1.Leiomyoma of the Urinary Bladder.

Leiomyoma is commonly found in the female genital tract, but occurrence in the urinary bladder is very rare with only 235 cases reported in the literature. These tumors have been classified as intravesical (63%), intramural (7%) and extravesical (30%) depending on the direction of the growth. We report a case of intravesical leiomyoma of the urinary bladder in a 36 year-old woman who exhibited dysuria and urinary retention. The gross and microscopical findings of leiomyoma of the bladder are similar to those of the uterus. Immunohistochemical stains for estrogen receptor (ER) and progesterone receptor (PR) revealed diffuse nuclear staining in smooth muscle cells, supporting the hypothesis of hormonal influence in tumorigenesis.

2.Wegener's Granulomatosis Involving Lung and Middle Ear: A case report.

A case of Wegener's granulomatosis is described, with special attention focused on the typical histologic findings and involvement of both middle ear and lung. The patient is a 37-year-old man presented with four-month history of cough and sputum. He had a past history of surgery of both ears because of otitis media followed by left facial palsy. Chest radiographs showed variable sized ill defined nodules in both lower lobes with internal airspace consolidation. Histologic preparations of the open lung biopsy specimens demonstrated a diffusely scattered palisading micro and macrogranulomas with central focus of neutrophils and necrotic collagen surrounded by histiocytes, histiocytic giant cells. Fibrinoid necrosis involved blood vessels and lung parenchyma. Chronic inflammation, diffuse granulation tissue formation and irregular fibrosis are also found in the lung parenchyma. The histologic findings of middle ear which was previously biopsied showed scattered palisading ill defined microgranulomas mixed with fibrotic tissue.

3.Effects and Significance of Cyclosporine Therapy in Chronic Actinic Dermatitis.

BACKGROUND: Chronic actinic dermatitis comprises a spectrum of chronic photosensitivity disorders. Treatment includes avoidance of UV light, application of broad-spectrum topical sunscreens, PUVA therapy, corticosteroid, azathioprine and cyclosporine. OBJECTIVE: Our purpose was to determine the efficacy of cyclosporine in the treatment of chronic actinic dermatitis. METHODS: Six patients with chronic actinic dermatitis refractory to conventional treatment were treated with cyclosporine 100-200mg a day for four to eighteen weeks. RESULTS: In all six patients improvement of the skin lesions and itching were dramatic, but in three of them hyperterision developed during the cyclosporine treatment. After stopping the cyclosporine therapy, their blood pressures normalized within two to five weeks. Other side effects of cyclosporine were not found. Although the skin lesions of all of the six patients were aggravated more or less after stopping the cyclosporine therapy, we could maintain their improved states with topical corticosteroids and oral antihistamines. CONCLUSION: 1. Cyclosporine is a good alternative in treating chronic actinic dermatitis patients who are suffering from severe symptoms refractory to conventional therapy. 2. Hypertension is the frequent side effect of cyclosporine.

4.A clinical study of throat culture in pediatric patients.

Infants and children of 227 cases with respiratory diseases and 743 cases with non-respiratory disease admitted to pediatric department of NMC for 3 years, from Sept. 1974 to Aug. 1977 were taken throat swabs for bacterial cultures and the results were evaluated for various disease state. The following results were obtained: 1. Total number of bacteria isolated from respiratory and non-respiratory patients were 87 and 168, respectively. 2. Age distribution of cluture revealed 52% of positively cultured patients were below 6 months of age and 65% were less than 1 year of age. 3. Frequency of bacterial isolation were as follows: In respiratory disease, Staphylo. Aureus, Klebsiella, E.Coli, and D.Pneumoniae were isolated in order of frequency. In non-respiratory disease, Staphylo. Aureus, Staphylo. Epidermidis, Klebsiella, D.Pneumoniae, and E.Coli were isolated in order of frequency. 4. Bacterial sensitivity to several antibiotics showed same rate of sensitivity to same kind of antibiotics on the both respiratory and non-respiratory patients. The concept that bacteria from throat swab does not accurately represent the pathogenic organism in the lower respiratory disease should be kept in mind in interpreting the results of the throat culture.

5.A Case of Pulmonary Histiocytosis-X Associated with Bilateral, Recurrent, and Spontaneous Pneumothorax.

Histiocytosis-X is a term used to define three diseases with similar morphologic characteristics : Letterer-Siwe diseae, Hand-Schuller-Christian disease and Eosinophilic granuloma. In general, they differ in terms of their age of onset, severity of clinical course and site of involvement. Eosinophilic granuloma typically is seen in young adults. Eosinophilic granuloma is diagnosed in the presense of diffuse pulmonary infiltrate, bony involvement. However, the Pulmonary radiologic findings of eosinophilic granuloma are variable accordinary to stage of disease. therefore pathologic diagnosis of involving site is essential for confirmative diagnosis of eosinophilic grananuloma. Pathologically. the three disease are characterized by granulomatous infiltration of alveolar septa and bronchial walls and often involvement of bone. The hallmark of this disease is proliferation of the Langerhans' cell. The identifying feature is the X-body or Birbeck granule that is present in Langerhans' cells and histiocytic cells found in the lung of EG patient. We report a case of bilateral, recurrent and spontaneous pneumothoraces in a 21 year old man with pulmonary histiocytosis-X which is confined by eosinophilc granuloma in bone marrow biopsy and ultrastructural examination in cells obtained from BAL.

6.A Case of Mixed Long Carcinoma Composed of Three Cell Type.

Carcinoma of lung is classified by histologic cell type, this is based on predominant major cell population by tissue specimen. But in fact a range of 13% to 63% of lung cancer has been appeared to be heterogeneous cell type at the light microscopic level. After treatment of the mixed lung cancer and/or with time there are reports that cell type is changed. Because it is possible that the tumor were pleomorphic and that the antitumor therapy eliminated the more sensitive cell population and permitted the subsequent emergence of the more resistant cell population. The authors reported here, a case of 32 year old female patient with mixed lung cancer composed of three cell type, and this is the first case in Korea.

7.Refractory pseudomembranous colitis that was treated successfully with colonoscopic fecal microbial transplantation.

Pseudomembranous colitis (PMC) is a nosocomial and opportunistic infection caused by Clostridium difficile. PMC is related to the use of antibiotics leading to intestinal dysbiosis and an overgrowth of C. difficile. Metronidazole or vancomycin is considered to be the standard therapy for the management of PMC. However, PMC has a 15%-30% recurrence rate and can be refractory to standard treatments, resulting in morbidity and mortality. Here we describe a patient who experienced refractory PMC who was treated with fecal microbiota transplantation. A 69-year-old woman was admitted to the hospital with consistent abdominal pain and diarrhea, which had been present for 5 months. She was diagnosed with PMC by colonoscopy and tested positive for C. difficile toxin. Even though she took metronidazole for 10 days, followed by vancomycin for 4 weeks, her symptoms did not improve. Because of her recurrent and refractory symptoms, we decided to perform fecal microbiota transplantation. Fifty grams of fresh feces from a donor were obtained on the day of the procedure, mixed with 500 mL of normal saline, and then filtered. The filtered solution was administered to the patient's colon using a colonoscope. After the procedure, her symptoms rapidly improved and a follow-up colonoscopy showed that the PMC had resolved without recurrence.

8.Body mass index, waist-to-hip ratio, and metabolic syndrome as predictors of middle-aged men's health.

PURPOSE: There is no reported evidence for an anthropometric index that might link obesity to men's sexual health. We evaluated the ability of an anthropometric index and the symptom scores of five widely used questionnaires to detect men's health problems. We determined the predictive abilities of two obesity indexes and other clinical parameters for screening for lower urinary tract symptoms and sexual dysfunction in middle-aged men. MATERIALS AND METHODS: A total of 1,910 middle-aged men were included in the study. Participants underwent a detailed clinical evaluation that included recording the symptom scores of five widely used questionnaires. The participants' body mass index and waist-to-hip ratio were determined. Serum prostate-specific antigen, urinalysis, testosterone, estimated glomerular filtration rate, evaluation of metabolic syndrome, and transrectal ultrasonography were assessed. RESULTS: By use of logistic regression analysis, age and total prostate volume were independent predictors of lower urinary tract symptoms. Metabolic syndrome was the only significant negative predictive factor for chronic prostatitis symptoms. Age and metabolic syndrome were independent predictive factors for erectile dysfunction. Waist-to-hip ratio had a statistically significant value for predicting erectile dysfunction. CONCLUSIONS: Our data showed that total prostate volume is a significant predictor of lower urinary tract symptoms, and central obesity has predictive ability for erectile dysfunction. Metabolic syndrome was the only significant negative predictive factor for chronic prostatitis-like symptoms. The management of correctable factors such as waist-to-hip ratio and metabolic syndrome may be considered preventive modalities against the development of men's health problems.

9.Studies on Hypertension in Korea: I. Clinical Study.

An analytical observation was carried out on clinical features manifested by 435 patients admitted to Sacred Heart Hospital under a diagnosis of hypertension for a period of June 1968 to August 1972. The following results were obtained: 1. The occurence rate of hypertension was like to be higher in males than in females, showing a ratio of 3:2. The group of the 6th decade of age showed the densest distribution followed the groups in the 5th and the 7th decades, in either sex. 2. The average measurement of blood pressure in patients with hypertension for less than 2 years of duration was 174.8+/-25.14/114.0+/-18.72 mmHg. One who had suffered for over 10 years showed 189.7+/-32.02/126.6+/-23.95mmHg. The value had a tendency to increase as the ailment duration prolonged. 3. In eye fundus findings,hypertensive retinopathy of K-W stage II was the most frequent, observed in 39.0% of all. The appearing rate of hypertensive retinopathy increased and the change in the eye ground was accentuated as the ailment lingered longer. There was no noticeable correlative evidence between serum cholesterol and retinopathy by K-W classification, while the appearing rate of arteriosclerosis in the eye ground by Scheie's classification somewhat increased as serum cholesterol elevated. 4. Albuminuria was observed in 62.9% of all cases, a tendency of a higher appearing rate in parallel with the protraction of illness and a higher measurement of blood pressure. 5. The appearing rate of albuminuria increased as the change of eye fundus deepened. 6. The mean value of serum cholesterol was 192.0mg%, and there was no difference in cholesterol determination by age. 7. The ECG revealed an abnormal tracing in 78.0% of all cases, of which LVH was the most common abnormality appearing in 62.3%. The abnormalities in ECG were found more frequently as the illness protracted longer and blood pressure measured higher. 8. The chest X-ray showed abnormal findings in 55.6% of all cases, of which cardiomegaly was the most common change evidenced in 43.9%. 9. The cerebrovascular accident(22.9%), cardiac failure(8.6%) and renal insufficiency(7.9%) were the major complications of hypertension. The cerebrovascular accident was also found in higher occurence in the cases of longer duration of ailment with hypertension.

10.The Effect of Hand Moxibustion Therapy on Pain during Menstruation.

PURPOSE: The purpose of this study was to identify the effect of hand moxibustion for reduction of pain during menstruation. METHOD: Fifteen women were selected as subjects and data collection was conducted from September to December 2003. The Visual Analogue Scale (VAS) and Digital Infrared Thermotraphic Image (DITI) were used to measure the degree of pain and body heat. RESULT: The results showed that there was a significant reduction of pain during menstruation when hand moxibustion was performed. Also, there were significant differences in the change of body heat. These were left hands (Z=-2.805, p=.005), right hands (Z=-2.805, p=.005) and lower abdomen (Z=-2.803, p=.005). CONCLUSION: It was found that hand moxibustion decreased pain during menstruation and increased body heat. Therefore, hand moxibustion may be a good method for intervention to decrease pain during menstruation.
