2.Total Hip Arthroplasty with Use of Proximal Modular Femoral Stem in Secondary Coxarthrosis of Hip Associated with Deformed Femur.

Purpose: The purpose of this study was to evaluate the clinical and radiological results of total hip arthroplasty using a proximal modular femoral stem in patients who had secondary coxarthrosis associated with a deformed femur. Materials and Methods: Forty-two patients (45 hips) with secondary coxarthrosis were evaluated after primary total hip arthroplasty using an S-ROM proximal modular femoral stem, between January 2001 and March 2004. The average follow-up was 44 months (range, 24 to 60 months). The mean age of the patients was 48.5 years old and there was a predominance of female patients (65.5%). The preoperative diagnoses included 26 cases of developmental dysplasia of the hip, 13 cases of sequalae of LCP, 2 cases of epiphyseal dysplasia, 3 cases of sequalae of pyogenic arthritis, and 1 case of congenital coxa vara. Results: The average Harris hip score improved from 52.2 points to 85.5 points. All the femoral stems demosntrated stable fixation, which included 37 cases by bony ingrowth and 8 cases by stable fibrous ingrowth. Neither osteolysis nor progressive radiolucent lines around the femoral stems were found at the latest follow-up. Postoperative complications included: 2 cases of hip dislocations, 1 case of periprosthetic fracture, 1 case of infected cup loosening, and 1 case of sciatic nerve palsy. Overall, forty-one hips (91.9%) exhibited excellent or good clinical results at the most recent follow-up. Conclusion: For advanced secondary coxarthrosis, total hip arthroplasty with use of a proximal modular femoral stem yielded good mid-term results based on clinical and radiological criteria.

3.Bone Marrow Pressure of the Femoral Heads of Korean Adults

Bone marrow pressure is neither arterial pressure nor venous pressure of extremity, that is thought the precapillary pressure of bone. It is different between long bone and flat bone, and is also different in various site of the bone. Author measured bone marrow pressure of 84 normal femoral heads of Korean adults for basic data of early diagnosis of avascular necrosis of femoral head. The results are followings : 1. The baseline bone marrow pressure of Korean healthy adults is average 22.5 ±7.2mmHg. 2. The most significant change of bone marrow pressure in stress test is observed at 5 min. after injection of 5cc normal saline. 3. The increase of bone marrow pressure at 5 min. after stress test was average 7.4 ±4.4 mmHg. 4. Sex and age factors do not influence bone marrow pressure.

5.Cases with Endometrial Polyp and Endocervical Polyp Associated With Tamoxifen Use.

Tamoxifen is a nonsteroidal triphenylethylene delivative that has been widely used in the treatment of breast cancer. tamoxifen is indicated for the adjuvant treatment of breast cancer in women following total mastectomy or segmental mastectomy, breast irradiation and chemotherapy. tamoxifen has been shown to have significant benificial effect in the treatment of breast cancer patient as hormonal therapy. However ,there is mounting evidence that tamoxifen may affect other hormon sensitive organs, including the uterus and ovaries. An increased risk of endometrial polyps, endometrial hyperplasia and adenocarcinoma has been reported in tamoxifen treated women. We have met cases of endometrial polyp and endocervical polyp which were associated with tamoxifen use, after modified radical mastectomy for infilterating ductal carcinoma of breast . So we report these cases with the brief review of literatures.

6.Effect of Gl Cyclins Expression on Clinical Prognostic Parameters in Cervical Carcinoma.

Alterations in the expression of genes that control the cell cycle may be of critical importance in tumorigenesis and malignant transformation. The major regulatory events leading to cell proliferation occur in Gl phase of cell cycle, and the deregulated expression of Gl cyclins is related to oncogenesis. Cyclins Dl and E play important roles in the progression of cell through Gl phase of the cell cycle. Amplification and/or overexpression of the cyclin Dl gene and aberrant expression of cyclin E has been described in various forms of human cancer. However, the role of cyclins Dl and E in cervical cancer has been poorly defined. In this study, we examined the expression of cyclins Dl and E by Northern blot technique and the status of human papil- lomavirus(HPV) type 16 and 18 by polymerase chain reaction in 25 cases of cervical carcinoma to explore the relationship between cyclins Dl and E and cervical cancer. We found cyclin Dl expression showed down-regulated expression in cervical cancer but cyclin E expression was increased in cancer group. Other clinicopathological prognostic factors were not correlated with cyclins D1 and E expression. Further study based on larger numbers of cases with correlation of cyclins D1 and E status and survival data will be needed to elucidate the use of cyclin expressions as prognostic factor.

7.Study of Vitiligo and Nevus Depigmentosus in Children by Clinical Evaluation and DOPA Staining.

We performed analysis of 79 childhood patients diagnosed as vitiligo and nevus depigmentosus in Tediatric Dermatologie Department of Seoul National University Hospital from March 1987 to September 1987. The results were as follows : 1. Of the 79 patients, 31 patients(39.2%) were vitiligo 21 patients(26.6% )were nevus depigmentosus and 27 patients(34.2%)were uncertain by cliniral diagnostic ctriteri.. 2. Viiligo began rarely in infancy(6.4% )and most appeared evenly in all childhood over 1 year old. Nevus clepigmentosus was present at birth in 57.1% of patients and was discovered within the first 6 months after birth in 42.9% of patients. 3. In vitiligo, most lesions were multiple. The most common site of involvement was the face, and there was this tendency of periorficial predisposition. In nevus ciepigmentosus, most lesions were solitary. The most, common aites of involvement were the lowed limbs and trunk, but there was no tendency of periorfcial presposition. 4. We found focal type of vitiligo in,51,6% of patients, vulgaris type in 32. 3 % acrofacial type in 9.6% and segmerital type in 6.5%. We also found isolated pattern of nevus clepigrnento us in 76.2% of patients, ciermatomal p<:ttern in 19.0ki and whorlec1 pattern in 4.8% The contrast enhiancement between the hypopigmented skin and nomal surrounding skin by Wood's light, examinatior reflected the epiclermal melanin pig merit, i. e., the more marked loss of pigmentation, the more accentuatian of the contrast The before, Wood's light examination without I:3OPA staining could help to ciiagnose the atisculte type of vitiligo showing eidermal melanocytes and nevu c.Iepigmeritosus.

8.Clinicopathologic Features and HBsAg and HBeAg Expressions in Hepatitis B Virus-associated Glomerulopathy.

Morphometry of nuclei of the benign and malignant prostatic lesions was performed to study the relationship between nuclear size and shape and the prognosis of prostatic adenocarcinoma. Fifty one cases of prostatic adenocarcinoma and 13 cases of benign prostatic hyperplasia were included to evaluate area, perimeter, Dmax, Dmin, and 5 form factors of the nuclei by image analyzer (Zeiss Ibas 2000) using hematoxylin-eosin stained slides. All analytic factors of nuclear size and shape were significantly different between benign lesions and adenocarcinomas. Increased nuclear size was associated with nu- clear irregularity, presence of metastasis, advanced clinical stage, and high Gleason's grade and score of prostatic adenocarcinoma. On Kaplan-Meier method, survival was decreased with older age, no hormonal treatment, stage D, high Gleason's grade and stage as well as with larger size and irregular shape of the nuclei. In conclusion, morphometry of nuclei of the prostate can be a helpful tool to differentiate between benign and malignant lesions. Nuclear morphology is thought to be associated with prognosis of prostatic adenocarcinoma.

9.A Case of Febrile Ulceronecrotic Pityriasis Lichenoides et Varioliformis Acuta.

Febrile ulceronecrotic PLEVA is an unusually severe from of PLEVA, characterized by the sudden onset of diffuse ulceronecrotic eruption aasociated with high fever. A mild eruption precede the acute fulminating course, We observed a 13-year-old boy presenting the form of febrile ulceronecrotic PLEVA. The histologic features were those of PLEVA. He received the systemic corticosteroids with aupportive care. To recognize this disorder is important because of a possibly fatal outcome, So close attention and vigorous therapy are necessary.

10.The Complication in the Treatment Modality of Intertrochanteric Fracture of Femur

Intertrochanteric fractures of femur were increasing in their incidence in morden life. The compression hip screw and Ender nail are popular treatment modalities of the interochanteric fracture of femur. We had tried to clarify the complications of intertrochanteric fracture according to the treatment modality-compression hip screw and Ender nail. We obtained the results as followings with the patients who had intertrochanteric fracture of femur which were managed by compression hip screw (56 cases) and Ender nail (22 cases) through the period of March 1987 to June 1993. The summary of the results; 1. Mechanical complications were developed in 9 patients(16%), 9 complications in the group with compression hip screw and 7 patients(32%), 12 complications in Ender nail. 2. Local complications were developed in 4 patients(7%) in compression hip screw and 10 patients (46%) in Ender nail and the most common local complication was the knee joint pain(7 patients; 32%). 3. Statistically, more significant decrease of neck-shaft angle was shown with group using Ender nail for unstable fracture than stable fracture group and compression hip screw group. 4. The degree of shortening was incrased in group with Ender nail than in group with compression hip screw, but there was no statistical significance. 5. The grade of osteoporosis was not related to the change of neck-shaft angle and shortening and also to the development of mechanical complication. According to the above results, compression hip screw was a more stable fixation device than Ender nail in the unstable intertrochanteric fracutre.
