1.Psychiatric and Psychological Characteristics of Sexual Offenders in Korea.

OBJECTIVES: In an effort to increase understanding of the relationship between sexual violence and mental illness, we conducted an investigation of the prevalence of psychiatric illness and associated psychological features of sexual offenders. METHODS: Twenty-two male sex offenders were referred for psychiatric diagnosis from the prosecutor's office and admitted to the hospital between August 2010 and May 2011. In order to identify Axis I and II disorders, including sexual disorders, we conducted the Korean version of the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorder, Fourth Edition. All participants completed psychological assessment batteries, including the Korean-Wechsler Adult Intelligence Scale, and self-report scales, including the Barratt Impulsiveness Scale, State-Trait Anger Expression Inventory, and the Rape Myth Acceptance Scale. Information on participants' legal, personal, and psychiatric family histories was also gathered. RESULTS: Many of the participants received diagnosis of Axis I disorders : Eight subjects (36.4%) had paraphilia, and five (22.7%) had pedophilia. Two subjects (9.1%) had schizophrenia, five (22.7%) had depressive disorder, and three (13.6%) had alcohol dependence. Participants also showed high rates of Axis II disorders : four of them (18.2%) had mental retardation or borderline intellectual functioning and seven (31.8%) had cluster B personality disorders. Participants' serum free testosterone level showed statistically significant correlation with motor impulsiveness (p<0.05). CONCLUSION: Sex offenders included in this study displayed high rates of mental illness, including paraphilia and intellectual disability. On the basis of our findings, we asserted that sex offenders should undergo careful evaluation for mental illness, and that management programs for sex offenders should include psychiatric evaluation and intervention.

3.Antibody response to pneumococcal vaccination in children with chronic or recurrent rhinosinusitis.

PURPOSE: Although chronic and recurrent rhinosinusitis is prevalent in children, little is known about its causes. Here, we investigated the humoral immunity in children with chronic or recurrent rhinosinusitis. METHODS: We examined 16 children attending the outpatient clinic at the CHA Bundang Medical Center including 11 boys and 5 girls, aged 3.11 years (mean age, 5.6 years), who had rhinosinusitis for >3 months or >3 times per year. The complete blood count with differential and total serum concentrations of Immunoglobulin (Ig) E, IgA, IgD, IgM, IgG, and IgG subclasses (IgG1, IgG2, IgG3, and IgG4) of all children were measured. All subjects received 23-polysaccharide pneumococcal vaccination (PPV), and the levels of antibodies to 5 serologic types (4, 6B, 14, 18C, and 23F) of pneumococcal capsular polysaccharide antigens were measured before and after vaccination. Post-PPV antibody titers > or =0.35 microg/mL or with a > or =4-fold increase were considered as positive responses. RESULTS: The titers of IgG, IgA, IgD, and IgM were within normal range in all 16 children, whereas the total IgE concentration was higher than normal in 2 children. IgG1 deficiency was observed in 1 patient and IgG3 deficiency in 3. After PPV, 1 patient failed to respond to all 5 serologic types, 2 failed to respond to 4 serologic types, and 2 failed to respond to 3 serologic types. CONCLUSION: Clinicians should consider the evaluation of humoral immune functions in children with chronic or recurrent rhinosinusitis who do not respond to prolonged antibiotic treatment.

BACKGROUND: From May to July 2015, the Republic of Korea experienced the largest outbreak of Middle East respiratory syndrome (MERS) outside the Arabian Peninsula. A total of 186 patients, including 36 deaths, had been diagnosed with MERS-coronavirus (MERS-CoV) infection as of September 30th, 2015. MATERIALS AND METHODS: We obtained information of patients who were confirmed to have MERS-CoV infection. MERS-CoV infection was diagnosed using real-time reverse-transcriptase polymerase chain reaction assay. RESULTS: The median age of the patients was 55 years (range, 16 to 86). A total of 55.4% of the patients had one or more coexisting medical conditions. The most common symptom was fever (95.2%). At admission, leukopenia (42.6%), thrombocytopenia (46.6%), and elevation of aspartate aminotransferase (42.7%) were observed. Pneumonia was detected in 68.3% of patients at admission and developed in 80.8% during the disease course. Antiviral agents were used for 74.7% of patients. Mechanical ventilation, extracorporeal membrane oxygenation, and convalescent serum were employed for 24.5%, 7.1%, and 3.8% of patients, respectively. Older age, presence of coexisting medical conditions including diabetes or chronic lung disease, presence of dyspnea, hypotension, and leukocytosis at admission, and the use of mechanical ventilation were revealed to be independent predictors of death. CONCLUSION: The clinical features of MERS-CoV infection in the Republic of Korea were similar to those of previous outbreaks in the Middle East. However, the overall mortality rate (20.4%) was lower than that in previous reports. Enhanced surveillance and active management of patients during the outbreak may have resulted in improved outcomes.

5.Dynamic postural stability in patients with diabetic peripheral neuropathy and relationship to presence of autonomic neuropathy

Diabetic neuropathy is a common complication that can negatively influence balance and is a major
cause of falls. We evaluated the association between postural sway and diabetic autonomic neuropathy
(DAN) among patients with diabetic peripheral neuropathy (DPN). Patients with DPN documented by
typical symptoms with abnormal results of nerve conduction study were included and postural stability
was assessed using dynamic posturography. Composite autonomic scoring scale (CASS) score was
calculated by evaluating sudomotor, cardiovagal, and adrenergic functions. CASS score ≥2 indicated
DAN and the severity of DAN was indicated by the CASS scores divided into three subscales of
10-point total CASS: none or mild autonomic failure (0-3), moderate failure (4-6), and severe failure
(7-10). A total of 34 patients comprised the DAN group (n=19) and non-DAN group (n=15). Patients
with DAN had higher prevalence of diabetic retinopathy(p=0.011), higher urine albumin-creatinine
ratio (p = 0.009), and lower HbA1c levels (p<0.001) than those with non-DAN. With regard to dynamic
postural instability, the presence (p=0.025) as well as the severity of DAN (p<0.05) was associated
with postural instability in the eyes-opencondition. Interestingly, the poorer dynamic postural instability
in moderate/severe DAN compared to mild DAN was observed only in medio-lateral direction and
this association remained significant after adjusting for age, sex, and glycemic control state (HbA1c).
In patients with DAN accompanied by DPN, the dynamic postural instability was affected by visual
feedback and medio-lateral directional instability was closely associated with the severity of DAN.
