2.Study on the (14)C-glucose metabolism by Clonorchis sinensis.

Radioactive (14)C-glucose(U) was given to Clonorchis sinensis in Tris buffer medium, in corder to trace the metabolic fate of the labelled carbon. The labelled carbon from glucose enters into intermediary metabolites and end products of anaeroblic glycolysis, Embden-Meyerhof pathway, and of aerobic Krebs cycle. These product were identified by one or two-dimensional paper chromatography in combination with autoradoigraphy. The labelled metabolites detected in this experiment corresponded to pyruvic acid, latic acid, malic acid, succinic acid and fumaric acid. Amino acids, such as alanine, aspartic acid, glutamic acid, valine, theronine, and serine, derived by the degradation of (14)C- glycose were also found. Labelled compounds behaving like alanine, aspartic acid and glutamic acid were observed in the chroma to gram of incubation medium. The preciptation which suggests a positive reaction for protein occured when absolute ethanol was added to the incubation medium.

3.The epidemiological studies on the filariasis in Korea II. Distribution and prevalence of malayian filariasis in southern Korea.

During 1964 to 1967, a survey of 30,534 persons for filariasis was made on all over the country with the following results: Among 24,816 draftees from all over the country in the army recruitment camp 155(0.63 %) were found infected with Brugia malayi. Cheju Do showed the highest microfilaria rate(3.5 %), North Kyongsang Do(1.4 %) the next and South Cholla Do(1.2 %) the third. Blood films from 2,308 inhabitants were examined and 407(17.6 %) showed microfilaria in Cheju Do. 30(3.1 %) out of 974 inhabitants in North Kyongsang Do were found to be infected. The microfilaria rates were 2.0 per cent for 400 inhabitants of Chindo island in South Cholla Do. However, no positive case of microfilaria was found in the inhabitants of Kokumdo among 1,820 persons examined in South Cholla Do and of Namhae island(among 165 persons examined) in South Kyongsang Do. The mean microfilarial density per 20 cu. mm of blood was 52.6 in the inhabitants of Cheju Do, 12.2 in North Kyongsang Do and 27.3 in Chindo island(South Cholla Do). Any distinct relationship between the incidence of positive cases of microfilariae and age or sex of the cases was not observed in Cheju Do. However, in Norh Kyongsang Do and South Cholla Do the microfilaria rate of the male group is higher than the one of the female group of the inhabitants. From the above survey results it turned out that filariasis was found throughout Southern Korea except Kyonggi Do and South Kyongsang Do. A total 30,534 persons examined, 601(2.0 %) were found to be infected by Brugia malayi. Therefore, it seems that there are some endemic foci of malayian filariasis in three main areas such as North Kyongsang Do, South Cholla Do and Cheju Do.

4.A Study for Incidence and Treatment of Vitamin B12 Deficiency after Total Gastrectomy.

PURPOSE: Vitamin B12 (VB12) deficiency is an inevitable sequela of a total gastrectom, which results in general symptoms, including easy fatigue, and hematological, neurological, and gastrointestinal complications. Especially in cases of neurological injury, it may be irreVersible if the timely treatment is delayed. Therefore the early diagnosis and treatment is essential. However, no guidelines exist for the incidence or treatments. METHODS: We investigated the symptoms and serum VB12 concentrations of 296 patients who underwent a total gastrectomy for a gastric malignancy. We defined 200~300 pg/ml as the mild decrease group, under 200 pg/ml as the severe decrease group, and over 300 pg/ml as the normal limit. RESULTS: The cumulative incidence of VB12 deficiency were 5.1, 11.2, 29.9, 44.7, 64.5% at 6 month, 1, 2 and 3 years, and at 4 or more years, respectively. The 90% of patients reported at least 1 symptom. The group under 200 pg/ml was supplemented at 1 month intervals; 10 of the 16 patients (63%) had their VB12 elevated to above 300 pg/ml. The group between 200~300 pg/ml was supplemented at 1 or 3 month intervals; 21 out of 23 (91%), and 12 out of 15 patients (80%) had their B12 elevated to above 300 pg/ml at the 1 and 3 month intervals, respectively, but with no statistical significance. CONCLUSION: The group with a V12 under 200 pg/ml should be supplemented 6 times, at 1 month intervals, regardless of the symptom presentation, and when the rechecked serum VB12 level has been increased above 300 pg/ml, it should be supplemented at 3 month intervals. In the group with a VB12 between 200 and 300 pg/ml, the VB12 should be supplemented at 3 month intervals if the symptom is present, and the asymptomatic group should be observed.

5.Adult Polycystic Kidney Disease with Cardiovascular Assault.

Adult polycystic kidney disease is a genetic disease characterized by bilateral multiple renal cysts and is transmitted as an autosomal dominant traits. The disease usually manifests in the fourth decade of life. The affected patients usually die with end stage renal failure, cardiovascular assaults and infection etc. Cardiovascular assaults include ruptured berry aneurysm, spontaneous intracranial hemorrhage, acute myocardial infarction, dissecting aortic aneurysm and hypertensive heart failure. Fatal intracranial hemorrhage and hypertensive heart failure occur in 15% and 2% of patients, respectively. Genetically-determined structural weakness in the arterial wall have been suggested as a possible factor in the genesis of berry aneurysm. We have experienced three autopsy cases with cardiovascular causes of sudden death. Two cases have intracranial hemorrhage (1 berry aneurysm and 1 intracerebral hemorrhage) and the remaining one is dead with hypertensive cardiomyopathy. All of them are aged over 40 years and associated with polycystic liver disease. Histologically, myriad cysts of enlarged bilateral kidney are lined by single layer of cuboidal cells with focal hyperplastic epithelial configuration. Diffusely scattered liver cysts are also lined by flat to cuboidal epithelium and often associated with portal fibrosis.

6.Isolated avuision fracture of the tibial attachment of the posterior cruciate ligament

Five patients with isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament were treated in our hospital from September, 1975 to May, 1978. Among the five patients. one was treated conservatively and the others by surgical repair using catgut suture, cancellous bone screw fixation and staple fixation. Four patients were followed from 4 to 10 months poatoperatively. Satisfactory results were obtained in four patients (one was loat in follow up).

7.The Effect of Polyurethane Film with Sustained Release Edxamethasone on Prevention of Experimental Fat Adherence Syndrome in Rabbits.

The fat adherence syndrome(FAS) is a severe form of the restrictive strabismus resulting from the adherence of orbital tissues to the globe of extraocular muscles and their attachments, for which no successful treatment currently exists. we developed anexperimental model for the FAS in the rabbit eyes and then evaluated the effects of polyurethane film with sustained release dexamethasone on the prevention of experimental FAS. Thirty eyes of fifteen white rabbits had a fat autograft placed between the inferior rectus and the periosteum of the inferior orbital rim and then, the inferior rectus, the fat and the periosteum were sutured to create an experimental FAS. In Group I the experimental FAS was only induced as a control group. The polyurethane film with no released substance and the polyurethane film with sustained release of dexamethasone were inserted between the inferior rectus and the fat in Group II and Group III, respectively. The force required to move the eyeball superiorly 4mm, 6mm, and 8mm was measured preoperatively and 4 weeks ostoperatively. At postoperative 4 weeks the surgical dissection to evaluate the degree of the adhesion and the histologic examination were performed. A significant increase in forced duction(postop-preop) was found at all levels of ocular rotation in Group I and it means that the induction of experimental FAS was successful. Histology revealed a moderate inflammatory response with partial loss of normal adipose tissue replaced by fibrovascular tissue. Group III had significantly less restriction than Group I and Group II. Group II had less restriction than Group I, but statistically not significant. In conclusion, the use of the polyurethane film with sustained release of dexamethasone may have a role in the prevention of experimental FAS.

8.A Family of Hereditary Olivopontocerebellar Atrophy (Menzel type OPCA, OPCA III with retinal degeneration).

This is a case report of the familial olivo-ponto-cerebellar atrophy (Menzel type OPCA, OPCA III with retinal degeneration). The patient is a 37 year-old male with 5 years history of slowly progressive cerebellar ataxia, dysarthria, visual change, horizontal nystagmus and signs of pyramidal dysfunction. The CT brain scan shows significant atrophy of brain stem and cerebellum with dilatation of cisterns. His younger sister, also, has similar clinical manifestations and radiological abnormalities, but mild. By history, his mother who died at 47 years of her age looked like to have same kind of chronic disease.

9.Acantholytic Acanthoma in a 40-Year-Old Female: A Case Report

Acantholytic acanthoma is a rare benign neoplasm of epidermal keratinocytes that clinically presents as a solitary, asymptomatic keratotic papule or nodule on the trunk. A 40-year-old female visited our hospital with a 20-year history of a single papule on her right thigh. On physical examination, the lesion measured 0.5×0.4 cm in size and appeared brown in color with central hyperkeratosis. Histopathological evaluation showed hyperkeratosis, acanthosis, papillomatosis, dyskeratosis, and acantholysis. Only six cases of acantholytic acanthoma have been reported in Korea. We report a rare case of acantholytic acanthoma that occurred in a 40-year-old female who presented with a keratotic papule on her right thigh.

Prurigo nodularis (PN) is a chronic cutaneous condition characterized by a papulonodular pruriginous eruption accompanied by pruritus. PN lesions are typically firm and itchy hyperkeratotic nodules and papules, which provoke a scratch response. PN may often represent an initial cutaneous manifestation of human immunodeficiency virus (HIV) infection. The association between PN and HIV is attributable to various etiopathogenetic mechanisms including cutaneous immunological abnormalities. Diagnosis of HIV infection in a patient with PN is challenging.We report a case of confirmed HIV infection in a patient with treatment-resistant PN, who was successfully treated using combination treatment that included HIV antiretroviral therapy and narrow-band ultraviolet-B radiation.
