1.The Effects of Obesity Stress, Weight Bias, and Heath Care on BMI in Soldiers of Non-combat Area.

PURPOSE: The purpose of this study was to identify the obesity stress, weight bias and health care on Body Mass Index (BMI) in soldiers of non-combat area and to provide data for improving the quality of their life. METHODS: This research involved 165 soldiers working in non-combat area. Data collection was conducted from November 1 to 20, 2015. Statistical analysis of the collected data were t-test and ANOVA, Scheffé method post hoc analysis, Pearson's correlation coefficients, and multiple liner regression using IBM SPSS 22.0. RESULTS: The mean score of obesity stress was moderate (19.05±5.28). The mean score of weight bias was 69.03 and health care was 2.41 points. There are a positive correlation between obesity stress and BMI (r=.19, p<.05). Weight bias (r=-.19, p<.01) and health care (r=-.26, p<.01) among the subjects had negative correlations with BMI. In a multiple liner regression, obesity stress (β=.18, p<.05), health care (β=-.18, p<.05) were associated with BMI. CONCLUSION: Based on the findings that obesity stress and health care influence BMI, there is a need to control stress and to properly set proper guidelines on health care for soldiers.

2.Immunohistochemical study of p21 and p53 expression in ameloblastoma.

The p53 protein was discovered in 1979 as cellular 53-kD nuclear phosphoprotein bound to the large transforming antigen of SV40 virus. P21WAF1/CIP1, which has been described as the critical downstream mediator of p53, is known to suppress DNA replication and arrest the G1 cell cycle by quaternary complex with cyclin D, cyclin-dependent kinase(CDK) and proliferating cell nuclear antigen(PCNA). In these days, some studies shows that the p21 can be induced by independent pathways. There are various reports about the expression of p21 (67%.82.4%) in oral squamous cell carcinoma. But these studies are mostly done in malignant tumor not in benign tumor. So we decided to study the expression of p21 in ameloblastoma and the relationship between p53 and p21 as a downstream mediator of p53 in ameloblastoma. We investigated the expression of p21 and p53 with the method of immunohistochemistry. We selected 30 cases of ameloblastoma tissue blocks (acanthomatous type: 5 cases, follicular type: 8 cases, plexiform type: 17 cases) imbedded in paraffin. We used 30 cases of normal gingival tissues and 30 cases of squamous cell carcinoma tissues (SCC) respectively and compared their results with those of ameloblastoma. We made slides with the streptavidin-biotin methods and used monoclonal antibody DO-7 (Novocastra, Newcastle, United Kingdom) as p53 antibody and monoclonal antibody M7202 (DAKO, California, U.S.A.) as p21 antibody. We used Pearson's correlation coefficient to analyse the relationship. The results were as follows: 1. p21 was expressed in ameloblastoma about 30% and this is lower than that of normal gingiva and SCC. 2. In normal gingiva and ameloblastoma, p21 expression was correlated with p53 expression. 3. In SCC, p21 were expressed about 83.3% and this is more than that of p53. But there was no correlation between p21 and p53 expression. We confirmed p21 expression and relation with p53 in ameloblastoma. But, to confirm the function of p21, more studies about p21 expression in malignant ameloblastoma and ameloblastic carcinoma are needed.

4.One Case of Quadriplegia Resulted from Cervical Penetration Wound.

A case of quadriplegia resulted from cervical penetrating wound was admitted to chosun university hospital intensive care unit, and undergone steroid therapy and antibiotics therapy. On the 6th hospital day, the septic complication was developed and it proved to be bacterial meningitis by CSF examination. Broad spectrum antibiotics was administered immediately after the diagnosis and the patient was monitored carefully. The bacterial meningitis was improved and no other serious complication was developed. The patient was discharged with permanent neurologic deficit, quadriplegia.

Apoptosis is a form of cell death in which the cells shrink and exhibit nuclear chromatin condensation and DNA fragmentation, and yet maintain membrane integrity. Many lines of evidence have shown that brain neurons are vulnerable to degeneration by apoptosis. Also it has been suggested that apoptosis is one of the mechanism contributing neuronal loss in Alzheimer's disease(AD), since the conditions in the disease(A beta peptide, oxidative stress, low energy metabolism) are the inducers that activate apoptosis. Indeed some neurons in vulnerable regions of the AD brain show DNA damage, chromatin condensation, and apoptic bodies. Consistently, mutations in AD causative genes(Amyloid precursor protein, Presenilin-1 and Presenilin-2) increase A beta peptide1-42(Abeta1-42) and sensitize neuronal cell to apoposis. However, several lines of evidence have shown that the location of neuronal loss and A beta peptide deposition is not correlated in AD brain and transgenic mice brain over-expressing Abeta1-42. Taken together, these data may indicated that A beta peptide(and other causative factors of AD) can interact with other cellular insults or risk factors to exacerbate pathological mechansim of AD through apoptosis. Thus, this review discusses possible role and mechanism of apoptosis in AD.

6.Prevalence of HCV infection in pregnant women and vertical transmission.

OBJECTIVE: The HCV seroprevalence rate and the vertical transmission rate in Korean pregnancy women have not been determined until now. The purpose of this study is to estimate the prevalence of anti- HCV in pregnant women and vertical transmission rate to assess the role of antenatal screening of HCV and the necessity of following babies from anti-HCV positive mothers. METHODS: We reviewed the medical records of 21,639 patients who delivered at Ilsin Christian hospital from January 2000 to October 2003. The HbsAg, HbsAb, anti-HCV, HIV test were performed in their 1st trimester of pregnancy and the HCV RNA test was performed by RT-PCR in samples positive for anti-HCV. 38 babies born to anti-HCV positive mothers were tested for anti-HCV and those who were positive anti-HCV were also tested for HCV RNA by PCR. RESULTS: The positive rate of anti-HCV in pregnant women was 0.42% (90/21,639) and that of HCV RNA was 57% (51/90). The positive rate of anti-HCV was 26.3% (10/38) and the result of HCV RNA test was all negative in babies whose mothes were positive for anti-HCV. CONCLUSION: The prevalence of anti-HCV in pregnant women who delivered at our hospital seems to be higher than that in the Korean young adult man and it is similar with the whole world prevalence rate. The prevalence of anti-HCV of babies born to HCV PCR positive mothers was higher than that born to HCV PCR negative mothers. None of anti-HCV positive babies was HCV RNA positive. With this, we conclude that the vertical transmission as the transmission route of HCV infection is negligible but routine screening for HCV is acceptable to pregnant women.

Complete trisomy 9 is a rare chromosomal aneuploidy in live born infants. The majority of cases of trisomy 9 end in spontaneous abortion in the first trimester. Clinical finding of co-mplete trisomy 9 demonstrate multiple organ abnormalities in the craniofacial, cardiovascular, skeletal, genitouronary systems. We report a fetus with Dandy Walker syndrome which was diagnosed prenatally and was subsequently found to have a complete trisomy 9.

9.Analysis of Judicial Precedents on Drug Eruption in South Korea

Background:
Both medicolegal disputes and the incidence of cutaneous drug eruptions are increasing in Korea. We were unable to find research that surveyed the legal disputes involving drug eruption cases.
Objective:
This study investigates medical litigation associated with drug eruption cases in Korea.
Methods:
Judicial precedents on drug eruption cases were searched using the Supreme Court of South Korea’s Written Judgement Management System. General characteristics, results, suspected agents, and recognized negligence were analyzed.
Results:
From the search results, 50 cases were selected. Of these, 34 cases had severe cutaneous adverse reactions, including Stevens-Johnson syndrome/toxic epidermal necrolysis (n=25, 50.0%), and drug rash with eosinophilia and systemic symptoms syndrome (n=9, 18.0%). Antimicrobial agents (n=17, 34.0%), non-steroidal anti-inflammatory drugs (n=6, 12.0%), and anticonvulsants (n=6, 12.0%) were the most common drugs implicated. Death was reported in 15 patients (30.0%). In this sample, 22 cases (47.0%) were awarded to the plaintiff, and violation of the duty to inform patients of risks was the most common legal issue cited.
Conclusion
Clinicians should be aware of and advise patients of the potential for severe adverse reactions that can lead to negative outcomes and medicolegal disputes.
10.Measurement of Mucin Amounts Using RTO3 in Patients with Chronic Airway Disease.

BACKGROUND: It has been anticipated that the amount and composition of mucin are changed in patients with chronic airway diseases. We evaluated whether RTO3 (mAb against rat tracheal mucins) could que ntify the amount of mucin from the airway in the patients with chronic airway diseases. METHODS AND RESULTS: 1) RTO3 was bound to high molecular weight of mucin based on Western blot in sputum and BALF from patients with chronic airway diseases. 2) The goblet cells and submucosal glands in main bronchus from human were observed by PAS stain. And immunohistochemical stain with RTO3 showed immunoreactivity on some goblet cells. 3) The amount of mucin was more increased in patients with chronic airway diseases compared to those in normal subjects. 4) In the exacerbation of asthmatics, mucin amounts were more increased than stable asthmatics. CONCLUSION: We suggested that secreted mucin in chronic airway diseases can be quantified by ELISA with RTO3.
