of treatment with oral isotretinoin (0.5 mg/kg/day).

10.Caterpillar Dermatitis Caused by Euproctis similis.

The yellow-tail moth Euproctis similis, belongs to the genus Euproctis (Lymantriidae) and has a wide distribution within Korea, Japan, China and Europe. Contact with the 'urticating hairs' of the Euproctis similis caterpillar induces dermatitis due to the combination of mechanical and chemical mechanisms. Clinically, patients with caterpillar dermatitis show wheals in the early stage and/or erythematous papules in the late stage after contact with the spicules. We report 5 young soldiers who developed pruritic wheals and erythematous papules on their upper back after contact with the hairs of the Euproctis similis caterpillar.
