1.Subjectivity on Childbearing in High School Students.

PURPOSE: This study was to explore subjectivity on childbearing in high school students. METHODS: A Q-methodology which provides a method of analyzing the subjectivity of each type was used. Forty-three high school students classified 40 selected Q-statements into 9 points standard. The obtained data were analyzed by using the pc-QUANL program. RESULTS: High school students' subjectivity on childbearing were analyzed into two types: Type 1 turned out to be 'FOLS (family oriented life style)' and Type 2 'CINK (couple important no kid)'. CONCLUSION: In order to resolve such problems as low birth rate and the advanced age of the population, effective youth and adult programs, policy and institution are required. The current demographic, economic and other factors such as personal values and policies may lower birth rate. In particular, youths' need for children and birth rate tend to be lower. There are positive and negative patterns in high school students' subjectivity on childbearing. Thus, national and social efforts are needed to change negative factors into positive ones. In order to maintain positive subjectivity on childbearing in high school students, it is necessary to apply family-centered educational programs and to implement birth-friendly and realistic programs for promoting child birth.

2.Angiomyofibroblastoma of the Vulva: A case report.

Angiomyofibroblastoma is a distinctive, hitherto uncharacterized, benign soft tissue tumor of the vulva with histology similar to an aggressive pelvic angiomyxoma. It can be distinguished from an aggressive angiomyxoma by its circumscribed borders, higher cellularity, more numerous blood vessels, the frequent presence of plump stromal cells, minimal stromal mucin, and rarity of erythrocyte extravasation. We experienced a case of angiomyofibroblastoma of the vulva occurring in a 45-year-old woman. The lesion was a well-defined but not encapsulated mass, 4.5x4.2 cm. Histologically the mass was characterized by alternating hypercellular and hypocellular edematous zones in which abundant blood vessels were irregularly distributed. Immunohistochemically, the spindled, plump spindled, and oval stromal cells were reactive for vimentin and desmin, but not for cytokeratin, or S-100 protein.

3.A Review of Improvements for Providing Safe and Secure Environments for Medical Treatment

On December 31, 2018, an incident occurred where a doctor was attacked and killed by a patient carrying a lethal weapon in the outpatients' clinic of the psychiatric department of a tertiary general hospital. The suspect was diagnosed with bipolar affective disorder (manic depressive disorder) and has been hospitalized and cared for in the psychiatric ward of this hospital. This incident illustrates the necessity of more active cures and therapeutic intervention for mental patients with intellectual developmental disorders who require treatment considering the fact that a radical outcome has been caused by such a patient. However, on the other hand, there is also a need for an approach and analysis from the perspective of crime prevention for all medical departments. The reason for this is that even a tertiary general hospital equipped with the largest human resources, medical devices, facilities, and so forth, is susceptible to violence. As for illegal actions perpetrated against health and medical service personnel in medical institutions, such as verbal abuse, assault, injury, etc. there have neither been understanding shown for the current extent of damage in detail, nor discussions of active institutional improvement related to the seriousness of the act. It can be said that violence in the field of medical treatment is a realm requiring serious discussion and appropriate remedial actions. This is because when such incidents take place, if a patient who is supposed to get treatment from the damaged health care provider is in an urgent situation or on the waiting list of serious cases, he or she could suffer serious damage caused by deprivation of treatment opportunity, or secondary damage might be caused to the patient and/or a guardian who can hardly have an opportunity to take action. Accordingly, in this review, we would like to help create the necessary conditions for both health and medical service personnel and patients/guardians, respectively, to provide and receive medical treatment in a more secure environment. Therefore, objective assessment of the institution and issues relating to this aforementioned incident and general cases of violence occurring in medical institutions, and by suggesting legal and institutional improvements and solutions.

4.Effects of Self Efficacy Promoting Programs on Self Efficacy, Self Care Behavior and Psychosocial Adaptation in Patients with a Colostomy.

PURPOSE: This study was conducted to investigate the effects of self-efficacy promoting program on self-efficacy, self-care behavior and psychosocial adaptation in patients with a colostomy. METHOD: A non-equivalent control group pre test post test design was used. The self efficacy promoting programs was composed of a CD image program based on varicaious experience, education and telephone coaching program based on verbal persuasion, and Stoma care practice, and Self care performance based on performance accomplishment. This study was carried out from July 2, 2005 to April 20, 2006 and 21 patients with a colostomy at one of 2 hospitals participated. Descriptive statistics, chi-square-test, and Mann-whitney U test were used to analyze the data. RESULTS: There were significantly different for specific self efficacy, self care and psychosocial adaptation between the experimental group and control group. CONCLUSION: The self efficacy promoting program for patients with stomas was effective in improving degree of specific self efficacy, self care and psychosocial adaptation.

5.Endobronchial hemorrhage after intubation with double-lumen endotracheal tube in a patient with idiopathic thrombocytopenic purpura for minimally invasive cardiac surgery: a case report.

Minimally invasive cardiac surgery (MICS) requires lung isolation. Lung isolation is usually achieved with double-lumen endotracheal tube (DLT). Patients with idiopathic thrombocytopenic purpura (ITP) have an increased risk of bleeding events. We suspected endobronchial hemorrhage after exchange of DLT during induction of anesthesia for replacement of mitral valve in a 62-year-old man with a known ITP. The MICS was stopped and bronchial artery embolization was performed in the angiographic room. In the present case, in order to reduce the risk of bronchial arterial injury in ITP patient we intubated with single lumen endotracheal tube. Lung isolation led to achievement of intermittent total lung deflation. Based on the results, we recommend a high-dose intravenous immunoglobulin therapy and platelet transfusion prior to cardiac surgery in patients with ITP to increase platelet count. Moreover, it is proposed that in order to clear the vision during the operation, ventilation can be held or made intermittent both prior to cardiopulmonary bypass or at its conclusion to permit exposure.

6.Clinical Usefulness of X-Ray Findings for Non-specific Low Back Pain in Korean Farmers: FARM Study.

OBJECTIVE: To elucidate the association between non-specific low back pain (NSLBP) and spinal X-ray findings in Korean farmers: Farmers' Cohort for Agricultural Work-Related Musculoskeletal disorders (FARM) study. METHODS: A total of 835 farmers (391 males, 444 females; mean age, 56.6±7.4 years) without red-flag signs of specific LBP were recruited. Presence of LBP more than one week or once a month with more than moderate degree of pain severity during the last year was assessed with a binary questionnaire (yes or no). Spinal degenerative changes were classified into disc height change (DHC) of L4-5 and L5-S1 (grade 0–5) and osteophyte formation of L5 (grade 0–5) by a radiologist based on X-ray findings. Additionally, spondylolisthesis, scoliosis and spondylolysis were assessed. RESULTS: General prevalence of NSLBP was 40.7%, revealing a higher incidence of NSLBP in female and younger farmers compared to male and older farmers (χ²=23.3, p<0.001; χ²=4.54, p<05, respectively). Among X-ray findings, DHC (L5–S1) grade 4 revealed significantly higher relative risk of NSLBP compared to grade 0 (odds ratio, 5.00; 95% confidence interval, 2.05–12.20) after adjusting age and sex, while other X-ray findings were not associated with NSLBP. CONCLUSION: The NSLBP of Korean farmers was significantly related to lumbar disc degenerative changes, suggesting clinical usefulness of X-ray findings in assessing LBP in farmers.

7.A CLINICAL STUDY OF PEDIATRIC MANDIBULAR FRACTURE.

As jaws of children differ from those of adults in the aspects of architectural and mechanical properties, site, form and treatment modality of pediatric fractured jaws are somewhat different from those of adults. This study takes stastics of 33 pediatric patients with mandibular fractures (55 fracture sites) who were admitted to the department of oral and maxillofacial surgery. Kyung-hee university from May 1986 to March 1995, aged up to 15years with primary or mixed dentition, by age, cause, site and treatment modality. The results are as follows. 1. The mandibular fracture has sexual predilection for male, 0 to 3 years group. 2. The main cause of mandibular fracture is falling from various position, followed by traffic accident. 3. The highest incidence of fracture site is symphysis and/or condyle area. 4. Circumferential wiring is widely used for pediatric mandibular fracture.

8.The neointimal hyperplasia effect of erythropoietin on carotid artery injury model of rat.

BACKGROUN: Neointimal hyperplasia is major cause of instent restenosis in coronary artery and stenosis in arteriovenous fisula for hemodialysis. Erythropoietin is known to show proliferative effect on vascular smooth muscle cells in vitro study, but there is few in vivo study. This study investigated the effect of erythropoietin given subcutaneously to injured carotid arteries of rats on neointimal hyperplasia. METHODS: Sprague-Dawley rat underwent common carotid artery (CCA) balloon injury. Erythropoietin was given subcutaneously to balloon-injured rats (n=6) in 100 U/Kg/week by dividing three times. The control group (n=6) was treated with normal saline only. Two weeks later, the lumen and the neointimal area were obtained and compaired. Actively proliferating cells of neointimal area were observed by microscopy using PCNA staining method. RESULTS: Luminal area was 0.233 +/- 0.073 mm2, 0.112 +/- 0.047 mm2 in the control and the erythropoietin treated group respectively. The area was significantly smaller in the treatment group (p<0.05). Neointimal area and neointimal area/medial area ratio were 0.148 +/- 0.029 mm2, 0.226 +/- 0.056 mm2 and 1.169 +/- 0.146 min, 1.953 +/- 0.465 min in the control and the erythropoietin treated group respectively (p<0.05), but no significant difference was observed in medial area between the control and the erythropoietin treated group. Actively proliferating cells of neointimal area were more commonly observed in erythropoietin treated group. CONCLUSION: Erythropoietin increases neointimal cell proliferation in the rat carotid artery injury model. So, erythropoietin treatment may contributes to the development of arteriovenous fistula stenosis caused by neointimal hyperplasia but, further studies are required to elucidate the mechanism and the effect of erythropoietin at various time and dose in neointimal hyperplasia.

9.A Case of Bronchiolitis Obliterans Organizing Pneumonia After Thoracotomy.

We report a patient who suffered from bronchiolitis obliterans organizing pneumonia(BOOP) after Ivor Lewis operation for esophageal cancer. The patient presented low grade fever, dry cough and mild dyspnea at 4 day after operation. Chest roentgenograms and chest CT revealed bilateral patchy and infiltrative shadows. The respiratory symptoms worsened and respiratory failure developed with mild elevation of WBC count despite of conservative treatment. An open lung biopsy was done and the biopsy specimen showed bronchiolitis obliterans organizing pneumonia(BOOP). After several weeks of steroid therapy, there were marked clinical, physiological and roentgenographic improvements. Our experience suggests that BOOP may be one of the underlying pathology in a number of patients presenting with ARDS after thoracotomy. Since steroid therapy may improve survival in these patients, thoracic surgeons should heighten their index of suspicion for this entity. Early histologic diagnosis should be considered in patients with treatment-resistant ARDS after thoracotomy.

10.Transabdominal sonography of the cavum septum pellucidum in normal fetuses in the second and third trimesters of pregnancy.

OBJECTIVE: To assess the visualization rate and size of the cavum septum pellucidum (CSP) by transabdominal sonography in normal fetuses in the second and third trimesters of pregnancy. METHODS: The CSP was prospectively researched and measured using an axial transventricular plane in 308 consecutive uncomplicated singleton pregnancies between 15 and 41 weeks of gestation. RESULTS: The CSP was seen in 42.5% of cases at 15 weeks, 84.78% at 16-17 weeks, 100% at 18-37 weeks and 81.25% at 38-41 weeks. Compared to biparietal diameter (BPD), the visualization rate was 35.29% between 31 and 32 mm, 47.8% between 33 and 34 mm, 84% between 35 and 43 mm, 100% between 44 and 88 mm and 87.5% between 89 and 101 mm. Mean CSP width was 5.5+/-1.7 mm (range 2-9 mm). The CSP width was increased with gestational age and BPD, but with a slight decrease around term. CONCLUSION: In normal fetuses the CSP should always be visualized between 18 and 37 weeks, or with a BPD of 44-88 mm. Failure to observe the CSP in this interval, or possibly the presence of a large CSP, may indicate abnormal cerebral development and warrant further investigation. Conversely, absence of the CSP prior to 18 weeks, or later than 37 weeks, is a normal finding.
