The tuberculosis of the stomach is very rare disease and its association with gastric carcinoma is more so. The diagnosis of gastric tuberculosis is usually made on the histological basis not clinical findings. This case of gastric tuberculosis was found incidentally after the subtotal gastrectomy performed under the impression of stomach cancer. There was an ill-defined ulcerative lesion at the lesser curvature. Microscopically, poorly differentiated adenocarcinoma extended to the serosa and typical tuberculous granulomas were found in the lesion of stomach and regional lymph nodes. Acid-fast bacilli were demonstrated by Ziehl-Neelsen stain in the granuloms of the regional lymph nodes, and a case of gastric carcinoma is presented here and briefly reviewed with the literature.

Although there have been many reports about the importance of the spleen's role in hepatic fibrogenesis, the exact mechanism is still uncertain. The author designed this study to evaluate splenic function on hepatic fibrogenesis. The degree of hepatic fibrosis and the population of splenocyte subsets were studied in the experimental animal model with fibrosis produced by injecting normal swine serum intra-peritoneally into Sprague-Dawley rats. The animals were divided into three groups; group A was subjected to injection of swine serum only, group B swine serum and complete Freund's adjuvant and group C swine serum and cyclosporin A. The experimental hepatic fibrogenesis by swine serum was augumented by coinjection with the adjuvant and inhibited by cyclosporin A. The study of the splenocyte subset revealed increased percentages of spienic B cell and CD4+ cell and a decreased percentage of CD8+ cell, and these changes of splenocyte subset were also augumented by the adjuvant and inhibited by cyclosporin A. The percent of monocytes was not significantly altered, although a tendancy of early decrease by the adjuvant was noted.

A total of 53 patients infected with Taenia solium were treated with praziquantel in two different dose levels. Twenty-six cases were treated with praziquantel in a single oral dose of 5 mg/kg body weight. All cases except one were completely cured. The cure rate was 96.2 per cent. The other 27 cases were treated with a single dose of 10 mg/kg body weight. All cases were completely cured and revealed a cure rate of 100 per cent. Side effects were mild and transient. About half of the cases complained of a mild abdominal pain. Soft stool or diarrhea and dizziness were observed in some cases. Tests on clinical hematology, serum biochemistry and urinalysis were performed immediately before and one day after treatment in all cases. There were no significant abnormalities detected in these tests.

To clarity the effect of biliary obliteration on copper metabolism of rat liver and on the hepatic morphology, 0.5% cuppuric sulfate was administered intraperitoneally for 42 days following ligation of the common bile duct (CBD) of Sprague-Dawley rats. The blood copper concentration, the hepatic copper content and the accumulation patterns of copper and copper binding protein in the liver were examined and compared with those of the simple CBD ligation group and the simple copper over loaded group. CBD ligation induced marked proliferation of bile ductular structures which, after expanding the portal tracts, invaded and divided the hepatic lobules. There was, however, no excess fibosis beyond what needed to support the new ductules. The blood copper concentration and the hepatic copper content were increased by copper overload with or without CBD ligation, particularly incases with CBD ligation. Liver cell necrosis did not occur by the overloaded copper alone in rats. The hepatic copper and copper binding protein were accumulated at periportal liver cells in the group of coppe overload after CBD ligatio, whereas they began to appear at perivenular hepatocytes in the simple copper overloaded group. In conclusion, it is suggested that CBD ligation does not induce excess fibrosis or liver cirrhosis in rat as far as during our experimental period, but affect significantly on copper metabolism by intrahepatic redistribution of the copper and the copper binding proteins.

A total of 73 enucleated eyeballs is reviewed and analyzed clinicopthologically. These eyeballs were selected among the enucleated spceimens that had been removed at the Yeungnam University Hospital during a period of 10 years beginning from 1983 to 1992. Following results were obtained. 1) When the eyeballs were classified according to me direct cause of removal, the neoplasm was the most common single cause accounting for 26 cases(35.6%) out of 73 cases, followed by phthisis bulbi l6 cases(21.9%), trauma 10 cases(13.7%), glaucoma 8 cases(10.9%), inflammation 5 cases(6.8%), staphyloma 4 cases(5.5%), retinal detachment 1 cases(1.4%), Coat's disease 1 cases(1.4%), corneal disease 1 cases(1.4%) and choroidal hemorrhage 1 cases(1.4%). 2) 39 cases(53.4%) were male and 34(46.6%) were female. 23 cases(31.5%) were below 10 years of age, which was the highest rate. 3) The neoplastic lesion included retinoblastoma 20 cases(76.9%) in 26 neoplasms, malignant melanoma 4 cases(15.3%), hemagioblastoma of optic disc 1 cases(3.9%), adenocarcinoma of Meibomian gland 1 cases(3.9%). 4) Retinoblastoma was the commonest intraocular tumor accounting for 20 out of 26 cases, In growth pattern, 80.0% of the tumor grew endophtytically. True rosette were seen 60% of the retinoblastoma.

PURPOSE: To evaluate the contrast enhancement patterns and mass detection rate of metastatic adenocarcinoma of the liver in the early and late phase of spiral bolus dynamic CT. MATERIALS AND METHODS: Two-phase spiral bolus dynamic CT of the liver was performed on 34 patients with clinically or pathologically proved metastatic adenocarcinoma of the liver after bolus administration of 120 mL of contrast material intravenously. CT scanning was started at 45 seconds and 6 minutes after I~eginning of injection in 23 patients, and at 35 seconds and 3 minutes in another 11 patients. The enhancement patterns of the tumors were classified into 6 types compared with attenuation of surrounding normal liver parenchyma, as diffusely high, peripherally high, centrally high, mixed, iso, diffusely low. The enhancement patterns of 76 tumors in 34 patients were analysed. RESULTS: In the early phase, peripherally high attenuation was seen in 34(45%) and diffusely low attenuation in 23(30%). In the late phase, diffusely low attenuation was seen in 27(36%) followed by iso and mixed in 14 (18%), in each, and peripherally high in 13(17%). The appearances of enhancement pattern from the early to the late phase were variable. 12(16%) were diffusely low in both phases, 11(14%) were peripherally high in the early phase and diffusely low in the late phase, and 10(13%) were peripherally high in both phases. Number of tumor lesions detected were more in the early phase in 12(35%)patients, and in the late phase in 3(9%) patients. CONCLUSION: Metastatic adenocarcinoma of the liver showed variable enhancement patterns in the early and late phases of spiral bolus dynamic CT, although the most common pattern was peripherally high in early phase, and diffusely low in the late phase. The detection rate of masses was higher in early phase than late. The two-phase spiral bolus dynamic CT should be good method in detection and differential diagnosis of metastatic adenocarcinoma of the liver.

Purpose: The purpose of our study was to assess the clinical and radiological results of isolated acetabular revision and the differences in the results between monoblock and modular femoral stems. Materials and Methods: Between October 1991 and June 2002, 39 patients (39 hips) underwent isolated acetabular revisions with use of uncemented cups and bone grafts. The mean period of follow-up was 5 years 3 months (range, 2 years 1 month to 9 years 8 months). The mean age of the patients at the time of the surgery was 51 years old (range, 26 to 75 years old). The monoblock femoral stems were used in 19 hips and the modular femoral stems were used in 20 hips. The monoblock stems were retained without exchange provided that there was no gross scratch on the femoral head and the femoral stem was confirmed to be stable and fixed intraoperatively. In the modular stems, the femoral heads were always exchanged with new ones. Results: The mean Harris hip score improved from 57 to 87 points. Radiologically, all the stems retained at surgery remained well osseointegrated without osteolysis. Re-revisions of the acetabular components were performed in 5 hips using monoblock stems due to periacetabular osteolysis caused by excessive wear of polyethylene and subsequent loosening of the cups in 3 hips and mechanically unstable acetabular cups without osteolysis in 2 hips. The mean wear rate of polyethylene coupled with the monoblock stem was 0.27 mm/year which was greater than the 0.11 mm/year in the cases with modular stems. Conclusion: To prevent failure of acetabular cups that may occur later by excessive wear of polyethylene, we recommend revision of the well-fixed monoblock femoral stems even though there is no gross scratch on the femoral heads of the monoblock stems.
