2.Prediction of response by FDG PET early during concurrent chemoradiotherapy for locally advanced non-small cell lung cancer.

PURPOSE: To evaluate the predictive value of the early response of 18F-flurodeoxyglucose positron emission tomography (FDG PET) during concurrent chemoradiotherapy (CCRT) for locally advanced non-small cell lung cancer (NSCLC). MATERIALS AND METHODS: FDG PET was performed before and during CCRT for 13 NSCLC patients. Maximum standardized uptake value (SUVmax), mean standardized uptake value (SUVmean), metabolic tumor volume (MTV), and total lesion glycolysis (TLG) were measured and the changes were calculated. These early metabolic changes were compared with the standard tumor response by computed tomograms (CT) one month after CCRT. RESULTS: One month after the completion of CCRT, 9 patients had partial response (PR) of tumor and 4 patients had stable disease. The percent changes of SUVmax (%DeltaSUVmax) were larger in responder group than in non-responder group (55.7% +/- 15.6% vs. 23.1% +/- 19.0%, p = 0.01). The percent changes of SUVmean (%DeltaSUVmean) were also larger in responder group than in non-responder group (54.4% +/- 15.9% vs. 22.3% +/- 23.0%, p = 0.01). The percent changes of MTV (%DeltaMTV) or TLG (%DeltaTLG) had no correlation with the tumor response after treatment. All the 7 patients (100%) with %DeltaSUVmax > or = 50% had PR, but only 2 out of 6 patients (33%) with %DeltaSUVmax < 50% had PR after CCRT (p = 0.009). Likewise, all the 6 patients (100%) with %DeltaSUVmean > or = 50% had PR, but only 3 out of 7 patients (43%) with %DeltaSUVmean < 50% had PR after CCRT (p = 0.026). CONCLUSION: The degree of metabolic changes measured by PET-CT during CCRT was predictive for NSCLC tumor response after CCRT.

3.Pseudo-Outbreak of Bloodstream Infections by Serratia mercescens.

BACKGROUND: Serratia marcescens proliferates well in a humid environment or soil and is recently considered as an important pathogen for the severe nosocomial infections. this organism is spreads easily by hand-to-hand transmission, and contaminates medical equipment used for invasive procedures, working environment, medications, and soap. METHODS: We investigated the source of an outbreak of bloodstream infections by S. marcescens isolated that occurred during the period from July to December, 2004, at a university hospital in Gyeonggi Province and attempted to intervene in the outbreak and control it. RESULTS: From July to December, 2004, S. marcescens grew from 296 blood culture from 283 patients. The medical charts of the patients were reviewed, and surveillance cultures were taken to identify the outbreak of nosocomial infections and risk factors. Only four cases of infection were identified and all remaining positive blood cultures were due to contamination. Nine isolates randomly selected from the 296 S. marcescens showed an identical pulsed-field gel electrophoresis pattern. To identify the source of infection, environmental culture and hand cultures of the related medical workers were carried out, but S. marcescens was not isolated. CONCLUSION: As the result of aggressive infection control activities, such as re-education on environmental management methods, hand washing techniques, and blood culture sampling techniques, no more S. marcescens had been grown in blood culture since January, 2005.

4.Trichofolliculoma: A case report.

The trichofolliculoma is a rare tumor of the hair follicle occuring most often on the head and neck. We have experienced a case of 24-year-old female who had a small, dome-shaped, tan brown firm nodule on the nose. Under the clinical impression of a granuloma, an excisional biopsy of the nodule was done. Microscopically the lesion was composed of a large central cystic hair follicle with many srhall but well differenciated secondary hair follicles branching from the wall. Well developed secondary hair follicles were often grouped and were surrounded by a prominent stroma.

5.Effects of a Qigong Prenatal Education Program on Anxiety, Depression and Physical Symptoms in Pregnant Women.

PURPOSE: The purpose of this study was to verify the effects of a Qigong prenatal education program on anxiety, depression and physical symptoms in pregnant women. METHOD: The subjects were a total of 40 pregnant women who received regular prenatal care at S hospital. Twenty people were enrolled in the Qigong prenatal education program and were assigned to the experimental group. The other 20 people who received regular prenatal care only were the control group. The Qigong prenatal education program was given to the experimental group once a week and 2 hours per session for 4 weeks. Anxiety, depression and physical symptoms were collected by a self-administered study questionnaire at the pre- and post test. RESULT: There were no significant differences in subjects' general characteristics or pretest scores of study variables at the pretest indicating both groups were homogeneous. Differential t-tests were used to test the effects of the Qigong prenatal education program on study variables. Pregnant women who received the Qigong prenatal education program had a lower level of depression than those who did not attend(t=2.23, p=.03). There were no significant differences on anxiety and physical symptoms. CONCLUSION: The Qigong prenatal education program was effective in alleviating depression during pregnancy. However, further study is needed to replicate the results with a greater sample size and to investigate the long term effects of the program on the labor and delivery process.

6.A study of upper and lower incisor position in normal occlusion.

This study was attempted to individualize upper and lower incisor position appropriate to individual characteristic dento-facial pattern. Lateral cephalometric radiographs of 75 Korean adults with normal occlusion and good facial profile whose ages were over 17 years were traced, digitized, and statistically analysed. The results of this study were as follows; 1. All cephalometric measurements of inciso, position and dento-facial pattern for normal occlusion showed wide range of variation. 2. There were no significant differences of incisor position between males and females. 3. Lower incisor position was highly correlated with ANB, N-A-Pog and SN/Mand. pl. angle and upper incisor position, with ANB, N-A-Pog and SN/Occ. pl. angle. 4. Multiple regression equations were established to individualize incisor position appropriate to individual characteristic dento-facial pattern. ANB and N-A-Pog angle were selected as the significant guiding variables affecting upper and lower incisor position. 5. Lower incisor position such as 1 to SN, 1 to occlusal plane and 1 to NPog(mm) and upper incisor position such as 1 to SN, 1 to palatal plane, 1 to NA, 1 to NA (mm) and 1 to NPog(mm) could be highly predicted from the variables of dento-facial pattern.

7.A study of osteonectin expression patterns in BAPN-induced cleft palate formed rats.

The purpose of this study was to investigate osteonectin expression patterns in cleft palatecompare to normal palate rats. We used 4 pregnant rats, and beta-aminoproprionitrile was oral dose to rat according to lg/kg body weight at gestation days 13 to induce cleft palate. Total 6 fetus was got with cleft formed, then 3 fetus was used for immunohistostain and 3 fetus was used for western blot analysis. Expression patterns of osteonectin in mesenchymal cells of cleft palate was more dilute to mesenchymal cells of normal palate with immnunohistostain, and width and length of band of maxilla in cleft palate was more thin than maxilla of normal palate with western blot study.

9.Effects of Polyamines on TNFalpha- or Tamoxifen-induced Apoptosis in Human Breast Cancer Cells.

PURPOSE: To investigate the effects of polyamines on tumor necrosis factor alpha (TNFalpha)-or tamoxifen (TAM)-induced apoptosis in estrogen receptor (ER)-positive MCF- 7 and ER-negative MDA-MB-231 human breast cancer cells. MATERIALS AND METHODS: Cell viability was assessed by using MTT assay. Reactive oxygen species (ROS) generation was measured using 2', 7'-dichlorofluorescin diacetste (DCFDA) by fluorescence plate reader. DNA fragmentation was assessed by 1.5% agarose gel electrophoresis. RESULTS: TNFalpah and TAM showed significant dose- and time- dependent inhibitory effects on the growth of MCF-7 human cells. However, the growth of MDA-MB-231 cells were not inhibited by TNFalpha or TAM treatment. The generation of ROS was increased in dose-and time-dependent manner by TNFalpha treatment in MCF-7 cells. Polyamines, especially spermine suppressed TNFalpha-induced ROS generation in MCF-7 cells. Antioxidant effects of polyamines were also demonstrated by DNA fragmentation, cell morphology as well as ROS generation assay. Polyamines also blocked TAM-induced cell death in MCF-7 cell. However, MDA-MB-231 cells showed resistance to the cytotoxic effects of TNFalpha or TAM. CONCLUSION: These results suggest that polyamines may prevent TNFalpha or TAM-induced apoptosis in MCF-7 human breast cancer cells.

10.The Correlation between the Tc2 Cells and the CCR4 Expression and the PASI Score in the Peripheral Blood of Psoriasis Patients.

BACKGROUND: Psoriasis is a chronic relapsing skin disorder that is characterized by abnormal epidermal proliferation, inflammation and angiogenesis. It causes emotional and social consequences that go far beyond the skin; therefore, many methods to measure and monitor the severity of psoriasis have been reported. OBJECTIVE: This study aims to evaluate the usability of the flow cytometric analysis of the T cell subsets and their chemokine receptors in the peripheral blood of the psoriasis patients as a severity index. METHODS: The T cell subsets and their chemokine receptor expression (CXCR3, CCR4) in the circulating blood of thirty psoriasis patients (PASI score:2.2~44.2) and twenty healthy controls were examined by flow cytometry. The relationship between the PASI score and the T cell subsets/chemokine receptors was also analyzed. RESULTS: The patients showed significantly higher number of Tc1 (CD8+CXCR3+), Tc2 (CD8+CCR4+) and CXCR3/CCR4 expressing cells than did the control group. Especially, the moderate to severe patients (a PASI score greater that 5) showed a higher number of Tc1, Tc2 and CCR4 expressing cells than did the control group. In the severe patients (a PASI score greater than 10), the frequency of circulating Tc2 cells and CCR4 expressing cells was directly correlated with the PASI score. CONCLUSION: Our findings suggest that flow cytometric analysis of the circulating T cell subsets with further classification could serve as an indicator of the disease severity in psoriasis patients.
