1.Evaluation of the Location of Peptic Ulcer.

Peptic ulcer can occur anywhere in the gastrointestinal tract that contact with acid and pepsin. But usually peptic ulcer occurs at the stomach and duodenum and the location of peptic ulcer are localized to specific area within the stomach and duodenum. So I think that predilection of peptic ulcer to localized area may have clue for releaving the pathogenesis of peptic ulcer. So I studied the location of peptic ulcer for the basic reserch of ulcer pathogenesis by means of dividing stomach into antrum, angle, lower body, middle body, upper body and lesser curvature, anterior wall, greater curvature, posterior wall and duodenum into anterior wall, lesser curvature, posterior wall, and greater curvature. The results obtained were as follows: 1) The chronic gastric ulcer was most common in the angle of lesser curvature (22.9%) and second common in the antrum of lesser curvature (22.3%). 2) The location of chronic and acute gastric ulcer was similar. 3) The proximal migration of ulcer site according to increasing age was not observed. 4) Chronic duodenal ulcer was occurred commonly in the lesser curvature (41.8%) and anterior wall (38.3%). 5) As the age increase, the incidence of chronic duodenal ulcer in the lesser curvature was increased and that of chronic duodemal ulcer in the anterior wall was decreased. 6) Acute ulcer of stomach and duodenum has no predilection site.

2.A Comparison of Clinical Characteristics between Adenoviral and Group A Streptococcal Pharyngitis in Children.

PURPOSE: To compare the clinical characteristics and laboratory finding between adenoviral and group A streptococcal (GAS) pharyngitis. METHODS: A retrospective review of medical records was performed in the patients with adenovirus infection among those who were admitted for febrile respiratory disease from January 2011 to July 2013 and GAS pharyngitis among those who visited for symptoms of scarlet fever from August 2006 to July 2013. RESULTS: 179 patients (AV1 group) were diagnosed with adenoviral pharyngitis and 37 (AV2 group) of these patients had adenovirus single infection. 26 patients (GAS group) were diagnosed with scarlet fever. Adenoviral infection (AV2 group) developed in younger patients compared to GAS group (2.8+/-2.1 years vs. 5.4+/-1.8 years, P=0.000). Total durations of fever and admission were longer in AV2 (6.3+/-2.6 days vs. 3.3+/-1.9 days, P=0.000; 4.1+/-1.2 days vs. 1.9+/-1.8 days, P=0.000, respectively). WBC counts were higher in AV2 (11,449+/-5,680 cells/mm2 vs. 6,722+/-6,941 cells/mm2, P=0.000). CRP was not significantly different between AV2 and GAS group (3.8+/-3.2 mg/dL vs. 5.2+/-5.1 mg/dL, P=0.368). No difference was found between two groups in the percentage of antibiotics use (91.9% vs. 100%, P=0.261). CONCLUSION: Clinical characteristics and measures of inflammation in the laboratory findings were similar between adenoviral and GAS pharyngitis group. It is necessary to conduct the test for respiratory virus and bacteria in early stage to differentiate in the pharyngitis patients with leukocytosis and elevation of CRP level.

4.A Case of Congenital Megacalyces.

Congenital megacalyces is a congenital renal disease characterized by hypoplasia of the medulla and a non-obstructive dilatation of the calyces. More than 100 cases have been reported in the literature after first described by Puigvert in 1963, but it has been reported only three cases (two cases of adults and one case of child) in the literatures in Korea. The etiology of the congenital megacalyces is unknown. Renal pelvis and ureter are mostly normal in size and there is no urinary obstruction or reflux. Renal medulla of congenital megacalyces is thinner than that of normal kidney but renal cortex retains its normal thickness and function. This is features that help to distinguish megacalyces from obstructive atropy. The calyces are polygonal in shape and the majority of kidney with megacalyces have increased number of calyces. Since congenital megacalyces is a non-progressive lesion, it is important to recognize the clinical entity and avoid unnecessary surgery. We report one case of congenital megacalyces without urinary obstruction or reflux with a brief review of related literatures.

5.The Correlation between the Tc2 Cells and the CCR4 Expression and the PASI Score in the Peripheral Blood of Psoriasis Patients.

BACKGROUND: Psoriasis is a chronic relapsing skin disorder that is characterized by abnormal epidermal proliferation, inflammation and angiogenesis. It causes emotional and social consequences that go far beyond the skin; therefore, many methods to measure and monitor the severity of psoriasis have been reported. OBJECTIVE: This study aims to evaluate the usability of the flow cytometric analysis of the T cell subsets and their chemokine receptors in the peripheral blood of the psoriasis patients as a severity index. METHODS: The T cell subsets and their chemokine receptor expression (CXCR3, CCR4) in the circulating blood of thirty psoriasis patients (PASI score:2.2~44.2) and twenty healthy controls were examined by flow cytometry. The relationship between the PASI score and the T cell subsets/chemokine receptors was also analyzed. RESULTS: The patients showed significantly higher number of Tc1 (CD8+CXCR3+), Tc2 (CD8+CCR4+) and CXCR3/CCR4 expressing cells than did the control group. Especially, the moderate to severe patients (a PASI score greater that 5) showed a higher number of Tc1, Tc2 and CCR4 expressing cells than did the control group. In the severe patients (a PASI score greater than 10), the frequency of circulating Tc2 cells and CCR4 expressing cells was directly correlated with the PASI score. CONCLUSION: Our findings suggest that flow cytometric analysis of the circulating T cell subsets with further classification could serve as an indicator of the disease severity in psoriasis patients.

6.Drug Resistance to 5 - Fluorouracil and Overexpression of Thymidylate Synthase mRNA in Human Gastrointestinal Malignancies.

PURPOSE: The cytotoxicity by 5-fluorouracil (5-FU) is mediated by inhibition of thymi- dylate synthase (TS), which is a rate-limiting enzyme for DNA synthesis. To test whether the resistance to 5-FU would be associated with cellular TS activity, we analyzed TS gene expression from human gastrointestinal cancer cell lines. MATERIALS AND METHODS: We established the experimental conditions for quantitating TS mRNA expression by competitive RT-PCR using mimic DNA. Based on this method, we compared TS mRNA expression between 5-FU resistant cell line and parent cell line and investigated the expression of TS mRNA following 5-FU administration in 6 human gas- trointestinal cancer cell lines. RESULTS: Competitive RT-PCR using mimic DNA seemed to be more effective than Northern blot analysis for quantitation of TS gene expression. The quantity of TS mRNA and IC50 value of 5-FU in 5-FU resistant H630 was found to be 2.5 and 10 times higher than in parent cell line, respectively. And also, we observed linear relationship between TS mRNA level and IC50 value of 5-FU (r 0.76) in 6 gastrointestinal cancer cell lines. CONCLUSION: These results suggest that overexpression of TS mRNA may play a role in the development of 5-FU resistance in human gastrointestinal malignancies

7.The Effect of Intrathecal Fentanyl on Hypnotic Requirement of Propofol during Spinal Anesthesia.

BACKGROUND: Intrathecal opioid, a useful adjunct to prolong analgesic effects, potentiates spinal anesthesia. The present study was designed to evaluate the effect of intrathecal fentanyl on the hypnotic requirements of propofol during spinal anesthesia. METHODS: Sixty patients scheduled for elective urologic surgery under spinal anesthesia were randomly assigned to one of four groups. In Group 1 (n =15), spinal anesthesia was performed with hyperbaric bupivacaine 10 mg, in Group 2 (n = 15) with hyperbaric bupivacaine 10 mg plus fentanyl 10microgram, in Group 3 (n = 15) with hyperbaric bupivacaine 10 mg plus fentanyl 20microgram, and in Group 4 (n = 15) with hyperbaric bupivacaine 10 mg plus fentanyl 30microgram. After performing spinal anesthesia, the target plasma concentration of propofol was set at 2.0microgram/ml. Effect site concentration (Ce) was measured when the BIS's reached 80 or 70. Ce was titrated to maintain a BIS of 70-80. Observer's sedation scoring (OAA/S), systolic and diastolic pressures, SpO2 and total infusion dose were measured every 10 minutes for 1 hour. RESULTS: Ce's at a BIS of 80 were 1.2+/-0.3microgram/ml (Group 1), 1.0+/-0.3microgram/ml (Group 2), 0.9+/-0.2microgram/ml (Group 3), and 0.8+/-0.2microgram/ml (Group 4), respectively. Ce's at a BIS of 70 were 1.6+/-0.2microgram/ml (Group 1), 1.5+/-0.4microgram/ml (Group 2), 1.2+/-0.3microgram/ml (Group 3), and 1.2+/-0.3microgram/ml (Group 4), respectively. Ce's of Group 3 and Group 4 at BISs of 70 and 80 were significantly lower than those of Group 1 (P<0.05), and the propofol requirements of Groups 3 and 4 to maintain a BIS of 70-80 were significantly less than those of Group 1 (P<0.05). CONCLUSIONS: These results indicate that adding more than 20microgram of fentanyl to hyperbaric bupivacaine for spinal anesthesia reduces the hypnotic requirement of propofol.

8.One portion size of foods frequently consumed by Korean adults.

This study aimed to define a one portion size of food items frequently consumed for convenient use by Koreans in food selection, diet planning, and nutritional evaluation. We analyzed using the original data on 5,436 persons (60.87%) aged 20 ~ 64 years among 8,930 persons to whom NHANES 2005 and selected food items consumed by the intake frequency of 30 or higher among the 500 most frequently consumed food items. A total of 374 varieties of food items of regular use were selected. And the portion size of food items was set on the basis of the median (50th percentile) of the portion size for a single intake by a single person was analyzed. In cereals, the portion size of well polished rice was 80 g. In meats, the portion size of Korean beef cattle was 25 g. Among vegetable items, the portion size of Baechukimchi was 40 g. The portion size of the food items of regular use set in this study will be conveniently and effectively used by general consumers in selecting food items for a nutritionally balanced diet. In addition, these will be used as the basic data in setting the serving size in meal planning.

10.A Case Report on Glass Fiber-induced Health Hazard due to Car interior Material.

Glass Fiber is manufactured from typical glass-making raw materials, silicon dioxide with various metals and other elements. There are three distinct types of commercial glass fiber products: glass wool, an entangled mass of interlocking fibers; continuous glass filament, a product with a more ordered arrangement of fibers; and special-purpose glass fiber, a small-diameter fiber (less than 3 um) . We report a case of glass fiber-induced health hazard due to continuous glass filament from fiber reinforced plastics (FRF) as car interior material. A 40 years old man complained a sudden onset of severe itching on whole body, especially neck and forearms while driving a new car. He also complained burning of eyes, sore throat and productive cough. We diagnosed the glass fiver-induced skin lesion by placing skin scraping on a slide glass and examining the specimen under a microscope. We found same fibers from the air samples in the car and from the material of the interior surface of the car by the microscopic examination. Therefore we confirmed that the patient's symptoms and signs were induced by glass fiber.
