We recently encountered two cases of emphysematous pyelonephritis with abscess formation, which have recovered by only conservative treatment and percutaneous abscess drainage(PAD). Two patients had diabetes mellitus. in the first case, initial CT showed left intrarenal abscess with air-fluid level. In the second case, CT showed right perirenal abscess with air-fluid level. PAD was performed under CT and US guidances, respectively. Both patients were successfully treated with PAD and conservative treatment. Follow up after confirmation of abscess resolution by CT scan and contrast study through the catheter, we removed catheters two months and 40 days after PAD respectively.

In 1978, Bjornberg and Friis reported 32 patients with persistent pusi ulosis(PP) on distal extremities, of whom 13 patients were initially diagnosed as scabies. In 1984, Elpern described 7 patients with infantile acropustulosis(IA), of whom 4 patients had antecedant scabies. We report herein a case of PP, which was found in a 18 month-old mal who was treated for scabies at the age of 5 month and 12 month. The pruritic vesicopustules dev',lo ed 2 months after the second treatment for scabies and relapsed for 4 months. We also discuss the relationship between scabies and PP and also between PP and IA.

A 37-year-old female presented with a lobulated 2.5 x 1.5cm sized tumor with several sat ellite papules on the left lateral malleolar area for 27 years. Hiopsy specimens from hoth the lobulated lesion and the satellite lesion showed findings similar with features of intradermal nevus.

BACKGROUND: The 5-hydroxytryptamine type 3 (5-HT3) receptor is a member of the Cys-loop superfamily of ligand-gated ion channels (LGICs) and modulated by pharmacologic relevant concentrations of volatile anesthetics or n-alcohols like most receptors of LGICs. The goal of this study was to reveal whether the site-directed single mutations of E-106, F-107 and R-222 in 5-HT3 receptor may affect the anesthetic modulation of halothane known as positive modulator. METHODS: The wild-type and mutant receptors, E106D, F107Y, R222F, R222V, were expressed in Xenopus Laevis oocytes and receptor function was assessed using two electrode voltage clamp techniques. RESULTS: E106D, F107Y, R222F, R222V mutant 5-HT3A receptors were functionally expressed. F107Y mutant 5-HT3A receptors displayed decreased sensitivity to 5-HT compared to the wild type 5-HT3A receptor (P < 0.05). Halothane showed positive modulation in both wild and F107Y mutant 5-HT3A receptors but F107Y mutant 5-HT3 receptor showed greater enhancing modulation comparing to wild-type receptor. Meanwhile, R222F and R222V mutant 5-HT3 receptor lost positive modulation with 1 and 2 MAC of halothane. Most interestingly, positive modulation by halothane was converted into negative modulation in E106D mutant 5-HT3A receptor. CONCLUSIONS: The present study implicate the amino acid residues known for agonist binding and linking agonist binding to channel gating might also have important role for anesthetic modulation in 5-HT3A receptor.

Osteochondrodysplasia is a heterogeneous group of disorders appearing short limbed dwarfism. Because many of these entities are lethal and hereditary, an accurate diagnosis is mandatory. The purpose of this study is to define the clinicopathologic features and radiologic findings of osteochondrodysplasia. We reviewed 29 autopsy cases of congenital short limbed dwarfism, consisting of thanatophoric dysplasia (TD) (12 cases), osteogenesis imperfecta (OI) (12 cases), asphyxiating thoracic dysplasia (ATD) (3 cases), short-rib-polydactyly syndrome (SRPS) (1 case) and hypochondrogenesis (1 case). The gestational age ranged from 16 to 41 weeks. Of 6 fetuses that were born alive, 3 were ATD, 2 were TD and 1 was hypochondrogenesis. TD was frequently complicated by hydramnios. Of 8 cases studied chromosomally, only 1 showed chromosomal abnormality -46XY, inv 9. Intrauterine growth retardation was frequently associated with OI. Pulmonary hypoplasia was present in 23 cases (79%), including all cases of ATD, SRPS and hypochondrogenesis, 11 in TD and 7 in OI. Other associated anomalies were present in 17 cases (59%).

PURPOSE: This study was done to explore and understand acculturation focusing on reproductive health of immigrant women. METHODS: For the research sixteen immigrant women were selected by snowball sampling. Qualitative data were accumulated by in-depth interviews and private document collection. Raw data was analyzed following Mandelbaum's conceptual framework. RESULTS: The dimensions of immigrant women consisted of existence: emerging from the new environment in which it was hard to communicate and to get acquainted with others, reproduction: in the absence of learning and experience, reproductive health crisis, parenting: unmanageable burden. Turnings of life involved 'Inconvenience in one's eyes, vent for conflict and tension: pregnancy', 'strange medical care: accoucheur, rapid medical service', 'pain of morning sickness: poor maternal nutrition', 'manifestation of protective instinct for life'. In adaptations, content was as follows. 1) Standing alone as a Korean housewife, 2) Becoming aware of Korean maternal instinct: thirst for education supporting, 3) Rediscovery of family: growing maternal sense of existence. CONCLUSION: The results of this study show the acculturation process and the meaning of events related to reproductive health in current lives and can contribute to an integrated understanding of married immigrant women in Korean culture.

Research suggests that medical students frequently experience mental health problems such as stress, burnout, and depression, which may, in turn, affect suicidal ideation and behaviors. Since mental health problems profoundly impact academic achievement and professionalism, it is vital to understand factors influencing students’ mental health and identify strategies to provide the necessary support. Some relevant influencing factors range from the personal level, including gender, personality traits, perfectionism, and social support, to the environmental level, including the grading system, educational phases, exposure to patients’ death, mistreatment, and culture of medicine. In this regard, a comprehensive mental health support system that encompasses environmental interventions, as well as personal-level support, is needed. Simultaneously, proactive approaches that address the improvement of self-care and alleviation of systemic burdens are essential, together with the predominant reactive approaches focusing on problems and deficits. Altogether, we proposed a framework for enhancing mental health constructed by four categories (personal-reactive, environmental-reactive, personal-proactive, environmental-proactive) based on the intervention level and goal of support. All four categories have important implications, and one cannot replace the other, but expanding environmental-proactive support will allow more students to learn how to pursue health independently.We expect that this comprehensive framework for enhancing mental health could expand support systems for medical students’ personal and professional development.
