1.Ethylene Oxide(EO) Induced Cutaneous Hypersensitivity in a Patient on Hemodialysis.

A 46-year-old male patient had recurrent episodes of generalized pruritic wheals during hemodialysis. He has experienced urticaria during hemodialysis whenever he used a capillary dialyser sterilized by ethylene oxide(EO, Polysulfone-) gas which is used to sterilize hemodialysers and other medical equipment. On the other hand, capillary dialyser sterilized by Gamma ray (Hemophad) has not evoked urticaria. Although the presence of EO-specific antibodies was not detected, urticarial rash never developed when the equipment was switched to a gamma-sterilized one. We herein report a case referred to ethylene oxide induced cutaneous hypersensitivity during hemodialysis.

3.Prognostic Factors in Outcome of Operative Treatment of Achilles Tendon Rupture.

PURPOSE: The purpose of this study is to assist the optimal placement of sacroiliac screw by evaluating the anatomic and radiologic features of upper sacrum. MATERIALS AND METHODS: Fourteen fresh-frozen cadaveric pelvises and 13 normal adults were studied for the evaluation of morphology of first sacrum and dysplastic patterns. Measurement of anatomic parameters and CT imaging of pelvis were performed. Thirty-two patients with complicated pelvic fracture were evaluated for detection of variations in the sacral alar anatomy and slope found in upper sacral segmentation. RESULTS: The mean distance between midpoint of sacrum and sacroiliac joint was 52.75 mm. The height of first sacral body was 21.71 mm. The longest antero-posterior distance in sacral alar was 52.36 mm. The mean angle between the coronal plane of S1 vertebra and anterior aspect of the alar was 29.3 degree. The mean angle between the superior aspect of S1 vertebral body and superior edge of the alar was 34.8 degree. The mean angle between the superior aspect of S1 vertebral body and supero-posterior edge of the alar was 12.5 degree. The studies revealed about 28 to 33% of cases as dysplastic sacrum. CONCLUSIONS: We studied anatomic and radiologic features of upper sacrum in cadaveric and clinical cases to gain information on the optimal placement of sacroiliac screw.

4.A study on the changes of sebaceous gland activity during aging by analysis of skin surface lipids.

BACKGROUND: The human sebaceous glands have long been known to change their activity with aging. Downing and his co-workers state that the ratio of wax ester/cholesterol+cholesterol ester in the skin surface lipids might be a good index for sebaceous gland acti ity. OBJECTIVE: Our purpose was to evaluate the effects of aging on the sebaceous gland activity and relative skin surface lipid composition by using thin layer chromatography. METHOD: Skin surface lipids of anterior chest from 65 healthy Korean indivisuals were collected by using the cup method. Skin surface lipid were separated and meaurd by thin layer chromatographic analysis. RESULTS: The sebaceous gland activity, vrhich was expressed by tlie ratio wax ester/[cholesterol+cholesterol ester] showed a ilistinct change from infancy to senescenc. The curve of the ratio makes a peak in the third decade and decreases with advancing age. CONCLUSION: This result disclosed that sebaceous gland activity iaifected by advancing age in Koreans and can be used as one of the biologic markers of aging.

5.Erythroplasia of Queyrat Involving the Distal Glans Penis Surrounding the Urethral Meatus.

Erythroplasia of Queyrat is an uncommon form of carcinoma in situ involving the glans penis and adjacent mucosal structures. Treatment modalities of Erythroplasia of Queyrat vary widely and topical 5-fluorouracil treatment is very effective and regarded as the choice of t eatment. This article reports a case of erythroplasia of Queyrat involving the perimeatal region of the glans penis and identifies the difficulty of effectively treating erythroplasia of Queyrat which is adjaer to the urethral meatus with 5-fluorouracil and usefulness of electron beam irradiation therapy.

6.A case of malignant fibrous histiocytoma.

The malignant fibrous histiocytoma is a predominantly pleomorphic arcoma usually occuring in the deep soft tissue of adults. Because of its highly variable morphologic pattern, this tumor has often been confused with other sarcomas such as pleomorphic rhabdomyosarcoma and liposarcoma. Although the histogenesis of this neoplasm remains controversial, it is best regarded as a primitive and pleomorphic nature reflected by collagen production and occasional phagocytosis. As report a case of malignant fibrous histiocytoma occurring in a 54-year-old female who presented with 4.5 x 5 x 0.5cm sized mass on the right thigh and metastasis later. Histopathologic findings shows plamp spindle cells appearing in a storiform-pleomorphic pattern around slitlike vessels. Immunohistochemical staining and electron microscopic findings led to the final diagnosis as malignant fibrous histiocytoma. The treatment was surgical excision.

7.A Case of Hereditary Hemorrhagic Telangiectasia.

Hereditary hemorrhagic telangiectasia, also known as Osler-Rendu-Weber disease, is an autosomal dominant hereditary disorder leading to easily bleeding telangiectases on skin and mucosal surfaces, and it is associated with the presence of arteriovenous malformations in multiple organ systems. It is usually recognized as classic triad of telangiectasia, recurrent epistaxis and a family history of the disorder. We report herein a case of hereditary hemorrhagic telangiectasia in a 12-year-old male who had episodes of recurrent epistaxis and compatible telangiectatic skin lesion. And his grandfather, father, uncle and cousin also had similar punctiform, linear and spider-like telangiectatic lesions.

A 7-year-old female was presentd with two black papules and linear pigmentation between papules on the vulva. No other abnormalities were found in her personal and family history. Histopathological examination was consistent with the apocrine hidrocystoma and the pigment in the luminal cells was iron negative and Fontana Masson positive, confirming its melanin nature. We herein report a 7-year-old female patient with pigmented apocrine hidrocystoma occuring on the vulva with the evidence of melanin as the underlying mechanism of the pigmentation.

9.A Case of Pemphigus Foliaceus Presenting as Verrucous Plaque.

Pemphigus foliaceus is a rare, relatively mild and chronic variety of pemphigus which is characterized by superficial flaccid bullae which show acantholytic changes in the upper part of the epidermis histopathologically. Present case is a 40-year-old female with a 2 month history of yellowish colored verrucous plaques on the whole scalp. 6 years ago, she had been admitted to our hospital because of nearly generalized erythematous eroded patches, crusted plaques and bullae and diagnosed as pemphigus foliaceus and treated with corticosteroid and methotrexate. 2 months ago, an additional lesion was found on the scalp, where yellowish colored, verrucous surfaced plaques were observed. Histologic examination on the verrucous plaque showed hyperkeratosis, acanthosis and papillomatosis with acantholysis in the granular layer, leading to a subcorneal blister and dyskeratotic granular keratinocytes were seen within the blister. On the basis of clinicopathologic examination we diagnosed as pemphigus foliaceus and then we treated her with prednisolone and the skin lesion had subsided 7 months later. We report a patient of pemphigus foliaceus who was presented with verrucous surfaced plaque, which clinically mimicked pemphigus vegetans.

10.Appraisal of Anterior Approach in Right Hepatic Lobectomy.

BACKGROUND/AIMS: Despite recent advances in liver surgery, complication and mortality rate in hepatectomy are still high compared to other abdominal surgeries. Intraoperative stress such as bleeding, vascular occlusion, excessive mobilization and prolonged operation time is the most important factor in postoperative complications. Anterior approach avoiding hepatic mobilization and vascular occlusion in right hepatic lobectomy is a useful method for decreasing intraoperative stress. We investigated the effectiveness of anterior approach in right hepatic lobectomy. METHODS: We studied 33 cases of right hepatic lobectomy for malignant tumor between January 1993 and June1997. Thre were 13 cases of Anterior approach (group A) and 20 cases of classic right hepatic lobectomy (group B). We analyzed liver function test, arterial ketone body ratio(AKBR), operation time, blood transfusion during operation, hospital stay, and postoperative complications. RESULTS: Total bilirubin levels at first and seventh postoperative days were 2.1+/-0.6mg/dl, 0.9+/-0.2mg/dl in group A and 2.7+/-1.3mg/dl, 1.0+/-0.3mg/dl in group B. AST were 189+/-65.3 IU/L, 43+/-13.5 IU/L in group A and 325+/-67.8 IU/L, 51+/-18.2 IU/L in group B. ALT were 169+/-30.5 IU/L, 52+/-17.4 IU/L in group A and 295+/-70.3 IU/L, 52+/-16.6 IU/L in group B. AKBR at intraoperative and immediate postoperative period were 0.58+/-0.06, 0.62+/-0.03 in group A and 0.38+/-0.04, 0.40+/-0.08 in group B. Neither operation time (in group A : 380.5+/-61.1 minutes, in group B : 342.9+/-54.8 minutes), transfusion volume during operation (group A : 1222+/-802cc, group B : 1410+/-476cc), nor hospital stay (group A : 22.8+/-3.5 days, group B : 19.1+/-1.4 days) were different between the two groups. Complication rate was lower in group A compared to that of group B (30.8% vs. 40.0%). There was 1 mortality in group B and no mortality in group A. CONCLUSIONS: It is suggested that right hepatic lobectomy through the anterior approach is a useful surgical procedure to reduce intraoperative surgical stress and postoperative complications.
