1.Pharmacocavernosography: Its Role in Impotence.

Cavernosography after injection of papaverine was done in 25 impotent patients who showed equivocal evidence for either psychogenic or organic erectile dysfunction and who were strongly suspicious of having vasculogenic impotence. Diagnostic value of the pharmacocavernosography was evaluated. Venous leakages through deep dorsal vein were noted in 13 patients, out of whom 3 patients showed leakage also via deep vein, abnormal cavernosal vein or cavernoso-glans fistula. All the arteriogenic and psychogenic, but 38.5% of the venogenic impotents, showed full erection at 30 seconds after infusion of contrast medium was begun. The response to injection of the papaverine was well correlated with findings of the cavernosogram in venogenic and psychogenic impotents. Therefore, the pharmacocavernosography is valuable in differential diagnosis of the venogenic and arteriogenic impotence as well as in diagnosis of the vasculogenic impotence.

2.Development and Evaluation of an 'Activity and Rest' Integrated Course.

PURPOSE: This study was conducted to develop an integrated undergraduate course including a PBL based on a blended learning strategy, and evaluate learners' responses. METHODS: The learning contents of cardiovascular, respiratory, and musculoskeletal medical systems, and nursing diagnoses of 'activity and rest' domain (NANADA's classification II, 2005) were analyzed. Six clinical scenarios with the clients in different life cycles were developed for PBL. Classical lecture and group presentation with on-line self learning were implemented in addition to PBL. The developed course was implemented on 84 junior nursing students in a university for 7 weeks with 5 hours per day, two days per week. Students were asked to complete structured questionnaires including problem solving, critical thinking, and nursing diagnosis differentiation abilities. RESULTS: Learner's evaluation was positive in problem solving skills and in the differentiation ability of nursing diagnoses relevant to an 'activity and rest' functional health pattern. CONCLUSION: Development and implementation of integrated courses based on a blended learning method need to be continued to enhance students' thinking and self-directed learning abilities. Supporting strategies for individual learners should be added for successful blended learning such as individual on-line feedback and consideration of individual learning outcomes.

3.Factors Influencing on the Compliance of Breast Self-Examination of Nurses in a Local Area.

PURPOSE: The purpose of this study was to identify the influencing factors on the compliance of breast self-examination (BSE) among nurses who work at three general hospitals in Kyung-Nam areas. METHOD: 258 hospital nurses were included in the study. Data were collected using structured self-administered questionnaires and analyzed by descriptive statistics, t-test, x2 test and logistic regression analysis. RESULT: Nurses reported medium levels of knowledge, self efficacy and health believes about breast cancer and BSE, and 26% of the nurses performed the BSE at least once during the last 6 months. Compliers of BSE perceived significantly higher levels of self-efficacy, susceptability and health motivation, and lower level of barrier compared to non-compliers. Significant influencing factors on BSE compliance were 'experience of getting recommendation for breast self-examination', 'susceptibility', 'barrier', and 'self-efficacy' and those variables explained 22.5% of variance in compliance of BSE. CONCLUSION: Nurses, who must play as a role model for health promoting behaviors, did not have enough knowledge of breast cancer and BSE. Also, their performance rate of BSE was quite low. Thus, it is essential to provide an educational program for breast cancer and BSE to nurses in order to enhance nurses' performance rate of BSE.

6.Characteristic Analysis of Internet nutrition Counseling Served by the Korean Society of Community Nutrition.

An analysis for a period of 15 months (from September 2000 to November 2001), of the nutrition counseling internet site, "Dietnet" served by the Korean Society of Community Nutrition, showed the following results. Nutrition counseling was conducted without charge by 50 professionals, including professors in the food and nutritional areas, medical doctors, and dietitians. Counselees consisted of mostly females under 30 years of age. Although two thirds of the users of this site asked questions anonymously, they were very sincere. They asked questions mostly related to their own concerns, but there were quite a few inquiries about their parents or grandparents. There were significant differences in the counseling contents depending on the ages of the counselees. Teenagers showed the highest interest in height growth and weight control, whereas people in their twenties were mostly interested in diseases, foods, and weight control. Those over the thirty made inquiries mostly about diseases. The most frequent questions were relevant to gastrointestinal, circulatory and hepatic diseases. As indicated above, counseling questions included of a variety of both comprehensive and specific questions. There were also quite a few questions related to decisions on the correct usage of specific foods. Therefore, we recommend that more professionals, such as food science majors, medical doctors who practice western and oriental medicine, and other professionals who specialize in psychological behaviors related to weight control and infant food habits be recruited to serve the site more effectively and intensively. In light of the further need for internet sites with nutrition counseling and delivery of nutritional information to the public, nutritionists capable of managing websites are also required.

7.Effect of an Intervention Using Voice Recording of a Family Member on Patients Undergoing Mechanical Ventilator Weaning Process

Purpose:
This study aimed to determine the impact of an intervention using voice recording of family members on pain, anxiety, and agitation in patients undergoing weaning from mechanical ventilation.
Methods:
A randomized control pre-post experimental design was implemented to 53 participants, with 27 and 26 participants in the experimental and control groups, respectively. A 70-second voice recording of a family member, repeated three times at 10-minute intervals was used as an intervention for the experimental group. Meanwhile, participants in the control group used headset for 30 minutes. Structured instruments were utilized to measure pain, anxiety, agitation, and the weaning process. Wilcoxon Signed Ranks test and the Mann-Whitney U test, or χ2 test, were used for data analysis.
Results:
The experimental group exhibited significant decrease in pain (Z = - 3.53, p < .001), anxiety (t = 5.45, p < .001), and agitation (Z = - 2.99, p = .003) scores compared with those of the control group. However, there was no significant difference between groups in the weaning process’ simplification (χ2 = 0.63, p = .727).
Conclusion
Intervention using family members’ voice recording effectively reduces pain, anxiety, and agitation in patients undergoing weaning from mechanical ventilation. This can be actively utilized to provide a more comfortable process for patients.
8.Papillary Thyroid Cancer: Clinicopathologic Characteristics and Factors of Contralateral Jugular Lymph Node Metastasis after Lymph Node Dissection.

PURPOSE: Contralateral jugular lymph node metastasis (CJLNM) of papillary thyroid cancer (PTC) is rarely found during operative procedures. HoweverPTC is being diagnosed with increasing frequency and lymph node metastasis is now recognized as a factor of prognosis and recurrence. Therefore, this study was conducted to evaluate the clinical and histological characteristics of papillary thyroid cancer and to determine the factors that lead to CJLNM. METHODS: Two-hundred patients with PTC were treated in our hospital between March 2005 and October 2006. A retrospective analysis of the patient's clinical and histological features and lymph node metastasis was conducted. RESULTS: The total ratio of CJLNM to PTC was approximately 7.5%, the male to female ratio was 1:6.5, and the mean tumor size was 14.93 mm. In addition, the multiplicity was 53.3% and the bilatrality was 53.3%. Further, there were 4 cases involving benign thyroid disease combined with goiter. In addition, the capsule invasion was 100%. Age under 40 years, bilaterality and capsule invasion were found to be significant clinicopathologic factors of CJLNM induced by PTC. CONCLUSION: A contralateral jugular lymph node biopsy of PTC may be considered in cases involving patients under 40 years of age with, bilaterality and capsular invasion.

9.The Pattern of Weight Gain and Change of Peripheral Serum Lipid Profiles in Idiopathic Epileptic Children Treated with Valproate.

PURPOSE: This study was performed to investigate the change of weight and body composition, and lipid profiles in the idiopathic epileptic children treated with valproate. METHODS: We measured the weight, height, left arm circumference, left triceps skinfold thickness of 30 idiopathic epileptic children treated with valproate before, at 1 month and 4 months of treatment. We also analyzed the serum lipid profiles of the patients before and at 4 months of treatment. We analyzed the change of Body Mass Index (BMI), Arm Muscle Area(AMA), Arm Fat Area(AFA), and the serum lipid profiles. Two-way analysis of variance, chi-square test and paired-t test were used for statistical analysis. RESULTS: There was no statistically significant change in BMI, AFA, AMA of the patients. But the weight of 13(44.3%) children increased greater than 10% with valproate treatment. AMA were not changed, but AFA were significantly increased, 791.2+/-209.6 mm2 before treatment, 858.1+/-246.6 mm2 at 1 month, 1051.7+/-393.2 mm2 at 4 months, in 13 patients, whose weight gain were greater than 10%(P<0.05). There was no change in cholesterols, low density lipoproteins, high density lipoproteins of the 30 children. CONCLUSION: The increase of the weight in the children treated with valproate resulted from the increase body fat. So the weight gain may be a risk factor of obesity

10.Three Cases of Prevention Therapy to Reduce Perinatal HIV Transmission.

In 2008, the Joint United Nations Programme on HIV/AIDS (UNAIDS) estimated that about 430,000 children worldwide became infected with HIV, mostly through mother-to-child transmission (MTCT) during pregnancy, labor, delivery, or breast-feeding. The MTCT prevention program proved to be feasible and effective in reduction of perinatal HIV transmission. Three babies born from HIV-infected mothers were admitted to the National Medical Center in 2009. Only two women received antiretroviral (ARV) therapy during pregnancy, labor, and after delivery, and their infants received zidovudine (AZT) for 6 weeks. The outcome, after a follow-up period of 4 months to 16 months, was favorable in all patients. Thus, we emphasize the need for expansion of antenatal HIV screening of pregnant women, implemented for early HIV diagnosis and effective ARV therapy for reduction of perinatal HIV transmission.
