1.Transcatheter transarterial gelfoam embolization

Transcatheter transarterial embolization is one of main scope of interventional radiology for bleedingcontrol, preoperative and devascularization of unresectable tumor. We authors performed Gelfoam embolization in 10cases of various patients including 3 cases of hypernephroma, 2 cases of renal arteriovenous malformation, 2 casesof unresectrable hepatoma, 2 cases of nasopharyngeal bleeding tumors and one case of bleeding urinary bladdertumor, at Department of Radiology, Korea University Hospital. Gelfoam particles were used as embolic material,which were cut into cubes approximately 1.5x1.5x10 mm3 in size, and total volume of used Gelfoam was 1.5cc to 12ccdepending on the nature of disease and size of the feeding artery. Usual angiographic catheter of 5-7F in diameterwere used. In all 10 cases, the embolization was succesful for bleeding control, preoperative preparation ordevascularization of the unresectable tumor. In four cases, postembolization syndrome was developed ascomplication, which were 4 cases of pain on embolization is very helpful for various diseases, and Gelfoam is oneof the useful and conventient absorable embolic material for transcatheter embolization.

2.An experimental study on radioprotective effect of 5-thio-D-glucose

To evaluate the radioprotective effects of 5-thio-D-glucose(5-TDG) on normal skin, an experimental study was carried out with total 140 mice, of which 30 mice were given 5-TDG followed by irradiation and 60 mice were given intraperitoneally (IP) two hours before irradiation, and the radiation doses were singly 2500, 3500 or 4500 radsrespectively. The skin changes of the irradiated right hind limb were inspected for 31 days and the skin scores were analized. The results are as follow; 1. Marked radioprotective effect was presented in the group of 5-TDG,1.5g/kg body weight, with 2500 rads irradiation. 2. Definite radioprotective effect was also revealed in the groupof 5-TDG, 1.5kg/ body weight, with 3500 rads irradiation, In the group of double dose 5-TDG, 3.0g/kg body weight, the radioprotective effect appeared to be enhanced. 3. In the group of 4500 rads irradiation with 5-TDG, 1.5g/kgbody weight, no radioprotective effect was noticed. 4. In addition to the radiosensitization effect on the hypoxiccells, the radioprotective effect of 5-TDG on the normal animal tissue suggests increased possiblility of itsclinical application.

3.Bronchioloalveolar carcinoma: a variety of radiographic patterns.

Bronchioloalveolar carcinoma may present with a variety of radiographic and clinical appearances. The authors reviewed the radiographs of 17 patients with pathologically proven bronchioloalveolar carcinoma, retrospectively. Seven cases were of the localized form. Among the seven five showed a localized mass and two showed localized consolidation. Ten cases were of the diffuse form. Among the ten, three showed diffuse nodules, two showed diffuse consolidation, and five showed a combined pattern of nodules and consolidations. Progression of disease was observed in 9 patients. In these cases, localized forms progressed to a diffuse forms and nodular patterns progressed to a consolidation pattern. Seven cases of the diffuse form were given antituberculous medications on hospital admission. In summary, bronchioloalveolar carcinoma has varied radiographic patterns, and often mimicks other pulmonary diseases such as pulmonary tuberculosis.

4.Preimplantation Development and Apoptosis of Mouse Embryos in the Medium Containing Extracellualr Matrix.

OBJECTIVE: To verify the effect of Matrigel, a ECM complex from Engelbreth-Holm-Swarm(EHS) mouse sarcoma on the preimplantation development and apoptosis of mouse fertilized eggs. METHOD: Late Pronucleus stage eggs were cultured through the blastocyst stage in the presence of Matrigel (0.5%, v/v). Characteristics of apoptosis and cell number assessed by Hoecst staining and TUNEL labeling at the blastocyst stage, respectively. RESULTS: Morphological development, number of cells per embryo was significantly increased but rate and number of TUNEL positive nuclei of the embryo were decreased in the presence of Matrigel. CONCLUSION: This result suggested that at low concentration of Matrigel improves both viability and morphological development in the preimplantation mouse embryos.

5.Differentiation of amebic versus pyogenic liver abscess with US and CT.

To differentiate amebic and pyogenic liver abscesses, sonography(US) and CT findings of 21 amebic and 22 pyogenic liver abscesses were reviewed retrospectively. US was performed in all cases and CT in 15 cases of each group. Eleven cases of amebic abscesses had well defined margin which was observed in 10 cases of pyogenic abscessed. Amebic abscesses showed hyperechoic wall in 7 cases and calcification in 4 cases. Ten cases of pyogenic abscesses had mearby intrahepatic duct dilatation which was observed in 2 cases of amebic abscesses. Pneumobilia was seen in 6 cases of pyogenic abscesses. US findings of clear margin and hyperechoic wall, and CT findings of intrahepatic duct dilatation, pneumobilia, and calcification were helpful in differentation of amebic pyogenic liver abscesses.

6.Ureteral fibrous polyp: report of 2 cases.

Two cases of ureteral fibrous polyp showing serpiginous filling defect on IVP are presented with characteristic radiologic features; easy flow of contrast medium around polyp. Prolapsed polyp in urinary bladder, and less ureteral obstruction or renal damage than in epithelial lesions. These rediologic findings enable to differentiation of ureteral fibrous polyps from malignant tumor, which is helpful for determining therapeutic approach.

7.Pattern of Distant Lymph Node Metastasis in Colorectal Carcinoma and its Correlation with Distant Organ Metastasis: CT Evaluation.

PURPOSE: To evaluate the pattern of distant lymph node metastasis in colorectal carcinoma and its correlation with distant organ metastasis. MATERIALS AND METHODS: We retrospectively reviewed abdominal CT scans of 46 patients with pathologically proven colorectal carcinoma. RESULTS: The incidence of distant lymphadenopathy in colorectal carcinoma was 30.4%(14/46). The most commonly involved distant lymph node was the left paraortic lymph node below the renal hilum(9/25). The most common type of distant lymphadenopathy was solitary type(7/14) and all of these lymphadenopathies were noted in the left paraortic lymph node below the renal hilum. Six cases of left sided colorectal carcinoma showed left paraortic lymphadenopathy with solitary type. The incidence of distant organ metastasis was 17. 4%(8/46) and markedly increased if distant lymphadenopathy was multiple and confluent, or confluent type(5/7). CONCLUSION: The incidence of distant lymphadenopathy in colorectal carcinoma was not high and the most common lymphadenopathy was the left paraaortic lymph node with solitary type below the renal hilum. The possibility of distant organ metastasis was high if distant lymphadenopathy was multiple and confluent, or confluent type.

8.Transcatheter arterial chemoembolization of hepatocellular carcinoma with portal vein invasion.

Transcatheter arterial chemoembolization(TACE) is an imperative method for the managment of inoperable hepatocellular carcinoma(HCC). It is well known that primary HCC frequently invades the portal venous system and forms a tumor thrombus obstructing the portal blood flow which makes unfavorable prognosis of patiebt. We retrospetively reviewed 58 patients who reveived TACE(minimum 3 times) of HCC invading into portal venous system. Group 1(n=29) which showed peripheral portal vein invasion had better clinical and laboratory response. Group 2(n=17) which showed first order portal branch invasion had similar response to Group 3(n=12), which had main portal invasion. Group 1 showed no difference in survival time between TAC and TACE, but, in Group 2 and 3, emboliation with chemotherapy made longer survival than chemotherapy only. Clinical level of AFP was meaningful in Group 1 and 2 as decreasing value. Our results provides that careful selection of TACE and case by case Coil/Gelfoam embolization can improve the mean survival and clinical response when HCC evidently invades portal venous system.

10.High-Resolution CT Findings in Swyer-James Syndrome.

PURPOSE: The purpose of this study was to evaluate the high-resolution CT features of Swyer-James syndrome with special attention to the airway lesions and to determine the clinical utility of high-resolution CT compared with that of chest radiography. MATERIALS AND METHODS: In seven patients with Swyer-James syndrome, we retrospectively reviewed chest radiographs obtained during inspiration and expiration and high-resolution CT scans obtained in inspiration. The high-resolution CT appearance was evaluated and compared with that of chest radiography. RESULTS: On both chest radiographs and high-resolution CT, the affected lung volume was relatively diminished in four patients and normal in three patients. In all seven patients, chest readiographs showed hyperlucency of the lung which was unilateral in four and bilateral in three patients. Unilateral small hilum was seen in six patients and bronchiectasis was demonstrated in one patient on chest radiographs. The hyperlucent lung volume was not diminished on expiratory radiographs in all seven patients. In all patients, high-resolution CT demonstrated low attenuation regions of the lung either bilaterally(n=5) or unilaterally(n=2). Pulmonary vessels were markedly decreased in size and number in the lung parenchyma with low attenuation. Six patients had bronchiectasis on high-resolution CT, which were cylindrical or varicose in five and cystic in one. Bronchiolectasis was observed in three patients on high-resolution CT. CONCLUSION: The high-resolution CT findings are characteristic of Swyer-James syndrome. High-resolution CT is more sensitive than chest radiography in detecting regions of low attenuation and bronchiectasis and may be useful for the diagnosis of Swyer-James syndrome. Our results suggest that bronchiectasis is a frequently associated airway lesion of Swyer-James syndrome and bronchiolectasis may be associated in some cases.
