Elephantiasis neuromatosa is one of 3 types of cutaneous tumors in Von Recklinghausen's disease. This 28 year-old male patient visited for adult-fist sized tumor associated with hairy pigmented patch on right antecubital fossa, and various sized tumors and cafe-au-lait spots on whole body of about 27 years duration. Past history was noncontributory. Family history revealed that his father and son had similar manifestations. Laboratory findings were noncontributory. Histopathology of cafe-au-lait spot and hairy pigmented patch revealed increase of melain in basal cell layer of the epidermis and increase in the concentratiton and activity of the melanocytes in basal cell layer of the epidermis. Histopathology of the mass revealed slightly thin, wavy fibrs lying in loosely textured strands extending in varying directions and a fairly large number of nuclei that are ovel to spindle-shaped and fairly uniform in size.

3.Successful repair of coracoid and femur fractures in a Eurasian eagle owl (Bubo bubo) and its post-release survival
Journal of Veterinary Science 2023;24(1):e17-
To our knowledge, this is the first case of concurrent diaphyseal left coracoid and left femoral fractures in a Eurasian eagle owl and its post-release survival in Korea. The femur was surgically repaired using an external skeletal fixator-intramedullary (IM) pin tie-in method, and the coracoid was repaired solely with an IM pin on day 6 after femur surgery. The eagle owl underwent a gradual rehabilitation process. The bird was successfully rehabilitated and released 101 d after initial presentation. The bird was monitored using a wildlife tracking device and was confirmed to have survived for over 5 mon in the wild.

Experiences with propofol in pediatric anesthesia are limited. We undertook a study to evaluate the quantity of induction and recovery from anesthesia with propofol compared to thiopental/halothane. Fifty children received 2mg/kg of propofol as a loading dose followed by a maintenance dose of 9mg/kg/hr for first 30minutes and followed by 6mg/kg/hr there- after. Fifty children received 5mg/kg of thiopental, and maintensnce was provided with halothane (0.5%-1.5%). The interval between the end of the administration of propofol or thiopental/halothane and extubation, as well as discharge to the ward, was significantly shorter with propofol (5.43 versua 10.72 min and 10.30 versus 18.20 min, respectively). Pain on injection site were seen more frequently with propofol, whereas nausea and hiccup were only observed with thiopental. During the first 6 hour after the surgical procedure, an- algesics were needed significantly more often in the thiopental group. In conclusion, propofol used as a single anesthetic for total intravenous anesthesia is a satisfactory agent for oph- thalmic surgery of short duration in children.

6.Tympanic Membrane Perforation Due to Metal Spark in a Welder.
Korean Journal of Occupational and Environmental Medicine 1999;11(1):113-118
Injury to the ear in welders is a recognized but poorly documented entity. The risk for ear injury from molten metal or hot sparks produced during welding procedures is low, but the effects can be significant. Burns, tympanic membrane perforations and chonic otorrhoea are the most common results. One case recently experienced at our institution highlight the risk that exists for middle ear injury. The traumatic thermal injury was caused by a metal spark perforating the tympanic membrane during welding. Neither subject was using ear protection at the time. The use of ear protection is strongly advocated during welding operations that place the ear at risk.

Korean Journal of Dermatology 1997;35(3):536-540
Cutaneous angiosarcorriias occur most commonly on the face and scalp of elderly individuals and are characterized by poor prognosis. We present a case of angiosarcoma in a 17-year-old boy not associated with any predisposing factors. The patient had three, ill-defined, 3 to 5 cm sized, erythematous, indurated subcutaneous nodules on the right upper extremity with a duration of 3 months. On histological examination, there were extravasated erythrocytes and irregular vascular channels lined by atypical endothelial cells in the dermis and subcutaneous tissue. The tumor cells appeared to dissect the collagen bundles. The patient was treated by wide surgical excision and post operative radiation therapy. However, 16 months after the operation, local recurrence was found to involve the close areas to the excision site on the right upper extremity and right axillary lymph nodes.

Journal of the Korean Academy of Rehabilitation Medicine 2003;27(1):75-79
OBJECTIVE: To determine the sensitivity of electrodiagnostic parameters in the patients with asymptomatic diabetic neuropahty. METHOD: The subjects were 26 patients with asymptomatic diabetic neuropathy and 40 healthy adults as control group. All subjects underwent electrodiagnostic evaluation of the following motor nerves: median, ulnar, tibial, and peroneal. Sensory nerves included: median, ulnar, radial, superficial peroneal, sural, lateral dorsal cutaneous branch of the sural nerve (LDSN) and medial plantar. And other studies were the sural/radial amplitude ratio, LDSN/sural amplitude ratio, peroneal and tibial F-responses, and H-reflex recorded from the soleus muscle. The frequency of abnormal parameters in the patients with asymptomatic diabetic neuropathy was obtained by comparison with the normative limits obtained from the control group. RESULTS: The most frequent abnormal electrodiagnostic parameters were the LDSN onset latency and the amplitude ratio of LDSN/sural (84.6%, respectively) followed by the LDSN peak latency, LDSN amplitude, and medial plantar onset and peak latency (80.8%, respectively). CONCLUSION: We concluded that the LDSN and medial plantar nerve conduction studies are useful for early detection of neuropathy in diabetes mellitus.

The Journal of the Korean Orthopaedic Association 1984;19(3):569-577
Open fractures of the shaft of the tibia occur frequently because of the progression of industrialization, Different methods of treatment have been advocated as regards the care of the open wound and the method of stabilization of the fracture fragments. The reports, published until recently, have differed with respect to rates of morbidity and end results. With this in mind, we reviewed a consecutive series of 30 open fractures of the tibia at the Department of Orthopedic Surgery of the Eulji General Hospital between 1981 and 1983 by a variety of methods. The following results were obtained. 1. The highest incidence of fractures was encountered in 3rd decades(43.3%) and male to female ratio was 4: 1. 2. The most common cause of fractures was traffic accident(66.6%). 3. The most common anatomical level of fractures was middle third of the shaft(46.7%). 4. The severity of injury was classified according to the Ellis classification as follows. Minor group, 8 cases(40%), Moderate group, 8 cases(26.7%,), Major group, 10 cases(33.3%). 5. The most common associated injury was ipsilateral fibular fracture. 6. Of 30 cases, 11 patients(36.6%) had an open wound more than 5cm in length, 10(33.3%.) between 2.5 and Scm and 4(13.4%) below 2.5cm, and primary closure after thorough debridement and copious irrigation was done in 17 cases(56.7%). 7. Pin and plaster method was applied in 3 cases of minor group and 3 of moderate group, intramedullary nailing was applied in 9 of minor group and 5 of moderate group, and Hoffmann device was applied in 10 of major group. 8. The majority of cases(66.7%) were operated on within 1 week after injury, 9(30%) between 1 and 2 weeks and 1(3.3%) between 2 and 3 weeks. 9. The fractures in middle third of the shaft of the tibia treated with intramedullary nailing revea.ed the shortest healing time(average 13 weeks). 10. Complications such as delayed union, infection and disabled joint shiffness were encountered mainly in cases with severe soft tissue injuries.
