1.A Study on the Perceived Burden and the Quality of Life of Family Caregivers of Hemodialysis Patients.

This study was designed to identify the relationship between the perceived burden that is felt by family caregivers of hemodialysis patients and the quality of life of these caregivers. The subjects for this study were 102 family caregivers of patients who received hemodialysis regularly in the hemodialysis units of 4 hospitals located in Chonbuk. The data was collected during the period from February 17 to March 28, 1997 through interviews and by distributing a structured questionnaire to family caregivers through hemodialysis patients, and by mail service. The instruments used for this study are as follows : The burden instrument was the Burden Questionnaire developed by Lee Sook-Ja by referring to the Burden Scale developed by Novak & Guest(1989) and Zarit et al.(1980). The quality of life instrument was the Andrews' Scale that was treanslated and proof-read by Jeong Chu-Ja and partially amended by the author of this study. The collected data was analyzed by descriptive statistics, ANOVA, the Scheff test, Pearson's correlation and Stepwise Multiple Regression using the SAS PC+ program. The results of this study are as follow : 1. Hypothesis 1 : "The higher the perceived burden, the lower the quality of life of family caregivers of hemodialysis patients" was supported(r=-0.463, p=.000). 2. Hypothesis 2 : "The burden of family caregivers of hemodialysis patients will differ depending on the demographic characteristics of hemodialysis patients and family caregivers" was partially supported-age of family caregiver(F=3.04, p=.020), educational level of family caregiver(F=3.81, p=.012), marital status of family caregiver(F=5.82, p=.004), relationship with hemodialysis patient(F=2.74, p=.016), sex of hemodialysis patient(F=6.79, p=.010). 3. Hypothesis 3 : "The quality of life family caregivers of hemodialysis patients will differ depending on the demographic characteristics of hemodialysis patients and family caregivers" was partially supported-age of family caregiver(F=2.82, p=.029), educational level of family caregiver(F=4.64, p=.004), marital status of family caregiver(F=6.62, p=.002), monthly total income of family(F=5.61, p=.001), age of hemodialysis patient (F=3.42, p=.011), occupation change of hemodialysis patient(F=3.54, p=.032). 4. Stepwise Multiple Regression Analysis indicated that the six variables-the perceived burden of family caregiver, monthly total income of family, religion of family caregiver, occupation of family caregiver, marital status of hemodialysis patient, education level of family caregiver-were significantly predictive of the quality of life of family caregivers of hemodialysis patients. The six variables explained 41.7% of the variance in quality of life. In conclusion, this study revealed that the perceived burden is an important factor related to the quality of life of family caregivers of hemodialysis patients. Therefore, nurses must recognize family caregivers as well as hemodialysis patients as important care receivers and incorporate an intervening plan that reduces the burden and promotes the quality of life, taking into account the demographic characteristics of hemodialysis patients and their family caregivers.

2.A Study on the Perceiced Family Support and the Self-Esteem of the Aged.

This study was designed to provide basic data for planning nursing interventions. This was done to help them adapt effectively by examining correlations between the perceived family support and the self-esteem of the aged. The subjects for this study were the 103 aged who lived in one city located in Chonbuk. The data was collected during the period from February 20 to March 13, 1999 through an interview with a structured questionnaire. The instruments used for this study are as follows: The family support instrument was the Family Support Scale developed by Kang Hyeon-Sook(1985), used with reference to the preceding research and partially amended by the author of this study. The self-esteem instrument was the Rosenberg Scale of Self-Esteem that was translated and proof-read by Jon Byong-Je(1974) and partially amended by the author of this study. The collected data was analyzed by descriptive statistics, ANOVA, the Scheff test, Pearson's Correlation Coefficients, and Stepwise Multiple Regression using the SPSS program. The results of this study are as follows: Hypothesis 1: "The higher perceived family support, the higher the self-esteem of the aged" was supported(r=.405, p<.01). Hypothesis 2: "The degree of perceived family support of the aged will differ depending on the demographic characteristics of the aged" was partially supported-religion (F=5.428, p=.000), monthly pocket money(F=2.517, p=.035), level of pocket money(F=7.016, p=.001). Hypothesis 3: "The degree of perceived self-esteem of the aged will differ depending on the demographic characteristics of the aged" was partially supported-sex(F=6.302, p=.014), religion (F=5.697, p=.019), health perception(F=5.154, p=.007). Stepwise Multiple Regression Analysis indicated that the five variables-the perceived family support, sex, obligation to support one's family, health perception, and religion-were significantly predictive of the self-esteem of the aged. These five variables explained 42.3% of the variance in self-esteem. In conclusion, this study revealed that the perceived family support is an important factor related to the self-esteem of the aged. Therefore, when nurses plan the nursing of the aged they must include their family in order to increase the self-esteem.

3.Evaluation of Left Ventricular Function by Dynamic Exercise Echocardiography in Normal Subjects.

To determine the level of exercise for dynamic exercise echocardiography and to evaluate the changes of cardiac work loads in normal subjects, the authors performed M-mode echocardiography at rest and during 20degrees supine bicycle exercise in 10 normal subjects with good image of left ventricle. A special, self-made 20degrees supine exercise table was used on which the subject's chest could be immobilized to record echocardiograms during exercise. The recordings were always performed just below the tip of the anterior mitral leaflet. Interpretable left ventricular M-mode echocardiograms were obtained at a submaximal exercise level with mean heart rate of 132+/-21 beat/min. Left ventricular end-diastolic dimension, an index of preload, showed no significant change during exersise. But left ventricular end-systolic dimension decreased significantly(p<0.01) and % fractional shortening increased significantly(p<0.01). Peak-systolic left ventricular wall stress increased signficantly(p<0.01) during exercise but end-systolic wall stress did not. Both end-systolic wall stress/end systolic dimension ratio and systolic blood pressure/end-systolic dimension ratio increased during exercise(p<0.01). The results suggest that dynamic exercise echocardiography is a suitable method measuring various parameters of cardiac mechanics during submaximal exercise, whereas it's not available during maximal exercise.

4.Multiple Eccrine Hidrocystoma Treatedwith 1% Topical Atropine Sulfate.

We report a case of multiple eccrine hidrocystoma (MEH) in a 57-year-old woman who had asymptomatic, discrete, skin-colored, shiny, firm papulovesicles on the face. These lesions enlarged during summer or when the patient did housework, and decreased in sue during winter or rest. Histologic examination showed a dilated, unilocular, cystic, invaginated structure within the middle dermis. The wall of the cyst generally consisted of two layers of Rat or cuboida1 epithelial cells. Decapitation secretion and myoepithelial cells were not observed. The lesions improved markedly with 1% topical atropine sulfate.

5.Trichofolliculoma: A case report.

The trichofolliculoma is a rare tumor of the hair follicle occuring most often on the head and neck. We have experienced a case of 24-year-old female who had a small, dome-shaped, tan brown firm nodule on the nose. Under the clinical impression of a granuloma, an excisional biopsy of the nodule was done. Microscopically the lesion was composed of a large central cystic hair follicle with many srhall but well differenciated secondary hair follicles branching from the wall. Well developed secondary hair follicles were often grouped and were surrounded by a prominent stroma.

6.Successful Pain Management for the Postherpetic Neuralgia of Ophthalmic Nerve by Continuous Subcutaneous Infusion of Ketamine: A case report.

A 25-year-old woman transferred to pain clinic complaining of severe throbbing eye ball pain, eyelid edema and severe tearing in her left eye under the diagnosis of herpes zoster ophthalmicus that was developed 3months ago. In addtion to conventional medication, she had been taken stellate ganglion block, supraorbital and supratrochlear nerve block to contol of pain, but the response was poor. So we tried to administer ketamine subcutaneously, because not only ketamine, NMDA receptor antagonist, played a significant role to reduce neuropathic pain and pain caused by nerve injury, but also she wanted to work in her office during the treatment. We used subcutaneous ketamine 3 mg/hr (0.06 mg/kg/hr) in patient with postherpetic neuralgia of the ophthalmic nerve, and accomplished almost complete pain relief without any sign of side effect.

7.Glomangiomyoma on the Face.

We report a rare case of glomangiomyoma in a 25-year-old man who had an asymptomatic, 1.5 × 1.5cm sized, round, skin-colored, soft mass on the right cheek. Histopathologically, the lesion was consistent with glomangiomyoma which showed irregularly dilated vessels lined by endothelial cells and several outer layers of glomus cells, and smooth muscles distributed within the tumor. The tumor cells were characterized immunohistochemically by the presence of smooth muscle actin, muscle specific actin, and vimentin. To our knowledge, this is the first case report of glomangiomyoma on the facial location in the Korean literature.

8.Perception of Artificial Hydration for Terminally Ill Cancer Patients: Patients, Families and General Public.

PURPOSE: The purpose of the study was to investigate how much understand about artificial hydration in patients with terminal cancer, according to the subject groups, including patients, families, and general public. METHODS: Data were collected from June 2007 to December 2007 and the participants included 22 hospitalized patients in the hospice unit of S Hospital, 100 families, and 101 participants who participated in a hospice education program for the general public. The questionnaire was developed through literature review, interview with patients' families, and expertise consultation. Data were analyzed using descriptive statistics with an SAS program. RESULTS: Understanding of artificial hydration among patients, families and general public was examined from three perspectives. From an ethical perspective, 'if you receive artificial hydration, you can live longer', 45.5%, 63%, and 52.4% of the above three groups, respectively, answered "yes". From an emotional perspective, 'artificial hydration must be provided', 81.8%, 70% and 58.4%, respectively, agreed. From a cultural perspective, 'if artificial hydration is not provided for the patient, the families will feel painful', 95.5%, 83%, and 88.2%, respectively, answered "yes". CONCLUSION: This study found the differences in understanding of artificial hydration among patients, families and general public, and also found that less than 50 percent of the participants understood artificial hydration appropriately. We suggest, therefore, that patients' understanding about artificial hydration should be determined in the clinical setting and then followed by individualized education according to given medical situations.

9.A Clinical Study of Pelviscopic Surgery 219 Cases.

From January 1995 to December 1997, a total of 219 cases pelviscopic surgery was performed at Il Sin Christian Hospital. To evaluate the safty and efficacy of pelviscopic surgery, we reviewed indication of operation, mean age, parity, history of abdominal operation, type of operation, duration of hospitalization and complication retrospectively. The number of pelviscopic surgery have risen from 33 cases in 1995 to 53 cases in 1996 and 133 cases in 1997. The most common indications were 118 cases for ectopic pregnancy (53.9%) and 42 cases for ovarian cyst (19.2%), 20 cases for infertility (9.1%) in order. Among the ovarian cyst, endometrioma was the most common. The mean age of patients was 31+/-6.7 years old and mean parity was 0.77. In most cases, mean blood loss was lesser than 100 cc except 500 cc in LAVH, 150 cc in CISH, 106.7 cc in ectopic mass removal. The postoperative hospital stay was varied from 0 day to 10 days, but usually 2 days. The 26 cases had complicated and the most common complication was fever above 38degrees C. In conclusion, it is evident that pelviscopic surgery is lesser invasive technique, has lower complication rate and shorter the length of hospitalization. So, we expect the number and indication of pelviscopic surgery will be increased in future.

10.The Effect of Adult ADHD Tendency on Psychological Characteristics in Young Male Adults with Depression

OBJECTIVES: This study investigated psychological factors in patients with adult ADHD tendency and the relationship of adult ADHD tendency with depression and anxiety.METHODS: Ninety patients with depression(38 with adult ADHD and 52 with non-adult ADHD) were recruited in this study. All patients were subjected to the following tests : Korean Wechsler Adult Intelligence Scale-IV(K-WAIS-IV), Adult Attention-Deficit/Hyperactivity Disorder Self-Report Scale(ASRS-v1.1), Lee Ji-Yeon's adult ADHD scale, Beck's Depression Inventory II(BDI-II), Beck's Anxiety Inventory(BAI), and Temperament Character Inventory-Revised Short version(TCI-RS).RESULTS: The adult ADHD group had higher scores for BDI-II and BAI, novelty-seeking, harm-avoidance, and self-transcendence than the non-adult ADHD group. Conversely, the adult ADHD group showed lower scores for self-directedness and cooperativeness than the non-adult ADHD group. Depression scores were positively correlated with ASRS(r=0.524, p=0.000), Lee Ji-Yeon's adult ADHD scale(r=0.598, p=0.000), anxiety scores(r=0.650, p=0.000), novelty-seeking(r=0.411, p=0.000), harm-avoidance(r=0.517, p=0.000), and self-transcendence(r=0.234, p=0.026). However, they were negatively correlated with persistence(r=−0.231, p=0.029), and self-directedness(r=−0.594, p=0.000). Higher Lee Ji-Yeon's adult ADHD scale scores and lower self-directedness scores among TCI, resulted in higher depression scores. In addition, anxiety scores increased in parallel with the the Lee Ji-Yeon's adult ADHD scale and harm-avoidance TCI scores.CONCLUSION: In this study, we found a difference in the severity of depression and anxiety between the adult and non-adult ADHD groups. In addition, it was confirmed that the difference in temperament and character between the two groups was related to adult ADHD tendency and severity of depression and anxiety.
