1.A Case of Fibrous Hamartoma of Infancy.

Fibrous hamartoma of infancy (FHI) is an uncommon, benign, solitary intradermal or subcutaneous tumor. It occurs typically in the axillary or shoulder region. The histopathologic examination of affected lesion shows the characteristic elements: dense fibrous tissue, adipose tissue, and primitive mesenchymal tissues. A 15 month-old girl had the multiple, asymptomatic, discrete, and skin-colored tumors that scattered on the scalp. These were present at birth. We report a rare case of FHI occurred on the scalp with multiple and congenital characteristics.

2.A Case of Infantile Digital Fibromatosis.

We report a case of infantile digital fibromatosis in a 34 month-old boy, who presented with a painless subcutaneous tumor on the medial aspect of the left third toe. A histological examination showed scattered small, round eosinophilic inclusion bodies in the cytoplasm of the tumor cells, which was consistent with infantile digital fibromatosis. A'immunohistochemical study revealed that desmin, a-smooth muscle actin, and vimentin were clearly positive in the cytoplasm of the tumor cells, but the inclusions themselves showed negative staining, thus indicating a hollow-like staining pattern. Electron microscopy showed either well-defined or ill-demarcated dense bodies in the cytoplasm of the tumor cells. In some areas, small vesicles and intracellular organells were observed' in the inclusions. In the course of conservative treatment, a new lesion developed on the lateral aspect of the left third toe, seven months after the appearance of the initial lesion.

3.Sebaceous Adenoma in Medial Caruncle of the Eye: A case report.

Sebaceous adenoma is a rare solitary or multiple yellow, circumscribed neoplasm consisting of sharply demarcated, proliferating lobules or irregular size and shape that are composed of three cell bypes: basal cells, mature and transitional sebaceous cells. We reported a case of extremely rare sebaceous adenoma arising in caruncle of the left eye in a 78-year-old female. A polypoid tumor nodule, measuring 0.6x0.5 cm, was noticed 6 months before resection. It was grayish white with granular surface. There is no evidence of associated visceral neoplasm. Histopathologically, it revealed a lobulated tumor, composed of equal amount of highly differentiated sebaceous and intermediate transitional epithelium with minor participation of basaloid cells. This tumor is thought to be the neoplasm of the sebaceous gland normaly present in the caruncle of the eye rather than ectopic origin.

4.A Clinical Study of the Femur Shaft Fracture in Children Treated by 90

Authors has analyzed 75 cases of the femoral shaft fractures in children, treated by 90-90 skeletal traction methods at Jung Ang Gil General Hospital during last six years, from March 1982 to April 1988. The results were as follows :1. The 90-90 Skeletal traction can be widely applicable to the age group ranging from 3 to 13 years, while other traction methods have a certain age limits. 2. Maintenance of initial reduction snd correction of angular and rotational deformity of femoral shaft fracture were easily achieved. In addition, later angular deformity can also be easily corrected. In angular deformity and instability type, the correction and maintenance of deformity and instability were helped by use of 2 cast slabs. 3. Follow-up observation as well as wound care of open fracture and associated soft tissue injury were relatively easy. 4. The period of immobilization necessory after fracture was not longer than the other methods of treatment, and during treatment, evaluation of bony alignment and union was possible by direct palpation and inspection with out the help of X-ray. 5. The limitation of knee motion as well as circulatory and neurogenic complication were not found. Therefore, these results revealed that 90-90 skeletal traction was the easy, safe and effective method in treatment of femoral shaft fractures in childeren.

5.Radiologic Findings of Meconium Aspiration of the Newborn

Authors reviewed 264 cases of meconium aspiration pneumonia of the newborn at Sohwa Children's Hospital from July 1981 to June 1984. The radiologic findings were retrospectively analysed with particular attention to the degree of pulmonic infiltrations. The results were as follow: 1. The male to female ratio was 1.8:1 and clinical conditions which commonly associated were postterm infants (41.3%), placental dysfunction syndrome (33%) and perinatal asphyxia (11.4%). 2. There were improved (76.9%) and expired cases (12.1 %) as clinical course. 3. Radiologic findings were noticed .as pulmonic infiltration (68.2%), hyperinflation (35.2%), pneumomodiastinum (18.6%), pleural effusion (14.4%), pneumothorax (13.3%) and suggestive interstitial emphysema (6.8%). The more the degree of pulmonic infi ltration was been severe, the more the incidence of other pulmonic lesions was increased. 4. Pulmonic infiltrations were commonly occured in both lungs but pneumothorax, pleural effusion and hyperinflation in right. 5. Radiologic findings of 32 expired cases were noticed as pulmonic infiltrations (93.8%), hyperinflation (53.1 %), pneumomediastinum (37.5%), pneumothorax (18.8%), suggestive interstitial emphysema (18.8%) and pleural effusion (18.8%).

7.A Case of Nevus Sebaceus Associated with Trichilemmoma.

We reported a case of nevus sebaceus associated with trichilemmoma in a 19-year- old female patient. The lesion was a 2 x 2 cm sized yellowish verrucous plaque with two 0.3 x 0.3 cm sized dome-shaped papules. Histopathological examinat,ion revealed hyperkeratosis, papillomatosis and acanthosis, proliferation of sebaceus glands, presence of apocrine gland and mild inflammatory cellular infiltration around superficial vessels. In the epidermis of the lesion, there were two foci of lobulated mass consisted of clear cells with columnar basal cells and thick basement membrane.

8.A Case of Cutaneous Metastasis from Esophageal Carcinoma.

We report a case of cutaneous metastatic carcinoma from an esophageal carcinoma in a 54-year-old male who complained of several pinhead-sized erythematous tender papules on the up-per chest. He had been admitted to the department of internal medicine for esophageal carcinoma. Radiological examinations revealed a luminal narrowing below the level of carina on esophagography, and a mass on the mid esophagus, left atrium and pulmonary vein on the chest CT scan. On endoscopy, about 30cm from the central incisor, a huge polypoid mass with a central ulceration and dirty surface margin was noted. Histopathological examination of the skin and esophageal biopsies showed the same atypical squamous cells with distinctive nucleoli, nuclear membrane and intercellular bridge, which were consistent with the squamous cell carcinoma of nonkeratinizing large cell type, suggesting these cutaneous lesions were metastasized from the esophageal carcinoma.

9.A Case of Acral Lentiginous Melanoma.

We report herein a case of acral lentiginous melanoma in a 65-year-old female. An about 2x3cm sized erythematous to brownish mass with central ulceration was located on the left heel accompanied by enlargement of the left inguinal lymph nodes. Histopathologic findings showed lentiginous proliferation of atypical melanocytes with pseudoepithliomatous hyperplasia and invasion of tumor cells from the epidermis into the dermis. Focal pigmented melanocytes were also noted. Complete surgical excision and chemotherapy with DTIC were done. Marked edematous swelling on the left lower leg was developed after 8 months and 1 month later she was expired.

10.A Pulmonary Atelectasis due to Nasal Bleeding after Nasotracheal Intubation: A case report.

A 20 years old male patient was transferred to the operating room for mandibulo-maxillary reconstruction. We planned general anesthesia with nasotracheal intubation. Reinforced endotracheal tube was inserted through right nasal os and progressed to the epiglottic region. But the first trial was not successful because of the profuse bleeding from the nasal cavity. After suctioning, the second trial through the same route was successful. But at the time of skin incision, we found the lips of the patient were becoming pale, the SpO2 was falling down to lower than 70%. We found the patients left chest wall was motionless. The chest A-P film showed total atelectasis of the left lung. When a large blood clot was removed by the flexible bronchoscopy, the chest wall started to move well and the SpO2 came up to 99%. The following chest A-P film confirmed the resolution of the atelectasis.
