1.A Solitary Glomangioma:An Immunohistochemical and Electron Microscopic Study.

A 62-year-old man presented with a tender, solitary nodule on the left upper arm. He had no specific family history of the lesion. A biopsy specimen showed various sized irregular vascular lumens in the upper dermis. The vascular walls consisted of an endothelial cell layer and rim of one to three layers of glomus cell nests outside the endothelial cells. The cell nests consisted of round shaped epitheloid cells with relatively uniform round or oval shaped nuclei and pale cytoplasm. These tumor cells were strongly positive for vimentin, and a-smooth muscle actin. On electron microscopic examination, the cytoplasm of these tumor cells contained fine filamentous components and many electron dense bodies were found at the plasma membrane and the cytoplasm. We report a case of a solitary glomus tumor(glomangioma) that showed the histological features of the multiple type of glomus tumor. In addition we describe the results of an immunohistochemical and electron microscopic study.

4.A Study of Mucocutaneous Manifestations Due to Cancer Chemotherapy.

BACKGROUND: Recently, inireaing numbers of chemotherapeutic agens are being used to treat cancer patients. Mucocutaeous complications are commonly sen in association with the administration of these medicaticins. OBJECTIVE: We conducted a study in cancer chemotherapy patient so determine the suspected chemotherapeutic agents ancl frequency of various mucocutaneous side effects in these patients. METHODS: The study involved 140 patients admitted from Decerrae 1993 to September 1994 for cancer chemot herapy at Korea University Guro Hospital. RESULTS: The mucocutaneous side effects during chemotherapy were alopecia(55%), hyperpig mentation(32.9%), stomatitis(20%), phlebitis(12.9%), flushing(8.6%) descending order. Visual grade II, IV alopecia patients were most common, but as the chemot retpy cycle increased so, the severer the alopecia. Although the onset of the alopecia were variake many patients experienced alopcia within 17 to 24 days after the start of chemotherapy. Serpeitie supravenous fluorouracil hyperpigmentation were commor,(17.9%), Diffuse and longitudinal or horizontal band like nail pigmentation were observed, and, pigmented macules were also observei, epecially on the palms and digits. Stomatitis usually developed within 10 days after the start of chemotherapy and it persist ed for about 7 days and the most of the lesions were resolved spor Laeously. Other clinical manifestations accompanied with chmotherapy were tinea infection(16.4%), acne(7.1%), oral thrush (7%), white nail band(4.3%), pruritus(2.9%) in descending order. CONCLUSION: So, physiciars need to be aware of the widely divergnal cutaneous reactions which may occur with the use of antiancer medications.

5.Early Detection of Anesthesia Machine Malfunction with Capnography during General Endotrachial Anesthesia.

BACKGROUND: Mechanisms of secondary injury (post-ischemic injury) in the central nervous system have cently reported in a vast of amount of experiments. Among many factors which give rise to post-ischemic neuronal damage, glial deterioration probably mediated by calcium paradox, could be another of the aggravating deleterious factars to the already ischemic neurophil. METHODS: Here we have designed experiment to investigate calcium paradox in astroglial cell line, humsn asttocytoma U1242MG. Intracellular calcium alterations in experimental cells were monitored by using calcium indicating dye fura-2 and epifluorescent photometry system. RESULTS: Intracellular free calcium changes during reperfusion phase after exposure to low calcium led to a prampt increase in intracellular calcium level after 10 and 30 minutes. The way of calcium entry during the reperfusion phase was mediated by the revase mode of Na+/Ca(2+) exchanger. Cells that had a reduction of reperfusate calcium to 10 uM increased cell viability. Also we observed an inverse relationship between major enzymatic activity in the astrocytoma cells (i.e., glutamine synthetase activity) and the duration of reperfusion in the the same protocols. CONCLUSIONS: A relatively small amount of intracellular calcium increase by the reverse mode of Na+/Ca(2+) exchanger during the reperfusion period is related to a limitation of enzyme activity and viability 24 hours later.

6.Effects of Frozen Gauze with Normal Saline and Ice on Thirst and Oral Condition of Laparoscopic Cholecystectomy Patients: Pilot Study.

PURPOSE: The purpose of this study was to examine the effects of using gauze frozen with normal saline or ice on thirst-relief and oral condition of laparoscopic cholecystectomy patients. METHODS: A quasi-experimental nonequivalent control group, pretest-posttest design was used. Participants (n=53) received either gauze frozen with normal saline (n=17), ice (n=18) or wet gauze (n=18) for thirst-relief. The subjective thirst level and oral condition of the participants were assessed before the intervention, 15 min after the first intervention and 15 min after the second intervention. RESULTS: After oral care was provided twice, there were significant differences in thirst level among the groups. When oral care was provided twice, the oral condition of tongue, saliva, mucosal membrane, and gingiva was improved in patients receiving gauze frozen with normal saline or ice. CONCLUSION: Gauze frozen with normal saline and ice can be effective for oral care in reducing the thirst level and improving the condition of the oral cavity.

7.The Suppressive Effect of Evening Primrose Oil on Murine Contact Sensitivity.

BACKGROUND: Evening primrose oil(EPO) is a rich source of cis-linoleic acid and gammalinolenic acid(GLA) and has been used as a therapeutic agent in various skin diseases such as atopic dermatitis. OBJECTIVE: The purpose of this study was to evaluate the suppressive effect of EPO on murine contact sensitivity. METHODS: BALB/c mice were divided into 3 groups, positive control, experimental and negative control groups: the positive control group represents a group of mice which were sensitized and challenged with DNFB, the experimental group represents EPO-pretreated positive control group and the negative control group represents a group of mice which were challenged only. The changes of ear thickness were measured, and H & E staining and immunohistochemical staining for ICAM-1 expression of ear skin were performed to evaluate the histological changes of each group. RESULTS: The Pretreatment of mice with EPO resulted in suppression of contact sensitivity by more than 82%. On H & E staining, only a mild inflammatory reaction was observed in the dermis. Also ICAM-1 expression of keratinocytes, the intensity of the staining was significantly decreased in the experimental group compared with positive group. CONCLUSIONS: Our study indicates that EPO was able to suppress the induction of contact sensitivity.

9.Implementation of the Bitmap Image File format for Dental Image Management.

Generally, PACS(Picture Archiving and Communication System) play an important role in the patient care support system of the HIS(Hospital Information System). In the dental PACS, image search and storage is very difficult without standardized DBI(Dental Bitmap Image) file format and view browser because each of the X-Ray instrument and the other image use various image file such as BMP, GIF, TIFF, PCX etc. We proposed the DBI for fast review, classification, storage and transmission of the dental image. It is based on personal computer environment. Using the thumbnail in the DBI, we made image files such as a type of album and operated on PC. We implemented DBI file format that include the DBI viewer and slide show application and other function of fundamental PACS by means of visual C++ and Windows API(Application Programming Interface). This program is consisted of three parts: First, convert the various image file format into DBI file format so as to integrated medical image file format. Second, additional descriptor such as patient name or necessary information to image file and management image file format. Second, additional descriptor such as patient name or necessary information to image file and management image file using patient information or date. Third, execution of the image of the image edit and image enhancement and image copression. Beside this Program Provide GUI interface for user's facility.

10.Development of Family Health Management System in Family Medicine.

With improvement of life quality, the incidence of disease and the concern about preventive care is increasing. With the concern about medicine is changed from therapy medicine to preventive medicine several type of life long health care program is developed. In this study, we developed the life long health management system in order to provide continuity and compressive medical care. The purpose of developed program is to check the performance of regular health examination and specific screen test state result. The family health management system is provided two main function. The first function is save performance state of regular health examination and specific screen test of registered individuals and their family. The next function is to mail the letter of information about the date of next health examination and the result of health examination and specific screen test. By the selection GUI(graphic user interface) environment, we want to make easily usable user interface with Borland Delpie as program language and with Borland Paradox as development tool of the database.
