1.The Correlation Between the Smoking and Oxygen Free Radicals in Men.

BACKGROUND: This study aimed to empirically investigate the determinants on oxygen free radical degree which causes biological damage focusing on smoking habit and health behaviors in daily life. METHODS: In this cross-sectional study, the subjects were adult males who submitted free radical tests among the examinees of the comprehensive medical testings at an university hospital in Suwon from 2008 to 2010. Total 497 samples were used in analyses for clarifying the relations of free radical degree to age, smoking, obesity, and other health behaviors. RESULTS: First, the degree of oxygen free radical increases by 0.164 as the age group increases (P<0.05). Second, the degree of oxygen free radical increases by 0.547 as the smoking habit increases (P<0.001). Third, the degree of oxygen free radical increases by 0.087 as the exposure to hazardous factors in workplace increases (P<0.001). CONCLUSIONS: The findings of the study imply that the smoking habit has more great influence on the increase of oxygen free radical in body than any other health behaviors. The causality of free radical to smoking and health behavior was not identified due to the limit of the cross-sectional study so it needs to be studied prospectively in the future.

Congenital bronchoesophageal fistula (BEF), congenital communication between esophagus and bronchus, is a rare malformation. Benign BEF shows only nonspecific symptoms that may delay a correct diagnosis and proper treatment. This is a case of a 3-year-old boy presenting congenital BEF with imperforate anus and atrial septal defect. He had recurrent pneumonia in the right lower lobe since he was 5 months old. A barium swallow examination showed a communicating fistula between the lower esophagus and the right lower lobe bronchus. Computed tomography scan supported to delineate the course of the fistula. He underwent resection surgery of the fistula and lobectomy of the right lower lobe. He recovered without any postoperative complications.

3.Analysis of the Macronutrient Composition of Breast Milk from Korean Women and Growth of Infants.

PURPOSE: The aim of this study was to determine the effect of the macronutrient composition of breast milk from Korean women on the growth of infants. METHODS: 173 healthy lactating women and breast-fed infants who visited Gangnam Severance Hospital and two breast-feeding centers in Seoul from October 2011 to March 2012 were recruited. We checked the birth weight and body weight of infants while collecting breast milk from the mothers, and analyzed the macronutrient component of breast milk with a mid-infrared milk analyzer (MIRIS(R) Human Milk Analyzer, HMA, Miris AB, Uppsala, Sweden). Group analysis was performed depending on more or less than 5 percentile and 25 percentile of body weight. RESULTS: The amount of daily intake/RDA for calories, proteins, lipids and carbohydrates of breast milk were significantly lower in the less than 5 percentile and 25 percentile group (P<0.05). Using multiple logistic regression analysis, the significant nutrient component that was insufficient in the less than 5 percentile and less than 25 percentile of body weight group respectively was carbohydrates (P<0.05). CONCLUSION: We conclude that each macronutrient level of breast milk is statistically low in infants with less body weight. These results suggest that nutritional consideration of breast milk is necessary for the growth of breast-fed infants.

Metachromatic leukodystrophy (MLD) is the rare neurometabolic disease caused by the deficiency of the enzyme arylsulfatase A resulting in a deficiency of sulfatide degradation and the target gene is ARSA gene. We report a case of the late infantile form of MLD that was confirmed by means of enzyme assay and gene analysis with typical brain MRI and MR spectroscopy finding.

5.A case report of Extragonadal endodermal sinus tumor occurring in the appendix and mesentery.

Endodermal sinus tumor (EST) is a rare malignant germ cell tumor arising usually in the gonads such as the testis or ovary. However, 10-15% of EST cases may arise in the extragonadal sites, such as sacrocoxygeal area, mediastinum, vagina, brain and retroperitoneum in a frequent occurring order. To our knowledge there are only 2 case reports of extragonadal EST, one originating from the omentum in 1998, and another which was localized in the pelvis in 2003. We report one case of extragonadal sinus tumor which arose in appendix and mesentery in the 19-year-old female with a review of the literature.

6.Multidrug-resistant Salmonella typhimurium and Salmonella enteritidis identified by multiplex PCR from animals.

Antibiotic resistance in Salmonella enteritidis and S. typhimurium, one of most frequent etiologic pathogens of food-borne bacterial gastroenteritidis in humans, is a serious health problem worldwide. Fifteen and 22 each of S. enteritidis and S. typhimurium were isolated from animals from 1983 to 1999 in Korea and tested for their antibiotic resistance patterns and phage types. S. enteritides isolates were highly resistant to sulfonamides (86.7%) and four of them (26.6%) showed multiple antibiotic resistance. The most frequent phage type (PT) of S. enteritids was PT1 (33.3%) even though none of them had multiple antibiotic resistance. S. typhimurium isolates were highly resistant to streptomycin, sulfonamides, and tetracycline, 100%, 95.5%, and 86.4% respectively. The incidence of multiple antibiotic resistance of S. typhimurium isolates was extremely high (100%) comparing to S. enteritidis isolates (26.7%). Two of the five ACSSuT type S. typhimurium isolates, resistant to ampicillin, chloramphenicol, streptomycin, sulfonamides, and tetracycline, were phage type DT104. All S. typhimurium isolates were sensitive to florfenicol. For the rapid detection of multiple antibiotic resistant S. enteritidis and S. typhimurium isolates, particularly ACSSuT type S. typhimurium DT104, antibiotic resistance genes, cmlA/tetR, PSE-1, and TEM, and Salmonella spp. Specific gene, SipB/C, were amplified using four pairs of primers in hot-started multiplex polymerase chain reaction. Two Korean isolates of S. typhimurium DT104 showed TEM amplicons instead of PSE-1 for the ampicillin resistance. The multiplex PCR used in this study was useful in rapid detection of ACSSuT type S. typhimurium and identification of b-lactamase gene distribution among Salmonella isolates.

7.Knowledge, Perception & Practice of Eye Care Nursing among Nurses in Intensive Care Units.

PURPOSE: The purpose of this study was to identify levels of knowledge, perception, and practice of eye care as part of nursing care in the ICU. METHOD: A descriptive, cross-sectional study design was used. The participants in this study were 269 registered nurses working in the ICUs of five university hospitals in Seoul, Korea. Between November 2005 and December 2005, data were collected using a semi-structured questionnaire developed by the researchers, and analyzed using the SPSS Program. RESULTS: In general, levels of knowledge of nursing care for the eyes were moderate among ICU nurses, but levels of perception of nursing care for the eyes were very high. Of the respondents, 61.7% reported that they would consult a doctor whenever they found an eye problem. Only 42.4% nurses answered that they provide eye care as part of the daily routine and 43.9% nurses responded that they cleanse the eye lids with wet saline gauze. In regression analysis, the practice of eye care as part of nursing was significantly influenced by perception of nursing care for the eyes, and knowledge of interventions for nursing care for the eyes. CONCLUSION: These results suggest that education on care of the eyes for ICU nurses and the development of a standardized eye care protocol should be done to improve quality of nursing care in the ICU.
