4.Comparison of Reconstructive Techniques after Total Gastrectomy as Determined by Patient Quality of Life and Nutritional Status.

PURPOSE: Many reconstruction procedures have been developed in order to resolve patient complaints after a total gastrectomy. However, until now, there has been no general agreement with regard to the ideal reconstruction to perform after a total gastrectomy. Moreover, there have been few reports of the long-term effects of different reconstruction procedures. In this study, we compared the long-term nutritional status and quality of life for patients that received either a Roux-en-Y esophagojejunostomy (R-Y), jejunal interposition (JI), jejunal pouch interposition (JPI), or double tract reconstruction (DT), to determine the optimum reconstruction procedure after a total gastrectomy. MATERIALS AND METHODS: The study consisted of 41 patients younger than 80 years who had undergone a total gastrectomy with curative resection and who had no evidence of recurrence at our hospital between August 2000 and January 2004. The patients were classified into the following four groups, according to the type of reconstruction; simple Roux-en-Y (R-Y group, n=15), jejunal interposition (JI; group, n=8), j Jejunal pouch interposition (JPI group, n=8), double tract interposition (DT group, n=10). The nutritional status was assessed by measuring body weight, serum albumin level, serum hemoglobin level, and the level of serum total protein, and the PNI (Prognostic nutritional index). Quality of life was assessed by GSRS. In addition, endoscopy was performed to confirm the presence of reflux esophagitis. RESULTS: The laboratory findings showed no significant differences between the four groups except for a lower total protein and album level in the DT group after 3 years postoperatively (P=0.006, P=0.033). The percentage of body weight at 1 year, and 3 years postoperatively in the JI group (P=0.013, P=0.011) were significantly less than other groups (P=0.011, P=0.000). The frequency of postoperative symptoms and reflux esophagitis and eating capacity showed no significant differences between the four groups. The GSRS score in the JIP group was significantly better than for the other groups at 1 year, and 3 years postoperatively (P=0.028, P=0.003). CONCLUSION: We believe that the jejunal pouch interposition is the most useful of the four procedures for improving postoperative quality of life.

Deep cervical space abscess may cause life-threatening complications despite the advent of antibiotics, so early detection and localization of the abscess are essential for management decision. To evaluate the efficacy of utilizing CT scanning in deep cervical space abscess, we retrospectively revieweed CT of 17 patients confirmed by surgery. CT was found tobe very accurate in defining the presence, location, and extent of the abscess along with the demonstration of life-threatening complications such as airway compression and mediastinal extension. In conclusion, CT can play and essential role in guiding surgical approach to deep cervical space abscess.

6.Blood lipid levels in Korean Air force Pilots.

This study was conducted to identify the factors for serum cholesterol level among Korean Air Force pilots. We obtained risk factors for 408 pilots examined during period from January 1996 through March 1997. The associations between total cholesterol, LDL-cholesterol, triglyceride, HDL-cholesterol and age, body mass Index(BMI), smoking, drinking, blood pressure, flight time and fat intake were analyzed. The mean serum total cholesterol, HDL-cholesterol, LDL-cholesterol, and triglyceride levels were 161.8+/-31.0 mg/dl, 475+/-11.0 mg/dl, 93.1+/-30.2 mg/dl, and 106.5+/-61.3 mg/dl respectively. Possible risk factors were total flight time, BMI, smoking, drinking, exercise, flight, and fighter or not. Total flight time and BMI were statistically significant factors In serum cholesterol level while controlling for the effect of the remaining factors. The longer the total flight time, the higher the level of serum total cholesterol. Total flight time, BMI, and drinking amount were statistically significant factors in serum HDL-cholesterol while controlling for the effect of the remaining factors. The higher the BIM, the lower the level of HDL-cholesterol. BMI was a statistically significant factor in seurm LDL-cholesterol while controlling for the effect of remaining factors. Total flight time, BIM, and flight were statistically significant factors in serum triglyceride while controlling for the effect of the remaining factors. Conclusively, BIM was a only significant factor in all four lipid levels, and total flight time took an affect to total cholesterol, HDL-cholesterol, and triglyceride levels.

Although digital gangrene is an uncommon clinical feature, it may lead to serious complications, such as amputation. Therefore, it requires prompt evaluation and treatment. Digital gangrene is often seen in systemic sclerosis, but is rare in systemic lupus erythematosus (SLE). In SLE, digital gangrene results from vasculitis, vasospasm, and thromboembolism. Here, we report a 15-year-old male SLE patient who initially presented with digital gangrene, and present a review of the relevant literature.

10.Occupational asthma caused by several kinds of herb materials.

There have been a few cases of occupational allergy caused by herb materials. In this study, we report a case of occupational asthma and rhinitis sensitized by six kinds of herb materials-Ostericum (Kangwhal), Angelica (Danggui), Cnidium (Chunkung), Pinellia (Banha), Zingerber (Kunkang) and Discoreae (Sanyak) in a pharmacist working at a pharmacy. The patient had shown negative responses to 80 common inhalant and food allergens, but strong positive responses to six herb material extracts, Kangwhal, Danggui, Chunkung, Banha, Kunkang and Sanyak, were noted on skin-prick test. Bronchoprovocation test showed a dual asthmatic response to Danggui extract. Serum specific IgE antibodies to Chunkung, Banha and Sanyak were detected by enzyme-linked immunosorbent assay (ELISA) with no specific IgE binding to Kangwhal, Danggui and Kunkang extracts. In order to further characterize the allergic components of these three extracts, sodium dodecyl sulphate-polyacrylamide gel electrophoresis (SDS-PAGE) and electroblotting studies were performed. One IgE binding components (60 kDa) were detected within Chunkung extract, two (10, 25 kDa) in Banha and four (33, 34, 65, 98 kDa) in Sanyak. It is suggested that Chunkung, Banha and Sanyak may induce IgE-mediated bronchoconstriction in an exposed worker. Further studies are needed to investigate the pathogenic mechanisms induced by Kangwhal, Danggui and Kunkang.
