The role of the donor and recipient gender have been a controversial point in the outcome of renal transplantation and the pathophysiologic mechanisms are not understood clearly. In order to evaluate the effect of gender on the renal graft survival, we reviewed our 400 consecutive living donor renal transplantation. The patients were divided into four groups, Group 1; male donor and male recipient(n=152), Group 2; female donor and male recipient(n=61), Group 3; male donor and female recipient(n=135), and Group 4; female donor and female recipient(n=52). To estimate the transplant outcome between the groups, we analyzed patient and graft survival, acute tubular necrosis, BUN, creatinine and rejection episode for maximum 5 years after transplantation. The level of BUN(34.7+/- 26.4, 19.8+/- 6.4, 30.5+/- 14.6, 23.1+/- 10.5 in group 1,2,3,4 respectively) and serum creatinine(2.62+/- 2.3, 1.48+/- 1.1, 2.24+/- 1.2, 1.65+/- 0.9 in group 1,2,3,4 respectively) were higher in male recipient groups regardless of donor gender. However, donor and recipient gender had no influence on post-graft blood pressure and acute tubular necrosis(p>0.05). Acute rejection episode was predominent at female donor graft than male donor graft(36.4% vs 30.1%). The 5 year graft survival in group 1,2,3,4 were 67.8, 67.2, 60.1, 72.7% and patient survival were 76.9, 75.6, 72.6, 80.5% in their orders. These results suggest that donor and recipient gender might play a role in the outcome of renal transplantation. The mechanism of these results must be analyzed by further evaluation using larger patient group.

PURPOSE: To evaluate the results of the compression plate fixation and autogenous bone graft in the management of humerus shaft nonunion. MATERIALS AND METHODS: Eighteen cases were treated for humerus shaft nonunion using compression plate fixation and an autogenous iliac bone graft. The mean follow-up period was 28 months. Bony union was confirmed from the serial radiographs and the clinical outcomes were assessed according to ASES scoring system. RESULTS: In 12 cases of initial plate fixation, the causes of nonunion were 6 cases of inadequate plate length, 2 with a broken plate, 2 with screw loosening, 1 infection and 1 noncompliance of a psychiatric patient. In 3 cases of initial intramedullary fixation, the cause of nonunion was a distraction of the fracture site. In 3 cases of external fixation, the cause of nonunion was inadequate fixation. All cases showed bony union after an average of 24 weeks. The clinical outcomes were 11 excellent, 6 good and 1 fair. CONCLUSION: In the treatment for nonunion, compression plate fixation with autogeneous bone graft after complete removal of the fibrous and necrotic tissue is believed to give satisfactory results

PURPOSE: To determine the natural history and clinical characteristics of keratitis associated with measles which is currently epidemic in Korea. METHODS: Eighty-four hospitalized patients with measles were prospectively examined between December 2000 and March 2001, during their admission period. Ocular and adnexal examination was performed with a slit lamp biomicroscope. Fundus examination and corneal sensitivity test were also performed. RESULTS: The age and sex distributions were not significant. Age of patients ranged from 18 months to 19 years. All patients with measles have conjunctivitis signs, such as follicles and injection. Superficial keratitis was detected in 7 of 84 patients (8.3%). Corneal lesions were bilateral, well demarcated, round, multiple and 0.2 to 0.4 mm in size. The mean duration of keratitis was 10.1+/-2.9 days in this study. All keratitis resolved spontaneously using artificial tears and prophylactic antibiotic eyedrops CONCLUSIONS:Keratitis associated with measles developed frequently (8.3%), so careful ophthalmic examination should be performed in measles.

The objective of this study was to evaluate the effect a chewable tablet containing sodium flouride and lauroyl sodium sulfate on removing plaque and inhibiting gingival inflammation. A randomized parallel study was designed. 100 voluteers participated in the study. There were two test groups each with 30 subject. Test group A was instructed to brush once in the morning, and to use the tablet once in the afternoon and once in the evening. Test group B was instructed to use the tablet three times a day without brushing. There were two control groups each with 20 subjects. Control group A was instructed to brush once in the morning only. Control group B was instructed not to brush all. Two weeks before the test period, the subjects received through tooth cleaning and polishing. At baseline, GI, PI, BOP, and GCF of the Ramfjord teeth were measured in all groups. Bacteria culture was done with the plaque sampled from tooth with the deepest pocket. After 5 days, clinical indices were measured and the bacterial culture was repeated. Control group B was dropped from the study after this period. All the other groups remained and the indices and the culture was repeated after 2 weeks, and 3weeks. Also whether the oral mucosa showed signs of irritation was monitored throughout the test period. Test group A showed less PI, GI, BOP, probing depth, GCF than test group B or control group A. Especially, PI and the BOP was significantly less than that of the group that was instructed to brush once a day. This implies that the added use of this tablet aids in plaque removal in people who brush just once a day. Test group A showed increase of cocci, decrease of motile rods, and decrease of spirochetes after 14-21 days. And this was significantly different from the control group A. At no time of the test period did any of the subjects show signs of irritation of the oral mucosa or adverse reactions. Following conclusions could be obtained from this study. This chewable tablet for enhanced oral hygiene could be used as an adjunct to oral hygiene in people who do not brush adequately. The use of this tablet decreased the number of subgingival bacteria, and this could be effective in plaque removal and for prevention of gingival inflammation.

PURPOSE: We report a 32-year-old man with DeMorsier syndrome, who was referred for seizures, esotropia and visual disturbance of left eye. METHODS: Fundus examination, visual field examination and the visual evoked potential were performed, and other diagnostic studies included endocrine determinations, the electroencephalogram and MR imaging. RESULTS: Ophthalmic examination revealed left optic nerve hypoplasia with the "double ring" sign. Axial MR T1 weighted image showed tiny left optic nerve and absence of the septum pellucidum. Subsequent evaluation revealed temporal visual field defect, abnormal VEP and deuteranomaly of left eye.

PURPOSE: To compare the radiographic results of unicompartmental knee arthroplasty using a navigation system with those using a manual alignment system. MATERIALS AND METHODS: The results of 50 cases (46 patients) of unicompartmental knee arthroplasty using a navigation system (24 patients, 25 cases) and using a manual alignment system (22 patients, 25 cases) were evaluated. Knee anteroposterior, lateral, and weight-bearing full-length lower extremity radiographs before and after the arthroplasty were used for the measurements. RESULTS: The mean postoperative femorotibial alignment was 3.6 degrees of valgus in the manual alignment group and 5.4 degrees of valgus in the navigation group. The postoperative mechanical axis measured using Kennedy and White's method was located at zone 1 in 4 cases (16%), at zone 2 or C in 21 cases (84%) in the manual group, and at zone C for all cases in the navigation group (p=0.001). In the coronal axis of the femoral component, 11 cases (44%) were excellent, 7 cases (28%) were good and, 7 cases (28%) were poor in the manual alignment group. In the navigation group, 15 cases (60%) were excellent and 10 cases (40%) were good and there were no poor results (p<0.05). In the coronal axis of the tibial component, 22 cases (88%) showed excellent results and 3 cases (12%) showed poor results in the manual alignment group. In the navigation group, all cases were showed excellent results (p>0.05). In the sagittal axis of the tibial component, 9 cases (36%) showed poor results in manual group and there were no poor results in the navigation group (p<0.05). CONCLUSION: Unicompartmental knee arthroplasty using navigation produced better results in restoring the alignment of the prosthesis and the mechanical axis of the lower extremity than that using manual alignment.

PURPOSE: To compare the clinical? results and stability of acute and chronic reconstruction of anterior cruciate ligament (ACL). MATERIALS AND METHODS: Patients who had ACL reconstruction with a quadruple hamstring tendon and ligament anchor (LA) screw with a minimum 2-year follow-up were included in this study. Acute (within 4 weeks) reconstruction group was composed of 27 knees and chronic (over 3 months) group was 44 knees. We compared the two groups with regard to Lysholm knee score, range of motion, thigh circumference, Lachman test, Tegner activity scale, associated meniscal injuries, and anterior laxity difference by Telos stress arthrometer. RESULTS: At last follow-up, no significant differences were found between the acute and chronic groups for Lysholm score, range of motion, Lachman test, Tegner activity scale, and instrumental laxity. 16 cases (59%) of acute and 33 cases (75%) of chronic patients had a meniscal injuries of which were reparable in 5 cases (28%) and 7 cases (17%). CONCLUSIONS: Acute reconstruction of ACL had excellent clinical and radiologic results as good as the chronic group without motion problem.
