1.Effect of PAF inhalation on the non-specific bronchial hyperreactivity according to the severity of asthma.
Korean Journal of Allergy 1997;17(2):129-140
PAF itself induces the non-specific bronchial hyperreactivity(NSBH) in asthma. NSBH induced by PAF is nearly as same as that by the sensitized allergen inhalation even though PAF never provokes the late asthmatic response. Usually the amount of allergen for BPT inversely correlated with the severity of asthma, but in PAF, nothing was clarified about that, yet. usually eosinophil is the main effector cell in PAF-induced NSBH. So we want to declare whether PAF inhalation affects the NSBH differently according to the severity of asthma or not in this study and eosinophil is the main effector cell in asthmatics. Single dose PAF(400microgram/microliter) inhalation challenge was done by the Cockcroft' s method but that of methacholine by Chai's method before and after PAF until NSBH returned the basal value. From that, we calculated PC20. Also we checked the eosinophil and platelet count in peripheral blood before and after PAF until those returned the baseline. All these were done in two groups, mild asthmatics and moderate asthmatics classified by the PFT and their PC20. The Bronchoconstriction started at the 3 minutes after PAF inhalation and then relieved completely 30 minutes after inhalation but in mild asthmatics bronchi constricted much more than moderate asthmatics. The fall of NSBH also persisted much longer in mild asthmatics. But the decrease in peripheral eoslnophil counts was nearly same in both groups. None of asthmatics showed the changes in platelet counts in peripheral blood. The degree of bronchoconstriction and decrease in eosinophil counts didn't have any correlation with the persistence of the NSBH. Taken together, PAF inhalation affected the NSBH inversely according to the severity of asthma in this study but eosinophil might not be the only effector cell in PAF-induced NSBH.

Malignant fibrous histiocytoma (MFH) of the liver is uncommon, representing less than 1% of the primary malignant lesions of the liver. We report primary MFH of the liver in a 59-year-old woman. The tumor, measuring 9.0 9.0 6.0 cm, was located in the left lobe of the liver. It showed multiple areas of hemorrhage and necrosis. Microscopically, the tumor consisted of plump spindle cells haphazardly arranged in short fascicle and focal storiform pattern. Multiple bizarre giant cells were also noted. Immunohistochemically, many of the tumor cells were positive for vimentin and alpha1-antitrypsin but negative for epithelial markers. Ultrastructurally, the tumor cells showed fibroblastic and histiocytic features.

In acupuncture practice of animals, preanesthetics sometimes are needed. The purpose of this study was to select the ideal chemical restraint at acupuncture for gastric motility. Nine healthy mixed breed dogs weighed 10-21 kg and aged 1-3 years old were used in this study. Two EMG surface electrodes were placed between the seromuscular and the mucosal layer of pylorus. Twenty minutes after feeding normal gastric motility was recorded for 60 minutes using physiograph (Narco-Biosystem). Then preanesthetic treated-gastric motility was observed for 30 minutes. Preanesthetics used were xylazine, diazepam, and acepromazine. Acupuncture needles were inserted to BL-21 (Wei-Yu) acupoint, and then changes of gastric motility were recorded for 60 minutes. The gastric motility following xylazine administration (1 mg/kg, IV) was markedly decreased. BL-21 (Wei-Yu) acupoints stimulation did not alter xylazine-induced depression of gastric motility. The diazepam (1 mg/kg IV) treated-gastric motility was increased mildly 20 minutes after drug administration. BL-21 (Wei-Yu) acupoint stimulation after diazepam administration enhanced gastric motility significantly. The gastric motility following acepromazine (0.3 mg/kg, IM) administration was not changed compared with normal gastric motility. Application of traditional acupuncture at BL-21 (Wei-Yu) significantly increased the gastric motility. Based on these results, acepromazine and diazepam could be acceptable chemical restraints for acupuncture therapy of gastric motility, but xylazine couldn't be.

We report thin-section CT findings of pulmonary fat embolism which was confirmed by clinical features and microscopic examination of cells obtained by bronchoalveolar lavage. Initial thin-section CT showed extensive air space consolidation and multiple ill-defined nodular densities in both lungs. Follow-up CT revealed ground-glass appearance and faint nodules in both lungs. Perfusion scan showed multiple small perfusion defects in the peripheral portion of both lungs.

Benign hemangioma of the mediastinum is rare. This slowly growing tumor is described as well circumscribed, cystic, hemorrhagic tumor. Histologically it can be differentiated into capillary or cavernous form. We present a case of mediastinal hemangioma. A 20-year-old-man was presented with a slowly growing posterior mediastinal mass of 6 years duration, 8x6 cm in size. The mass was relatively well defined but focally invasive. Microscopically, it was differentiated into vessels of capillary, cavernous, and venous patterns. A solid cellular proliferation with inconspicuous capillary lumens was focally seen. The stroma between variable-sized vessels showed marked myxoid change associated with some smooth muscle bundles and adipose tissue. Ultrastructurally, areas of solid cellular proliferation showed formation of lumens. These lumens were lined by active endothelial cells showing plasmalemmal vesicles and Weibel-Palade bodies on the abluminal surface.
