2.Clinical and Radiological Outcomes of Hook Plate Fixation in the Lateral End Fracture of the Clavicle and Acromioclavicular Dislocation.

BACKGROUND: The purpose of this study was to identify the clinical and radiological outcomes of hook plate fixation for lateral end fracture of the clavicle and acromioclavicular dislocation. METHODS: There were a total of 20 cases with lateral end fracture of the clavicle and 16 cases with acromioclavicular dislocation. All patients were evaluated for range of motion, functional score by using Constant score, and American Shoulder and Elbow Surgeons shoulder index at just before implant removal and at final follow-up. Coracoclavicular distance was measured in acromioclavicular dislocation and bony union was evaluated in the lateral end fracture of the clavicle. RESULTS: The clinical outcomes and range of motion were increased at the final follow-up compared with just before implant removal in both the lateral end fracture of the clavicle and acromioclavicular dislocation. In acromioclavicular dislocation, all cases—except one—showed maintenance of reduction after implant removal. Moreover, in the lateral end fracture of the clavicle, all cases—except one—showed bony union. CONCLUSIONS: Hook plate fixation in the lateral end fracture of the clavicle and acromioclavicular dislocation resulted in good clinical and radiological results.

3.Psoriasiform Sarcoidosis.

Sarcoidosis is a systemic granulomatous disease of undetermined etiology and pathogenesis that involves the skin and many of the internal organs. It involves any organ of the body; how-ever, the most common sites are the lung, lymph node, skin, and eye. Psoriasiform eruption is a rare morphologic manifestation of sarcoidosis. We report herein a case of 36-year-old man with psoriasiform papules on the elbows, forearms and knees of 5-year duration. Histopathologic examination revealed sarcoid granuloma throughout the dermis. This is the second case of psoriasiform sarcoidosis in Korea.

4.Clinicopathologic Study of Labial Melanotic Macule.

BACKGROUND: Labial pigmented lesions include labial melanotic macule, ephelids, lentigo, venous hemangioma, amalgam tattoo, junctional nevus, Peutz-Jeghers syndrome, Addison's disease, Laugier s disease, and superficial spreading melanoma. OBJECTIVES: The purpose of this study was aimed at investigating the clinical and histopathological characteristics of labial melanotic macule during the past 10 years. METHODS: Clinical information of 49 patients with pigmented lesion of the lips diagnosed in Asan Medical Center from 1989 to 1999 was obtained from the medical records and clinical follow-ups. We re-evaluated all the biopsy specimens obtained from the patients. RESULTS: Twenty-six patients with labial melanotic macule were enrolled in this study. There were 16 women and 10 men. Age at onset varied from 20 to 65 years in women and from 28 to 68 years in men. The duration of the lesion ranged from 4 months to 12 years (mean, 4.5 years). The majority of patients had solitary lesions on the lower lip. Histopathologic examination of biopsy specimens showed increased pigmentation of the basal layer, mild acanthosis without elongation of rete ridges, and scattered melanophages in the dermis. CONCLUSIONS: We suggest that labial pigmented lesions appearing in adults should be biopsied and labial melanotic macule should be diagnosed after the histopathological examination.

Actinic granuloma (O'Brien) is an annular inflammatory reaction that develops in skin after long-term sun-exposure. Anetoderma is characterized by discrete skin lesions that appear to be loose and wrinkled. The primary type arises in clinically normal skin, and the secondary type appears in lesions of other disorders. We describe a case of anetoderma that developed on the neck, both arms and calves, and showed histopathological findings of actinic granuloma.

6.Dynamic MRI of Internal Derangement of Temporomandibular Joint.

PURPOSE: To evaluate the value of fast field echo(FE) technique in accesing temporOmandibular joint(TMJ) dynamics. MATERIALS AND METHODS: A series of 120 TMJ MRI examinations of 60 symptomatic patients was performed. The fast FE technique was used to provide dynamic images of the TMJs in various phase of mouth opening with a step-bite-block. RESULTS: Among 120 cases, there were 30 cases which were normal, 21 cases of anterior displacement with reduction, 66 cases of anterior displacement without reduction, 11 cases of deformity of menisci, 23 cases of bony deformity, and 3 cases of medial displacement. The menisci and condyles were clearly delineated and the motions of the menisci during jaw opening were well evaluated in both normal and pathologic conditions. CONCLUSION: The dynamic TMJ MRI scan using fast FE technique permits both fast scanning and study of joint dynamics as well as TMJ anatomy.

8.Comparison of Cine Magnetic Resonance Imaging with Doppler Echocardiography for the Quantative Evaluation of Tricuspid Regurgitation in Newborn.

BACKGROUND: Cine MR imaging has been found to bo a non-invasive technique in evaluation cardiac anatomy and cardiovascular function in a variety of cardiac diseases. The capability of this technique for quantatively estimating the severity of regurgitation has not been tested in newborns. The purpose of this study is to compare the degees of tricuspid regurgitation by echorcardiography with those by cine MRI, and verify the utility of MRi in evaluating tricuspid regurgitation. METHODS: Fifty-one newborns with tricuspid regurgitation were evaluated on the same day by echocardiography and cine MRI. Area and regurgitant fraction of area were estimated at four chamber view by two techniques. RESULTS: The area of tricuspid regurgitation was similar by two techniques. The correlation for the area(r=0.66, p<0.025) and the regurgitant area fraction(r=0.647, p<0.025) of tricuspid regurgitation obained by cine MRI with echocardiographic data was good. CONCLUSION: Cine MR imaging can provide quantative data estimating for evaluation of tricuspid regurgitation in newborns.

9.A Case of Superficial Basal Cell Epithelioma of the Labium Majus.

A 40-year-old woman presented with a pruritic erythematous eroded patch on the right labium majus which had been for 3 years. Histopathological findings were consistent with superficial basal cell epithelioma. Basal cell epithelioma of the labium majus is a rare neoplasm and its clinical appearance is usually nodular. In contrast to the other types of basal cell epithelioma, the superficial type occurs mainly on the trunk. We report a case of superficial basal cell epithelioma arising from the labium majus.

10.A Clinical Study of Buoy Flap

Buoy flap was useful method for soft tissue defect, bone defect or bone and soft tissue defect. Also the buoy flap could use monitoring for blood flow. The advantages of the buoy flap were long length of perforating cutaneous branch, large diameter of Peroneal vessel, transposition in same leg and the another advantage was that the fibula is the long, straight and mainly cortical bone. To obtain successful result of buoy flap, one must have the anatomic concept for vascular pattern (straight branch, proximal oblique branch, branch from muscular branch, distal oblique branch), relationships with surrounding muscles (between Peroneus m. and Soleus m., Through Peroneus m., through Soleus m.) and length of perforating branch. So, we studied total 19 person, 22 cases of buoy flap, who were admitted and took the operation at Kyung Hee University College hospital, from October 1985 to March 1987.
