1.Unilateral bone Transport System using Bifocal Monofixator

Limb lengthening with any kind of lengthening apparatus is accepted as a standard method to correct leg length discrepancy. And furthermore, the deformity accompanying shortening is corrected by multifocal lengthener. twenty two patients have undergone lower limb reconstruction by the technique of unilateral bone transport for diaphyseal bone defect, nonunion or deformity in the presence of shortening from May 1990 to August 1993 in Wilson Leprosy Center & Rehabilitation Hospital. All Patients had bifocal procedure using bifocal monofixator(Orthofix) and bone defects were graduaily filled by bone transport. Average healing index(days/cm)was 61. Average bone defect was 7,4cm, and average transported length was 5.4cm. In conclusion, the unilateral bone transport system using bifocal monofixator has satisfactory outcome, and salvaged limbs where amputation has been previously the only option.

2.The Shelf Procedure for the Dysplastic Hip in Poliomyelitis Patients

The hip joint subluxation and dislocation due to poliomyelitis worsen the limping and caused the painful hip. Shelf procedure has been described as a safe, conservative and reliable approach to the of a dysplastic acetabulum. We performed shelf procedure for the dysplastic hip in poliomyelitis in thirteen patients since May, 1990. The age at operation was 21 to 38 years old and three of them were male and ten were female. All the patients were followed up for average 2.5 years(1-4. 5yrs). The hip joints showed better stability after shel'f procedure and less limping. There was no complication except one redislocation.

4.A Case of pustulosis Palmaris et Plantaris.

A case of 42 years old male with pustulosis palmaris et plantaris was reported. The skin lesions were characterized by several vesicles, pustule, scaly crusts with a erythematous infiltrated base on the both palms and lateral side of the fingers. The physical examination showed dental caries on the left second molar teeth. The pus culture on the Sabouraud's media and the blood agar showed no growing organism. The gram stain of the lakes of pustules reveald many WBC but no organism. He was treated with oxytetracycline, DDS and wet dressing with potassium permanganate solution for 2 weeks and favorable effect was noted.

5.Percutaneous catheter drainage of intraabdominal abscesses and fluid

Percutaneous catheter drainage has been reported to bean effective method in the management of selectedpatients with abscess and fluid collection. Its high sucess rate and relatively low complications make theprocedure an alternative to surgery in the individual cases. During past two years percutaneous catheter dainagein 25 patients with intraabdominal abscesses and fluid collection was performed at the Department of Radiology,Yonsei Universtiy College of medicine. Here the technique and auathor's results were summarized; 1. The total 25patients who had percutaneous catheter drainage are 10 liver abscesses, 3 subphrenic, one subhepatic, 4 renal andperirenal, 2 pelvic, one psoas, one anterior pararenal fluid from acute pancreatitis, one pancreas pseudocyst and2 malignant tumor necrosis. 2. The modified Seldinger technique used for all cases of abscess and fluid draingeunder guidence of ultrasound scan. The used catheters were 10F. Pigtail and 14F. Malecot (Cook c/o) catheters. 3.The abscesses and fluid of 17 patients among 25 were cured by the percutaneous catheter drainage and 4 patientswere clinically improved. The catheter drainage was failed in 2 patients and 3 complications were developed. 4.The success rate of this procefure was 91.3%, failure rate was 8.7% and complication rate was 12%.

7.The Study of Diversity of Carbohydrate Specificities on Rat Genital Tract and Spermatogenesis.

The nature of carbohydrate in the male genital tract of the rat was studied through the carbohydrate-binding specificities of lectins. Lectins used in this study were BS-I, DBA, SBA, WGA, PNA, Cona and RCA. The results were as follows : 1. Secondary spermatocyte showed positive reactions to RCA, spermatid to PNA and RCA and sperm to WGA, PNA and ConA. The sperm in rete testis were positive to DBA, SBA, PNA, ConA and RCA while in epididymis the reactions to SBA, WGA and PNA were positive in all part and the reactions to ConA and RCA were positive only partly. 2. Cell membrane at luminal side of epithelial cell in rat genital tract showed positive reactions to DBA, WGA, and ConA in rete testis; in epididymis, the reactions to WGA and ConA were only partly positive; in vas deferens, the reactions to PNA and ConA were positive in all part and the reactions to DBA and WGA were only partly positive. 3. Cytoplasm of epithelial cell in rat genital tract showed positive reactions to DBA, WGA and ConA in rete testis; in epididymis, the reaction to WGA was positive in all part and the reaction to SBA was only partly positive; in vas deferens, the reaction to ConA was positive in all part and the reactions to BS-I and DBA were positive in some part. 4. Basal side of epithelial cell in rat genital tract showed positive reactions to DBA, WGA and ConA in rete testis; in epididymis, the reaction to WGA was positive in part and in seminal vesicle, the reaction to BS-I was partly positive. 5. Lamina propria of rat genital tract showed positive reactions to BS-I, WGA, ConA and RCA in rete testis; in epididymis, the reaction to WGA was positive in all part and the reaction to SBA was positive in some part; in seminal vesicle, the reaction to BS-I was positive. 6. Muscle layer of rat genital tract showed positive reaction to WGA in epididymis; in seminal vesicle, the reactions to BS-I and DBA were positive. 7. Adventitia of rat genital tract showed positive reaction to WGA in rete testis; in epididymis, the reactions to SBA, PNA, ConA and RCA were positive; in vas deferens, the reactions to BS-I and PNA were positive; in seminal vesicle, the reactions to BS-I and PNA were positive. In conclusion, variable lectin binding properties in the uroepithelia and the cells in the process of spermatogenesis, which were demonstrated in this study, offer the basic data for carbohydrate research in the male genital system.
