1.Clinical Study of 326 Cases of Vitiligo.

We evaluated the clinical manifestations of 326 patients of vitiligo who had visited Seoul National University Hospital. The results were as follows : 1) There were 141 males (43.3%) and 185 females (56.7%). 2) The mean age of onset was 19.6 years (male : 18.3 years, female : 20.6 years). 3) The mean age at the first visit wss 23.9 years (male : 22.5 years, female : 24.9 years). 4) Duration of disease was less than 2 year in 156 patients (47.9%). 5) The most common site of initial involvement was face (36.2%). The common sites of involvement were face (55.2), abdomen (32.5%), neck (26.7%) and scalp (24.8%) in decreasing order of frequency. 6) There were 167 cases (51.2%) of generalized type, 118 cases (36.2%) of localized type, 31 cases (9.5%) of dermatomal type and 10 cases (3.1) of halo nevus. The most frequent site of dermatomal type was neck. Gray hair was noted in 91 cases (27.9%) and mucosal involvement was present in 43 cases (13.2%). 7) Family history was obtained in 24 cases (7.4%). Koebner phenomenon was present in 48 cases (14%). There were no precipitating factors in 309 cases (94.8%) prior to development of vitiligo. 8) The associated diseases were 1 csse of thyroiditis and 3 cases of alopecia areata.

2.Foreign-body Granuloma After Honeybee Acupuncture.

Bee sting therapy is said to have been practised since the age of ancient Egypt. Some people believe in the benefit of apitherapy for articular inflammation, hematoma, sciatica, hay fever, etc. However, honeybee has several venoms that may induce various clinical and histopathological findings. We report a case of foreign-body granuloma after honeybee acupuncture for polyarthralgia. To our knowledge, this is the first report of foreign-body granuloma developing at an acupunctured site in English literature.

3.The Comparative Study of the Side Effects of Copper Vapor Laser and Flashlamp - Pumped Pulsed Tunable Dye Laser Treatment.

BACKGROUND: There are two kinds of mechanism for laser therapy according to selectie photothermolysis. Selective photothermolysis means that a chromophore can be selectively dei, stroyed with a laser of n appropriate wavelength and of a short pulse duration that is shortor than the thermal relaxation time of the chromophore. It is supposed that the side effects of the continuous wave laser, in which selective photothermolysis does not exist occur more often than when a pulsed wave laser is used. OBJECTIVE: We compar.d the side effects of flashlamp-pumped pulsed tunable dye laser(SPTL), whose treatment is baseal on selective photothermolysis, with those of copper vapor laser(CVL , which is a quasi-continuous thermal laser. METHODS: The laser cl arts of 498 patients with SPTL treatment and 485 patients with CVL, treatment in Asan Medical Center from 1989 to 1994 were examined, retrospectively. RESULTS: 1) Among 498 patients, with SPTL treatment, we observed various side effects in 47(9.4%). The incidences of individial side effects are as follows, hyperpigmentation in 6.2%(31), vesicl formation in 1.6% (8), surface texture change in 0.8% (4), Hypopigmentation in 0.6% (3), scar format,ion in 0.2% (1). 2) After CVL treatment, side effects occurred in 30(6.2%) among 485 patient.. Hyperpigmentation in 3.1%(15), vesicle formation in 2.5%(12), surface texture change in 0.2%(1), hypopigmentation in 0.2%(1), and scar formation in 0.1%(1) were observed. CONCLUSION: The total incidence of side effect.s after CVL treatment is not statistically different, from that after SPTL treatment, which contradicts the previous theory.

4.A Clinical Observation in 14 Children with SLE.

To elucidate the clinical characteristics of childhood onset systemic lupus erythematosus (SLE), we carried out a retrospective reivew on medical records of fourteen patients who were diagnosed as SLE on the base of "the 1982 revised criteria for the SLE"(by american Rheumatism Association) at the Department of pediatrics, Hanyang University Hospital from January 1980 through August 1992. The results were summerized as follows: 1) Eleven girls and three boys were recruited in our study, The eight of them were early teens at the time of SLE diagnosis. 2) The frequencies of their clinical manifestations on admission were persistent facial rashes, edema, hepatosplenomegaly, renal abnormalities were proteinuria and hematuria. 3) Immunologic abnormalities were noted as follows: Positive ANA 100%. positive LE cell 64%, elevated anti DS-DNA antibody 64%, and decreased only C3 level 28%, decreased both C3 & C4 64% of the patients. 4) Serum anti-DS DNA(>10units) titer was increased in clinical exacerbated three patients and decreased in four resolved. 5) The high ANA titers( >1;80)obtained in eleven and/or homogenous pattern shown in seven strongly suggested SLE. 6) Renal biopsies of the four patients revealed lupus nephritis and three of the four patients had lupus band in their biopsied skin. 7) Three of 14 patients were recoverd spontaneously with supportive care. Other 11 patients were treated: 7 with prednisolone only, 4 combined prednisolone and other agents. After treatment five were clinically improved. But 4 patients were frequently exacerbated over 2 years, and 2 patients were died of their complications. In conclusion, serum anti-DS DNA level and ANA may be useful for the screening of SLE and for predicting clinical course and prognosis of SLE.

7.Porokeratosis: Clinical Observation of 29 Cases.

BACKGROUND: Porokeratosis is a disorder of epidermal keratinization which is characterized by the presence of cornoid lamellae. Since the original description of the plaque form of Mibelli, various types of porokeratosis have been reported. OBJECTIVES: The purposes of this study were aimed at evaluating the clinical and biological features of porokeraosis. METHODS: The hospital charts and slides of 29 patients with porokeratosis were reviewed. Only the cases showing characteristic cornoid lamellae by histopathological examinations were included. RESULTS: Female patients outnumbered the male ones (M:F = 1:2.2). Ages of the onset of porokeratosis varied from infancy to 68 years. Disseminated superficial actinic porokeratosis (DSAP) was the predominant type (22 cases). The Mibelli type was seen in 3 cases and the linear type in 4 cases. Only one case had a family history of porokeratosis. Two patients had complained of pruritus in the lesions, which were the DSAP and linear type, respectively. In that linear type, squamous cell carcinoma developed. Eight patients had other associated diseases. Although 6 patients were treated with CO₂ laser, topical isotretinoin or cryotherapy, the lesions tended to recur, persist or even progress. CONCLUSIONS: Porokeratosis showed various clinical features. DSAP was the predominant type. Two patients complained of itching of the lesions, which were the DSAP and linear types, respeciively. Within that linear type, squamous cell carcinoma developed. We speculate that pruritus might be an ominous sign of malignant transformation of the porokeratosis. We suggest that the Mibelli type and linear type of porokeratosis should be either excised or destroyed, or in widespread cases close follow-up should be mandatory.

Adenomas of anogenital mammary-like glands have been rarely reported with various nomenclatures in the English literature. The knowledge of this rare disease entity may be important to diagnose and treat the lesions arising on the anogenital areas.

Digital mucous cysts encompass two pathogenetically distinct entities, the cutaneous myxomatous type and ganglion type. We describe an 8-year-old boy with a digital mucous cyst on the ventral side of the proximal phalanx of the right fifth finger. The case shows characteristic histopathological findings, but in an unusual location.

10.Renal Replacement Therapy in Acute Kidney Injury: Indication, Proper Initiation, and Prescription.

Despite substantial advances in dialytic techniques and machines, acute kidney injury (AKI) requiring renal replacement therapy (RRT) is associated with up to 60% in-hospital mortality. But, there is no full detail of definite RRT to overcome the significant morbidity and mortality of AKI. What is most important in the treatment for AKI is that RRT is not a cause-specific therapy but a life-supportive management. This review discusses the indications of, proper initiation of, and optimal prescription for RRT to improve the survival of the patients with AKI.
