3.Foramen of Morgagni Hernia in Infant.

Morgagni hernia is an uncommon finding in adult and rarer still in infants and children, adn they say that Morgagni hernia occur about one in every 300 diaphragmatic hernias diagnosed. This paper presents a 16 months old Korean male infant with symptomatic Morgagni's hernia in whom the diagnosis was established during the course of treatment of upper respiratory tract infection and who were operated successfully. This well nourished patient was admitted with the chief complaints of mid dyspnea, coughing and restlessness for 2 days. During treatment and investigation we detected a peristaltic sound in the right side lung field and confirmed it as Morgagni hernia by chest X-ray and barium swallowing. We performed corrective surgery through a midline supra-umbilical incision. As opened the peritoneum, transverse colon was gushed out, which suspected as herniated intestinal loop. After well mobilization of visible bowel, hernia sac was notified from the anterior portion or right crus of diaphragm. The hernia was reduced very easily. The patient made an uninterrupted recovery and on the 8th post-op. day, he went to home with symptom-free state. We made a brief literature review.

5.Factors Influencing Smartphone Addiction in Adolescents.

PURPOSE: The purpose of this study was to verify the relationship among depression, school adjustment, parent-child bonding, parental control and smartphone addiction, and to identify factors which influence smartphone addiction in adolescents. METHODS: A cross-sectional design was used, with a convenience sample of 183 middle school students from 3 middle schools. Data collection was conducted through self-report questionnaires from April to May, 2017. Data were analyzed using χ² test, Fisher's exact test, t-test, one-way ANOVA, correlation coefficient analysis, and binary logistic regression with SPSS Ver. 21.0. RESULTS: The mean score for smartphone addiction was 29.40. Of the adolescents, 21.3% were in the smartphone addiction risk group. Logistic regression analysis showed that gender (OR=7.09, 95% Cl: 2.57~19.52), school life (OR=0.86, 95% Cl: 0.79~0.93), smartphone usage time (OR=1.32, 95% Cl: 1.04~1.66), and parental control (OR=4.70, 95% Cl: 1.04~21.29) were effect factors for the smartphone addiction risk group. CONCLUSION: Findings indicate that school satisfaction was an important factor in adolescents' smartphone addiction. Control oriented parent management of adolescents' smartphone use did not reduce the risk of smartphone addiction and may have worsen the addiction. Future research is needed to improve understanding of how teachers and parents will manage their adolescents' use of smartphones.

10.Hypermetabolism of Compensatory Laryngeal Muscles in Unilateral Vocal Cord Palsy: Comparison Study between Speech and Silence with Normal Subjects by Co-registered PET-CT Fusion Images.

PURPOSE: There are a few case reports on asymmetric vocal cord uptake on FDG-PET in patients with unilateral vocal cord paralysis, which could be a potential pitfall in the interpretation of FDG-PET images. We evaluated the metabolic activity of laryngeal muscles of patients with unilateral vocal cord paralysis in comparison to normal controls during both speech and silence. METHODS: Eleven patients with unilateral vocal cord palsy (thyroidectomy=7, lung cancer=1, others=3) and 12 normal controls underwent FDG-PET with usual protocol. They were divided into two groups respectively; one group read books aloud for 20 minutes (phonation group) and the other kept silence (non-phonation groups) after FDG injection. Recent neck CT scan were co-registered with FDG-PET to produce PET-CT fusion images to elaborate small laryngeal muscles. RESULTS: In patients with unilateral vocal cord palsy, contralateral non-paralyzed vocal cord showed hypermetabolism mainly on thyroarytenoid muscle, more intensely with phonation group (SUV=5.88+/-2.65) than with non-phonation group (SUV=2.30+/-0.39). Normal control subjects showed hypermetabolism (3.68+/-0.96) in interarytenoid muscle and symmetric mild hypermetabolism in both lateral cricoarytenoid muscles in only phonation group. CONCLUSION: FDG-PET with fusion images using CT scan in patients with unilateral vocal cord paralysis showed hypermetabolism of contralateral non-paralyzed thyroarytenoid muscle, suggesting compensatory action during phonation. Phonation during FDG-PET study enhanced FDG uptake on different laryngeal muscles between patients with unilateral vocal cord paralysis and normal subjects.
