2.Gastric Pseudotumoral Lesion Caused by a Fish Bone Mimicking a Gastric Submucosal Tumor.

Gastric complications following unintentional foreign body ingestion are extremely rare. Here, we report the case of a 59-year-old healthy woman who presented with nonspecific abdominal pain and an apparent gastric submucosal tumor that was incidentally detected by gastrofiberscopy. The patient underwent laparoscopic surgery, which revealed an intact gastric wall with no tumor invasion, deformity, or evidence of a gastric submucosal lesion. However, an impacted fish bone was found.

3.The clinical relevance of nm23 protein expression in resected gastric cancer patient.

The aim of present study was to elucidate whether the expression of nm23 protein might be of clinical value as prognostic factor in gastric cancer. The expression of nm23 protein was analyzed using immunohistochemical method in formalin-fixed and paraffin-embedded tissue samples of 76 gastric carcinoma patients. The cytoplasmic immunoreactivity of nm23 protein were detected in 53.9%(41/76). When the immunoreactivity of nm23 protein with TNM status and other histopathologic findings were compared by using Chi-Square test, nm23 was found to have correlations with the lymph node metastasis(p=0.04), the number of metastatic lymph node, and the invasion of lymphatic vessels(p=0.007). But, it has no correlation with TNM status. The conventional prognostic factors such as the depth of invasion, lymph node metastasis, distant metastasis, Borrmann type, size of tumor, and the curability of operation was found to have strong correlation with the survival time(p<0.003). But, the expression of nm23 protein was not significantly correlated with that in survival analysis. These results showed that the expression of nm23 protein is not a useful prognostic indicator in gastric cancer.

4.Effect of Repeated Monothermal Caloric Stimulation on the Bithermal Caloric Response.

We investigated normal response of bithermal caloric stimulation after repeated monothermal caloric stimulation. 12 subjects were selected for cold(30degreesC) water stimulation. 8 subjects were stimulated by warm(44degreesC) water. After the bithermal caloric test, caloric stimuli(cold or warm) was repeated 5 times on both ear with a 10 minute time interval and the next bithermal caloric test was performed. Monothermal caloric stimuli was done at the same temperature as the previous test a day later. The bithermal caloric test was repeated after 4 days. We checked the maximum slow phase eye velocity(SPEV) and calculated the canal paresis and directional preponderance. The SEPV was changed from 26.4(+/-10.9) degrees/sec to 26.7(+/-20.5) degrees/sec in repeated warm stimulation; from 25.0(+/-11.4) degrees/sec to 27.3(+/-11.1) degrees/sec in cold stimulation. Canal paresis was changed from 12.4(+/-8.7)% to 6.3(+/-5.1)% in repeated warm water stimulation; from 9.6(+/-9.9)% to 6.9(+/-3.1)% in cold stimuli. Directional preponderance was changed from 11.9(+/-6.9)% to 8.7(+/-9.9)% in warm stimuli; from 11.5(+/-7.6)% to 10.6(+/-5.5)% in cold stimuli. There was no significant change of caloric response after repeated monothermal caloric stimulation.

5.The Changes of Na+-K+ ATPase Activity and Ultrastructure of Endolymphatic Secretory Epithelium by Local and Systemic Streptomycin Treatment.

The endolymphatic secretory epithelium are stria vascularis in cochlear and dark cell in vestibule which are regulated by Na+-K+ ATPase. It is important that we study intracytoplasmic Na+-K+ ATPase for the physiologic research of inner ear. Recently cerium-based method for stain of Na+-K+ ATPase was developed. This study was underkaken to investigate the morphologic changes and Na+-K+ ATPase activity in stria vascularis and vestibular dark cell of mongolian gerbil after systemic intramuscular injection(200mg/kg or 300mg/kg) for 7days or local infiltration of streptomycin through round window. The results are as follows. 1) The strong Na+-K+ ATPase activity was seen at basolateral infoldings of marginal cell in stria vascularis but weak Na+-K+ ATPase activity in dark cell near transitional area. 2) There was no change of Na+-K+ ATPase activity in the stria vascularis and dark cell by systemic injection of streptomycin. The decrease of Na+-K+ ATPase activity in stria vascularis was seen at destruction site of infoldings by local infiltration of streptomycin but no changes in dark cell. 3) The ultrastructural changes of marginal cell by local infiltration of streptomycin were intracytoplasmic vacuole, partial loss of cytoplasmic infoldings, edema, and increase of melanin particle. but, there was no change of ultrastructure in dark cell except increase of melanin particle. The changes of ultrastructure of stria vascularis was variable by systemic streptomycin injection and there was no dark cell change except increased melanin particle. From the above results, the changes of ultrastructure and Na+-K+ ATPase were more severe by local infiltration of streptomycin through round window than systemic injection of streptomycin. The local infiltration of streptomycin through round window may be suitable method for the induction of inner ear damage.

7.The Comparison of Survival Rates of Postoperative Adjuvant Chemotherapies in The Stage III Gastric Cancer Patients.

PURPOSE: Various postoperative adjuvant chemotherapy regimens have been proposed for the patients with advanced gastric cancer. The majority of clinical trials have shown no significant difference in the survival benefit. The aim of this study was to compare the survival rates of postoperative adjuvant chemotherapies used in stage III gastric cancer patients who received curative gastrectomy. MATERIALS AND METHODS: Between 1990 and 1999, a survival analysis was performed in 260 patients who received curative gastric resection and postoperative adjuvant chemotherapy. The patients were divided into four groups according to the chemotherapeutic regimens received. The groups were: the F group: furtulon alone, FM group: furtulon and mitomycin, FAM group: 5-FU, adriamycin and mitomycin, FLEP group: 5-FU, leucovorin, etoposide and cisplatin. The survival rates were analyzed using the Kaplan-Meier method and the Cox proportional hazards model. RESULTS: There were no differences among the groups of patients with regard to tumor characteristics except for lymph node metastasis and the ratio of metastasis to lymph nodes. In the FLEP group, the ratio of metastasis to lymph nodes was higher than in the other groups. The five and ten year survival rates of F, FM, FAM and FLEP were 51.9%, 28.9%, 59.5%, 49.8%, 66.1%, 57.4% and 30.0%, 27.5%, respectively. The univariate analysis showed that age, Borrmann type, lymph node metastasis, ratio of metastasis to lymph nodes, postoperative adjuvant chemotherapy and recurrence were significant factors for survival. For the multivariate analysis, recurrence, age, Borrmann type, ratio of lymph node metastasis and lymph node dissection were independent prognostic factors; however, the postoperative adjuvant chemotherapy was not an independent prognostic factor. CONCLUSION: The FAM regimen was the most beneficial postoperative adjuvant chemotherapy for improved survival rates; the FM regimen was the second and the FLEP regimen was the last. In order to determine the effectiveness of postoperative adjuvant chemotherapy in stage III gastric cancer, well designed prospective studies including a surgery only group will be needed.

9.Clinical Outcomes according to Primary Treatment in Gastric Cancer Patients with Peritoneal Seeding.

PURPOSE: Peritoneal seeding of gastric cancer is known to have a poor prognosis. With the diagnosis of peritoneal seeding, there is no effective treatment modality. Gastrectomy with chemotherapy or primary chemotherapy is basically one of major options for this condition. This study was conducted to compare the clinical outcomes of these treatments and to identify the better way to improve the prognosis of patients with peritoneal seeding. MATERIALS AND METHODS: Between 2001 and 2007, gastric cancer patients with peritoneal seeding by preoperative or intraoperative diagnosis were reviewed retrospectively. The enrolled patients were divided as primary gastrectomy and primary chemotherapy group. Clinicopathologic characteristics and clinical outcomes of groups were analyzed and compared. RESULTS: Fifty-four patients were enrolled. 21 patients belonged to the group of primary gastrectomy and 33 patients were to the primary chemotherapy group. Among 33 patients of the primary chemotherapy group, 17 patients were received only chemotherapy and 16 patients were received gastrectomy due to the good responses of primary chemotherapy. The 3 years survival rates were 14% in primary gastrectomy group, 55% in patients who received gastrectomy after primary chemotherapy, and 0% in patients with primary chemotherapy only. CONCLUSIONS: Although this study had many limitations, some valuable information was produced. In terms of survival benefits for the gastric cancer patients with peritoneal seeding, primary gastrectomy and additional gastrectomy after primary chemotherapy revealed the better clinical outcomes. But, prospective randomized clinical study and multi-center study are should be performed to decide proper treatment for gastric cancer patients with peritoneal seeding.

10.Malignant Solitary Fibrous Tumor of Retroperitoneum Mimicking Gastric Submucosal Tumor.

Solitary fibrous tumors (SFTs) are an uncommon neoplasm characterized by the proliferation of spindle cells. The diagnostic criteria of malignant solitary fibrous tumors (MSFTs) include high cellularity, high mitotic activity (4>10 HPF), pleomorphism, hemorrhage and necrosis. This tumor frequently involves the pleura and MSFTs of retroperitoneum mimicking gastric submucosal tumor are very rare. We report a rare case of MSFT that presented as a gastric submucosal tumor. A gastroscopic examination showed a large bulging mucosa in the gastric body. Abdominal computed tomography revealed a well-defined heterogeneous enhancing mass between the left hepatic lobe and gastric body. Surgical resection was performed and histologic features were consistent with a MSFT.
