1.Large-Cell Acanthoma: A case report.

Large-cell acanthoma is a generally hyperkeratotic, sharply demarcated patch on sun-exposed skin with the outstanding pathologic feature being composed of large, relatively uniform keratinocytes. We describe a case of large-cell acanthoma that involved the skin of the nasal bridge. Patient was a 56-year-old women with a tannish brown patch, 2 cm in size and of 5 years' duration. Controversial issues about nosologic entity of large cell acanthoma are discussed.

The purpose of this study is to investigate the effect of the ball-cone contact interface variation on the ceramic ball stress which is the dominant factor of the design of the moldular artificial hip joint. A contact stress analysis problem is formulated using the optimization principle and finite element analysis technique on the ball-cone system of three dimensional axi-symmetrical model, Numerical procedure is employed to get the solution for varying angles of cones and angular mismatches between the ball and cone interfaces. There is a fair amount of friction in the interface and included in the analysis model.

3.A case of xanthoma disseminatum.

We report a case of xanthoma disseminatum in a 24 year old male paitient. Multiple yellow-brown papules developed on the flexor aurfaces, such as the neck, axillae, antecubital fossae, groin, and perianal regions. Some papules were detected arouns the eyes and uvulai. biopsy specimen revealed a dense infiltrate of histiocytes, foam cells, Touton giant cells, and other inflammatory cells. No Langerhans granules were seen in the electron microscopic analysis.

5.Effect of Community Based Adolescent Drug Abuse Program for Smoking Students.

This study was designed to suggest community-based adolescent drug abuse program model to prevent, treat and rehabilitate the adolescent drug abusers, which works by cooperation between community hospitals and schools, by proving the effectiveness of community-based adolescent drug abuse problem for adolescent smoking students since smoking is one of the most common adolescent abuse problems in the community. To gain the basic data for proving the effect of community drug abuse program for adolescent smoking students, one male high school was selected as the studying school located in Seong Nam city. Among this high school students 146 students were selected randomly to investigate drug abuse status of this school. The experimental group was 33 students who participate in community-based drug abuse problem and the control group was 24 students who were selected randomly among the 60 smoking students who participated in investigating the drug abuse status in the studying school. The both experimental group and control group were reinvestigated of the drug abuse status eight months after completion of community-based drug abuse program, and change during this period was analysed with x2-test and t-test. The results and conclusions were as follows. 1) The smoking onset time was ranged mostly from senior in middle school to freshmen in high school and the most student smoked already habitually when they were freshmen in high school. 80% of smoking students had tried to quit smoking more th% one time and 20% had tried to quit smoking more than six times and failed. This data suggest that the smoking prevention program must be started as soon as possible far effectiveness of adolescent drug abuse program just before many students start smoking and the smoking cessation program for smoking students is needed desperately. 2) The the community-based drug abuse program for smoking students was effective. The smoking students who participate hi drug abuse program(experimental group) showed 19.2% decline of recent one month smoking rates eight months after completion of the program, but the smoking students who did not participate in drug abuse program(control group) showed 22.6% incline of recent one month smoking rates. In the experimental group, the Fagerstrom's nicotine dependance score and recent one month drinking rates showed no difference between initial and 8 month follow-up investigatement, but in the control group, the Fagerstrom's nicotine dependence scores were increased significantly from 1.80 to 3.43 and recent one month drinking rates showed 18.1% incline after 8 month 1311ow-up periods. 3) The smoking students who participate in drug abuse program has less school problems and increased family functioning and decreasing tendency of friends problems but less know ledge about cigarettes and more permissive in attitude to smoking after eight months follow-up period. These results suggest that students who participated in community-based drug abuse program experienced reduction of stress, which results in decline of smoking rates. Therefore, I think that the drug abuse program which deals with the psychological and emotional problems of adolescents is needed as much as direct education about the harmful effect of cigarettes in adolescent drug abuse program for smoking students.

6.Relationship of Leukoaraiosis with Cerebrovascular Disease.

Non-specific periventricular white matter lucencies on computed tomography(leukoaraiosis) were found in 130(40%) of 324 patients with ischemic or hemorrhagic strokes. Patients with leukoaraiosis were significantly older than those without it and correlated with hypertension and lacunar infarction but not with cortical infarction. In patients with hemorrhage, leukoaraiosis occurred sinigificantly more often when aneurysm or arteriovenous malformation were not demonstrated. Our fiindings suggest that leukoaraiosis in cerebrovascular disease is associated with small vessel disease.

7.Development of Family Health Management System in Family Medicine.

With improvement of life quality, the incidence of disease and the concern about preventive care is increasing. With the concern about medicine is changed from therapy medicine to preventive medicine several type of life long health care program is developed. In this study, we developed the life long health management system in order to provide continuity and compressive medical care. The purpose of developed program is to check the performance of regular health examination and specific screen test state result. The family health management system is provided two main function. The first function is save performance state of regular health examination and specific screen test of registered individuals and their family. The next function is to mail the letter of information about the date of next health examination and the result of health examination and specific screen test. By the selection GUI(graphic user interface) environment, we want to make easily usable user interface with Borland Delpie as program language and with Borland Paradox as development tool of the database.

8.Implementation of the Bitmap Image File format for Dental Image Management.

Generally, PACS(Picture Archiving and Communication System) play an important role in the patient care support system of the HIS(Hospital Information System). In the dental PACS, image search and storage is very difficult without standardized DBI(Dental Bitmap Image) file format and view browser because each of the X-Ray instrument and the other image use various image file such as BMP, GIF, TIFF, PCX etc. We proposed the DBI for fast review, classification, storage and transmission of the dental image. It is based on personal computer environment. Using the thumbnail in the DBI, we made image files such as a type of album and operated on PC. We implemented DBI file format that include the DBI viewer and slide show application and other function of fundamental PACS by means of visual C++ and Windows API(Application Programming Interface). This program is consisted of three parts: First, convert the various image file format into DBI file format so as to integrated medical image file format. Second, additional descriptor such as patient name or necessary information to image file and management image file format. Second, additional descriptor such as patient name or necessary information to image file and management image file using patient information or date. Third, execution of the image of the image edit and image enhancement and image copression. Beside this Program Provide GUI interface for user's facility.

9.Compound Nevus Occurring Near External Auditory Canal: Successful Treatment by CO2 Laser Abrasion.

Melanocytic nevus is common pigmented skin tumor, but it has rare occurrence near the external auditory canal. A skin graft is required if a wide surgical excision is performed for the nevi occurring near the external auditory canal. A CO2 laser was considered to be a good alternative for the removal of melanocytic nevus in this area. We herein report a case of compound nevus occurring near the external auditory canal that was successfully treated by CO2 laser abrasion.

10.Effect of Dapsone on Red Cell Indices in Patients with Leprosy.

BACKGROUND: Hemolysis is one of the chief side-effects of dapsone. But the frequency and severity of hemolysis have not been fully elucidated in patients with leprosy. OBJECTIVE: The purpose of our study was to determine the effect of dapsone on hematologic profile in patients with leprosy. METHODS: We did a retrospective analysis of the effect of daily dapsone(100mg/day) in 26 newly-registered cases undergoing multi-drug therapy. Complete blood count(CBC) was performed on their initial visit and at intervals between one and six months. In 10 cases, glucose-6-phosphate dehydrogenase(G-6-PD) levels were determined and in 12 cases iron study was performed. RESULTS: 1. The average hemoglobin was found to fall by 2.9+/-1.4g/dl, from 13.2+/-1.5g/dl, to a nadir of 10.5+/-1.5g/dl(p<0.01). 2. In 13 cases in whom red cell profiles were available, there was a hemoglobin fall from 12.8+/-1.5g/dl to 10.8+/-1.7g/dl(p<0.01) and a hematocrit fall from 37.0+/-4.6% to 32.9+/-4.8%(p<0.05). There was a increase in mean corpuscular volume(MCV) from 90.7+/-2.9fl to 95.2+/-2.8fl(p<0.01). The mean corpuscular hemoglobin(MCH) and mean corpuscular hemoglobin concentration(MCHC) changed from 31.3+/-2.3pg/cell to 31.2+/-1.4pg/cell(p<0.05) and from 34.6+/-2.2g/dl to 32.9+/-0.7g/dl(p<0.05), respectively. 3. Ten patients had G-6-PD measured and nine had normal levels. One with a low G-6-PD had a relatively dramatic hemoglobin fall. 4. Serum iron studies in 12 cases revealed reduced serum iron in 6 cases but normal or increased ferritin in all cases and decreased total iron binding capacity(TIBC) in 6 cases. CONCLUSION: After dapsone treatment almost all patients had a fall in their hemoglobin concentration and an increase in MCV. Impaired iron utilization was a common finding and deficiency of G-6-PD may be a factor aggravating the hemolytic effect of dapsone in leprosy patients.
