1.Rabbit Corneal Endothelial Cell Damage by Q-switched Nd:YAG laser.

Corneal endothelial damages following Nd: YAG laser application were evaluated rabbits. The first group underwent anterior capsulotomy, while the second group received laser applications at the corneal center and anterior capsule. The rabbits were evaluated with the intraocular pressure, the corneal endothelial cell number, the thickness of the corneal center, and the morphologic change of the cornea, before laser application and at postaplication 1 day, 7 day, and 14 day. But the endothelium was damaged when laser shots were applied at the cornea center and anterior caps ulotomy. In ophthalmologic procedure with Q-switched Nd:YAG laser, the minimal effective poser should be selected, the laser beam should be applicated on extremely fine focus and contact lens should be used to minimize the corneal endothelial injury.

2.Studies on anticancer effect of momordin on oral carcinoma (KB) cells.

Treatment of oral cancers with chemotherapeutic agents are evaluated as an effective method for remission to reduce cancer proliferation nowadays. But, minimization of side-effects such as bone marrow suppression, gastrointestinal toxicity and renal damage is another problem to be solved. Thus, a possible approach to develop a clinically applicable chemotherapeutic agents is to screen anticancer activity among traditional medicinal plants which have been used for thousands of years with very low side-effects in orient. In this study we focused on anti-oral cancer activities of momordin, which was medicinal plant extracts that was revealed anticancer activities, on KB cell(oral cancer cell). The results were as follow : 1. Momordin showed the excellent anti-oral cancer activity against KB cells. Obtained IC50 value of Momordin was 10.4 microgram/ml. 2. When KB cells were treated with Momordin, dose and time dependent DNA fragmentation of KB cells were observed. DNA fragmentation was initiated on three days at the concentration of 20 microgram/ml Momordin. 3. Flow cytometry showed dose-dependent apoptotic cell increase of KB cells on Momordin. 18.55% apoptotic cell were observed up to 72 hours at the concentration of 20 microgram/ml of Momordin 4. Momordin induced nonspecific apoptosis without specific cell cycle arrest. 5. Through MTT assay, DNA fragmentation assay and flow cytometric analysis. anticancer effect of Momordin against KB cell was induce of apoptotic cell death.

4.Effect of Indomethacin Therapy in Prematurity with Patent Ductus Arteriosus:study of Its Effectiveness in Treatment Modality.

PURPOSE: Conventional indomethacin therapy(0.2mg/kg every 12 hours for three doses) has been used for closure of PDA. The effect of prolonged low dose of indomethacin therapy(0.1mg/kg daily for six days)had been reported in foreign country but, nothing had been reported in our country. So we attempted this study to examine effects of these two methods. METHODS: Forty one infants with PDA of prematurity from January 1992 to July 1995 who were admitted in NICU of Il Sin Christian Hospital were included. 27 of these infants received conventional dose of indomethacin therapy and 14 received prolonged low dose of indomethacin therapy, and we examined with closure rate and complication etc. RESULTS: 1) Closure of PDA was observed in 15(55.6%) and relapse was 3(11.1%) in conventional dose therapy group. In prolonged low dose therapy group, closure was 8(57.1%) and relapse was none. 2) Intraventricular hemorrhage was observed in 20(74.1%), 6(42.9%) and gastrointestinal tract bleeding was 6(22.2%), 13(92.2%) in each group. There was statistically significant between the two groups(p<0.05). 3) The rise of serum BUN, creatinine was observed in 9(33.3%), 6(46.2%), bleeding tendency was 9(33.3%), 8(57.1%), necrotizing enterocolitis was 2(7.4%), 0 and retinopathy of prematurity was 8(29.6%), 3(21.4%) in each group.But there was not statistically significant correlation between the two groups. 4) The development of sepsis and broncopulmonary dysplasia was slightly more in prolonged low dose therapy group. 5) The reduction of urine output was observed in 11(40.7%), 2(14.3%) in each group but, absolute oliguria was not observed in both groups. CONCLUSIONS: Though the closure rate of PDA was similar in both groups, prolonged low dose indomethacin therapy can be recommanded with its effectiveness on preventing the relapse of PDA and the accurrence of necrotizing enterocolitis.

A potent antiproliferative agent, Mitomcin-C, has been known to improve the surgical outcom of glaucomatous eyes with poor prognosis after filtering surgery. Twenty one eyes of 21 patients underwent trabeculectomy with Mitomyc-C. Overall success rate is 81%: 2 of 3 eyes with glaucoma after unsuccessful filtering surgery, 3 of 4 eyes with neovascular glaucoma, 2 of 3 eyes with secondary glaucoma, 1 of 2 eyes with aphakic glaucoma and all of 9 eyes with advanced glaucoma. Postoperative complications were prolonged conjuntival wound leakage in two eyes, conjuntibval wound leakage and corneo-lenticular touch in one eye, choroidal detachment in one eye, progression of cataract in one eye, hyphema in 3 eyes.

7.The Effect of Augmented Lateral Tarsal Strip for Paralytic Ectropion in Leprosy Patients.

PURPOSE: To analyze the effect of the augmented lateral tarsal strip for the correction of the paralytic ectropion in leprosy patients. METHODS: Ten leprosy patients (16 eyelids) with exposed keratitis and lagophthalmos from paralytic ectropion underwent surgery of the augmented lateral tarsal strip. Preoperative and postoperative vertical palpebral aperture, marginal reflex distance, lagophthalmos, and anterior segment findings were recorded at 3 and 6 months after surgery. Postoperative symptomatic and functional improvements were assessed at 6 months after surgery. RESULTS: There was a significant reduction between preoperative and postoperative measurements for vertical palpebral aperture (3.1+/-0.4 mm), lower marginal reflex distance (2.1+/-1.0 mm), and lagophthalmos (2.0+/-1.2 mm). Eye irritation symptoms and lid functions were improved in all patients. In a survey, the symptomatic, functional satisfaction was achieved in 90% of patients. CONCLUSIONS: The augmented lateral tarsal strip is a sufficiently effective surgical procedure to be considered in the treatment of paralytic ectropion in leprosy patients.

8.Neurilemoma of the Bulbar Conjunctiva: Report of One Case.

PURPOSE: To report one case of neurilemoma of the bulbar conjunctiva with a review of the relevant literature. CASE SUMMARY: A 15-year-old girl presented with a bulbar conjunctival mass in the right eye. There were no other medical or ophthalmologic problems. The patient wanted to remove the mass for cosmetic reasons. We performed an excisional biopsy and simple wound closure. The mass was composed of bipolar spindle shaped cells that were arranged in bundles and fascicles in dense fibrous tissue. Immunohistochemical staining for the S-100 protein was positive. Two months after mass excision, there was no evidence of recurrence. CONCLUSIONS: We successfully treated a case of bulbar conjunctival neuriliemoma by simple excision.

9.Clinical Significance of Myocardial Perfusion Imaging Using Oral Dipyridamole Thallium-201 SPECT in Coronary Artery Disease.

The clinical utility of thallium-201 SPECT combined dwith pharmacologic vasodilation induced by oral dipyridamole as an alternatiove to intravenous dipyridamole was incestigated in 21 patients who had concomitant coronary arteriography. Tomographic images were assessed visually. Sensitivity & specificity for overall detection of coronary artery disease were 93.7% and 80% respectively. Sensitivity & specificity for identification of indevidual diseased vessels were 84.6% and 87.5% for the left anterior descending artery, 75% and 84.6% for the right coronary artery, 60% and 100% for the left circumflex artery, respecitively. Of the 26 patients unergoing thallium scintigraphy 11 patients(42.3%) had some adverse effects between 20 and 50 minutes after oral dipyrdamole ingestion, including headache(26.9%), chest pain(26.9%), electrocardiographic changes(19.2%), and nausea(11.5%). Intravenous aminophylline was used to resolve these adverse effects in 8 patients & most of the adverse effects were subsided within 10 minutes. There was no ventricular arrhythmias, myocardial infarctions or deaths. In conclusion, oral dipyridamole thallium-201 SPECT is safe and accurate test for the overall detection of coranary artery disease and identification of disease in individual arteries. Furthermore it is useful for determining the necessities of coronary reperfusion and prognstically stratiofying the patients with coronary artery disease.

10.A Case of Verrucous Carcinoma of Renal Pelvis.

Verrucous carcinoma is a form of low grade squamous cell carcinoma that is characterized grossly by its exophytic and wart-like appearance. We experienced a case of verrucous carcinoma in a 63-year-old female patient who presented with pyelonephritis caused by staghorn calculi. Abdominal CT scan showed a renal pelvic mass and nephrectomy was done. Histologic examination showed extensive keratinizing squamous metaplasia of pelvic urothelium with an area of verrucous acanthosis. To our knowledge, this is the second case of verrucous carcinoma of renal pelvis that has been reported in literature. We describe a case and review the literature concerning verrucous carcinoma.
