2.A Case of Renovascular Hypertension Cured by Percutaneous Transluminal Angioplasty.

Pertutaueous transluminal angioplasty is the treatment of choice for renovascular hypertension in cases due to the renal vascular occlusion which is not severe enough to pass dilatation catheter. The advantages of percutaneous transluminal angioplasty are the use of local anesthesia, its noninvasiveness without surgical risk, feasibility of repetition and shorter hospital days compared with surgical methods. We are presenting a case of renovascular hypertension due to atherosclerosis cured by percutaneous transluminal angioplasty with the review of literatures.

3.A Case of Retrocaval Ureter.

Retrocaval or postcaval ureter is a well-recognized venous congenital anomaly commonly causing ureteric obstruction. A 42 year-old male was admitted to Korea University Hospital because of right flank pain. The right AGP reveals the S-shaped coures of ureter and hydronephrosis. The ureteral and to end anastomosis was done with double J ureteral stent. Herein we present a case of retrocaval ureter with brief review of literatures.

4.Comparison of Clinical Results between 2.2 mm and 2.8 mm Incision Cataract Surgery Using Ellips Ultrasound.

PURPOSE: Introduction of phacoemulsification and development of foldable artificial lens has facilitated smaller incisions, even micro-coaxial incisions. However, there have been several studies showing that micro-coaxial incision has no benefit compared with the conventional small incision method. Cases where Ellips ultrasound was used have not yet been reported. Therefore, we compared the postoperative results between 2.2-mm and 2.8-mm incision groups using Ellips ultrasound. METHODS: Among 49 eyes receiving cataract surgery from March, 2012 to August, 2012, 27 eyes in the 2.2-mm group and 22 eyes in the 2.8-mm group were examined to obtain cumulated dissipated energy (CDE), use of balanced salt solution (BSS), best-corrected visual acuity (BCVA), corneal endothelial cell count (ECC), corneal thickness at center and incision site, and keratometric astigmatism before and after surgery. RESULTS: There were no statistically significant differences between the 2.2-mm and 2.8-mm groups in CDE (2.5 +/- 2.0 vs. 2.5 +/- 2.3) and use of BSS (188 +/- 127 vs. 138 +/- 43 mL) during the surgery, BCVA (-0.45 +/- 0.62 vs. -0.55 +/- 0.79 log MAR), ECC (-178 +/- 210 vs. -99 +/- 114 cells/mm2), corneal thickness at center (23 +/- 23 vs. 27 +/- 23 microm) and incision site (24 +/- 19 vs. 27 +/- 19 microm) and keratometric astigmatism before and after the surgery. CONCLUSIONS: A 2.2-mm micro-coaxial incision using Ellips ultrasound showed no statistically significant differences in BCVA, ECC, corneal thickness at center and incision site, and keratometric astigmatism compared with 2.8-mm small incision.

5.A Case of Priapism.

Priapism is a relatively rare condition consisted of persistent painful erection, unrelated to sexual activity. The management is often delayed or ineffective, resulting in the development erectile bodies into impotence. The most common form in adult is idiopathic but in the childfood sickle cell disease is more common. Initial therapy consist of Analgesic, Antibiotic, Sedatives, Ice water enema, and Anticoagulant irrigation of corpus cavernosum. We used Hemo-Vac drainage which was continuous drainage from corpus cavernosum and obtained a good results.

A case of ovarian dermoid cyst invading the posterior bladder wall with pilimiction and recurrent bladder stone was presented. Fifty-four years old female patient had a symptoms of frequency, dysuria, hematuria and pilimiction intermittently She had received cystolithotomies and litholapaxy due to recurrent bladder stones for 3 times in other hospitals Cystoscopic examination revealed elevated flat mass on the base of bladder and hairs outgrowing a from the surface of mass. Also two-large bladder stones were seen at cystoscopy. So, recurrent bladder stones caused by the hairs growing from invading dermoid cyst was diagnosed. Removal of bladder stone, partial cystectomy and left oophorectomy were performed.

9.Two Cases of Primary Signet-Ring-Cell Adenocarcinoma of the Urinary Bladder.

Primary Signet-ring-cell adenocarcinoma of the urinary bladder is an extremely rare variant that promotes considerable interest, 21 cases reported as of 1981. Morphologically, this tumor is identical to signet-ring-cell carcinomas of the gastrointestinal tract, breast, lung and nasal cavity, and differentiation must be carried out with great care. Characteristically, primary signet-ring-cell adenocarcinoma carries a poor prognosis with early penetration of the mucosa, submucosa, and muscularis. This tumor is sessile, infiltrates early, extends locally to adjacent organs, and metastasized late. Many patients reported died of local extension with ureteral obstruction and uremia rather than wide spread metastasis. We are reporting two cases of primary signet-ring-cell adenocarcinoma of the urinary bladder treated with radical cystectomy and transurethral resection with radiotherapy, which died of local extension with ureteral obstruction and uremia, with pathological findings and review of the literatures.

10.Combined Mitomycin-C Assisted Photorefractive Keratectomy in the Treatment of Epithelial Ingrowth after LASIK.

PURPOSE: To report 2 cases of thick, late-onset and long-standing epithelial ingrowth after laser-assisted in situ keratomilusis (LASIK) using mitomycin C-assisted photorefractive keratectomy and flap suturing. CASE SUMMARY: The first case involved a 29-year-old woman who had LASIK in both eyes in 2002. She was diagnosed with epithelial ingrowth OD after blunt ocular trauma in 2007. The second case involved a 30-year-old man who had LASIK in both eyes in 1999 followed by enhancements in 2004. Slit lamp examination showed dense epithelial ingrowth OS in a geographic pattern. Two eyes of the 2 patients with epithelial ingrowth had flap lifting followed by debridement of interface epithelial cells, photorefractive keratectomy, interface application of mitomycin C, and the placement of interrupted 10-0 nylon sutures. Two eyes regained better uncorrected visual acuity and showed reduced corneal astigmatism postoperatively. There was no evidence of residual or recurrent ingrowth throughout the follow-up period (case 1: 11 months, case 2: 1 month). CONCLUSIONS: Combined mitomycin C-assisted photorefractive keratectomy and flap suturing can be an effective and safe method for treating and preventing the recurrence of thick, late-onset and long-standing epithelial ingrowth involving the visual axis.
