Malignant melanoma is unique in its propensity to metastasize to the heart with frequencies ranging from 50% to 71%. We report a case of a resection of large intracavitary malignant melanoma causing obstruction of the right ventricular inflow and outflow tract of the heart. A 49-year-old-woman presented clinical symptoms and signs of life-threatening congestive right heart failure. Transthoracic and transesophageal echocardiography revealed a large intracavitary mass occupying the entire right ventricle. Inguinal lymph node biopsy demonstrated metastatic melanoma. The cardiac main mass was palliatively resected and demonstrated to be a metastatic melanoma. The patient improved symptomatically about 60 days after the operation.

This study was made to investigate the influence of mouth breathing to tongue, mandible and hyoid bone position. It has been clinically suggested that the mouth breathing is induced by the respiratory dysfunction of nasopharyngeal airway causing by the Adenoids. The author used the 50 children, who were the nasal breathes with normal occlusion as the control group, and 50 children, who were mouth breathers with Adenoid as the experimental group. Results were as following: 1. In experimental group, the tongue was positioned more anterior and lower than that of the normal children. 2. In experimental group, the mandible was positioned more lower than that of the normal children. 3. In experimental group, the hyoid bone was positioned more anterior and lower than that of the normal children.

This study was conducted to evaluate the effect of prolonged treatment in children with chronic hepatitis B with combination of alpha-interferon and acyclovir. The study population consisted of 7 patients with chronic hepatitis B who showed histological findings compatible with chronic active hepatitis (4 cases) or chronic persistent hepatitis (3 cases) on liver biopsy and had elevated AST and ALT with positive HBsAg, HBeAg and HBV-DNA for more than 6 months. Recombinant interferon-alpha-2 was given in a dose of 3 million IU/m2 daily for 3 months with acyclovir in a dose of 15mg/Kg of body weight every 12 hours for 7 days of each month for 3 months. Then, the same dose of interferon was given thrice a week for 9 more months. One patient who had vertically transmitted hepatitis B from her mother had received only 6 months of treatment in total as she did not show any effect after 6 months of treatment. Among 6 patients who had positive HBV-DNA on entry to the study, 4 patients (66.7%) showed conversion of HBV-DNA to negative. HBeAg was converted to negative in 3 patients (42.9%). AST and ALT became normal in 6 cases but HBsAg remained positive in all cases. prolonged treatment in children with chronic hepatitis B with combination of alpha interferon and acyclovir seems to be beneficial but the proper dosage and duration of therapy need to be determined with further and controlled study.

Leiomyomas are benign tumors of smooth muscle origin and are very rare in the female genital tract. To date, approximately 35 cases of urethral leiomyoma have been reported in the literature. A 34-year-old woman presented with a 3-year history of a mass at the urethral meatus. Physical examination showed 2 x 1.5 cm lump at the urethral meatus, posterior lip. Histologically the tumor was mainly composed of benign cigar shaped smooth muscle izells which were arranged in interlacing fascicles without cellular atypia or mitosis. Immunohistochemistry confirmed leiomyoma with positive staining for vimentin, desmin and muscle specific actin.

Many articles concerning the accuracy of diagnosis of malignant tumor cells in body fluids have appeared in the literature, but few authors have attempted to describe the characteristics of these cells to determine the site of the primary tumor as they relate to tumors of specific primary sites. This paper presents the results of a retrospective study on malignant body cavity effusions of which the primary site was established on the basis of either biopsy or surgical resection of the primary neoplasm during the period of 6 years beginning from January 1983 to December 1988. The results obtained are summarized as follows: 1) The 143 fluid specimens from 129 patients were composed of 51 cases of pleural, 69 of peritoneal, and 9 of pericardial origin. 2) Adenocarcinoma was the most frequent type of malignant effusions (78.3%). The most common primary site was the lung (50%) in pleural fluid and stomach (55.2%) in ascites. 3) The results of this study show that the primary site of tumor cells can be identified in the body fluid of accurate cytomorphologic criteria are used. This study was made to investigate the effect of adenoidectomy on dentofacial skeleton in naso-respiratory dysfunction children. The clinical material compromised the 24 children in a previous study who had naso-respiratory dysfunction and 24 children who were the nasal breathing with normal occlusion. The cephalograms were taken at the initial examination and 1 year later for the control group and experimental group the paired sample statistical analysis was performed. The results were as follows. 1. In cranial base variable, difference between two groups were not statistically significant. 2. In craniofacial variables, experimental group showed brachyfacial pattern but control groups didn't show significant growth pattern. 3. In maxillary variables, experimental group showed flattening the plane. 4. In mandibular variables, experimental group showed the decrease of mandibular plane angle and gonial angle. 5. In facial height variables, experimental group showed horizontal growth rotation.

The predictive accuracy of the SRK II, SRK/T and Holladay formulas was evaluated in 22 eyes with sever myopia who had phacoemulsification and implantaton of posterior chamber IOL without suture. These patients were followed up for at least 6 months postoperatively. Correlation coefficient between the predicted refraction and actual refraction was 0.57(P<0.01) in the eyes using the SRK/T, 0.566(P<0.01) in the Holladay and (0.06(P=0.791) in the SRK II. The mean absolute error in the SRK/T 0.85 was smaller than that of the Holladay 1.04 and the SRK II1.08(p<0.05). The percentage of the eyes with absolute error within 1D was 68% in the SRK/T, 68% in the Holladay and 53% in the SRK II, but there was no statistically significant difference between them(p>0.05). The percentage of the eye with hyperopic shift(more hyperopic actual refraction than the predicted refraction) was 77% in the SRK II, 77% in the SRK/T and 86% in the Holladay, but there was no statistically significant difference between each formula(p>0.05). In conclusion, the SRK/T has the better predictive accuracy than the SRK II and Holladay in severe myopia and hyperopic shift tends to occur with the application of all three formulas.
