1.Analysis of Operative Treatment of 154 Herniated Intervertebral Disc in Lumbar Spine

One hundred and fifty-four operative cases of herniated intervertebral disc were evaluated in department of Orthopedic Surgery, National Police Hospital from June 1973 to May 1979. The results were obtained as follows: 1. Out of the 154 patients, there were 137 male and 17 female cases. The ratio between male and female was 8:1. 2. The majority (72%) of the herniated intervertebral disc was found in the age group of 20 to 40 years. 3. The most common site of the lesion was between L4 and L5 vertebrae (70%). 4. About 90% of the patient showed significant sign in myelographic and clinical studies. 5. To prevent adhesion of sciatic nerve & dura after the operation, spraying the methyl-prednisolone over the operative site and covering dura with fat in spinal canal could be resulted in good method. 6. The dissecting type is considered to be what operators should observe attentively during the operation. 7. The result of treatment was excellent in 94 cases (61%) and good in 47 cases (30%).

2.Clinical Study of Meniscus Tear

It is assumed that the frequency of the meniscus tear on the right and the left knee and the mechnism of injuries in westerners are different from those in easterners. The writer had experienced 32 cases of the meniscus tear in National Police Hospital, during the period from 1973 to 1977, and inquired into its frequency and cause based on appropriate references. This is the report on the result of our study. 1. Out of 32 cases, 21 cases were found in the group of 20 to 29 years of age. 2. Eleven years of age with a discoid type was noticed as the youngest. 3. Out of 32 cases, 20 cases were found on the lateral meniscus and 12 on the medial, showing every five lateral cases to every three medial cases. 4. Out of 32 cases, 17 cases were found on the right knee joint with 14 lateral and 15 on the left knee joint with 9 medial. 5. Out of 32 cases, 27 cases were found on the male and 5 on the female, showing 5. 4 male cases to every one female case. 6. Out of 32 cases, 29 cases had the apparent history of trauma. 7. Transverse tear was found more frequent on the lateral meniscus and longitudinal or bucket handle tear was found more frequent on the medial meniscus.

4.A Study on the Reliability and Validity of Seoul-Activities of Daily Living(S-ADL).

OBJECTIVES: Seoul-Activities of Daily Living(S-ADL) was developed to assess elderly person's basic activities of daily living. This study aims to develop standardized ADL assessment scale and confirm the reliability and validity of the S-ADL. METHODS: It was participated in 336 controls and 145 patients diagnosed Alzheimer's disease. RESULTS: In controls, it was statistically significant to age, but not sex, education, region and presence of spouse. Also, reliability and validity were statistically significant. Principal axis factoring analysis revealed three factors that accounted for 66.67% of the total variance(1.self-care/hygiene, 2.ambulation, 3.toileting). According to each CDR stage, there were significant difference, except for CDR 0.5 and CDR 1 suggesting early dementia. Particularly, it was remarkable for functional impairment in CDR 2 and CDR 3 suggesting moderate to severe dementia. The order of the loss of function was (1) self-care/hygiene, (2) toileting, and (3) ambulation. CONCLUSIONS: Our study showed that the S-ADL could be a very reliable and valid tool for the assessment of functional disabilities of Korean dementia patients. Particularly, S-ADL would be useful in assessing daily function of moderate to severe AD.

6.Analysis of Clinocopathologic Difference between Type II and Type III Cancers in Siewert Classification for Adenocarcinomas of the Cardia.

PURPOSE: To determine the clinical value of the Siewert classification for gastic-cancer patients in Korea, we evaluated and compared the clinicopathologic factors of type II and type III cancer. MATERIALS AND METHODS: The medical records of 89 consecutive patients who had undergone surgery for an adenocarcinoma of the gastroesophageal junction (GEJ) at the Department of Surgery, Hanyang University Hospital, between Jun. 1992 and Dec. 2003 were reviewed retrospectively. RESULTS: There were one patient with type I, 12 pateints with type II and 77 patients with type III. During the same period, 1,341 patients underwent surgery for a gastric carcinoma, so proportion of GEJ cancer being 6.6%. The median follow- up duration was 31 months (range: 2~135 months), and the follow-up rate was 100%. Between type II and type III cancers, there were no significant differences in the clinicopathologic variables including age, sex, gross appearance, histologic type, depth of invasion, and pathologic stage. The longest diameter of the tumor was larger in type III (6.1+/-2.1 cm) than in type II (3.9+/-1.1 cm)(P=0.001). A total gastrectomy with Roux-en-Y esophagojejunostomy was done most frequently, while jejunal interposition was done in 3 cases of type II and 2 cases of type III. More than a D2 lymphadenectomy was done all cases. The numbers of dissected lymph nodes and metastatic lymph nodes in type II were 43.8 and 5.8 respectively, while they were 49.8 and 8.1 in type III, but the difference between the two groups were not statistically significant. The mean length of the proximal resection margin was 15+/-5 mm in type II and 21+/-13 mm in type III, but this difference was not statistically significanct. The time to recurrence after operation was 19.3 months in type II and 16.9 months in type III. The five-year survival rates of type II and III were 68.8% and 52.7% respectively, but difference was not significant. CONCLUSION: There were no significant differences in the clinicopathologic variables, including survival rate, between type II and type III cancers in Korean patients. According to these findings, it appears to be reasonable to classify type III cancer as a cardia cancer in a broad sense.

7.Optic Disc Hamartoma Combined with Optic Neuritis.

PURPOSE: Optic disc harmatoma is usually seen in tuberous sclerosis patients, but, it may be seen in otherwise normal people. Visual acuity is usually not affected by this lesion. METHODS: We experienced a 40-year-old woman with optic disc hamartoma who presented with acute visual defect. With oral triamcinolone 48mg/day, her visions recovered to normal in 2 weeks. RESULTS: Her age, symptom, and course of disease supported the diagnosis of optic neuritis. CONCLUSION: We report this patient as a case of optic disc hamartoma combined with optic neuritis.

10.Effect of Removal of Internal Limiting Membrane in Macular Hole Surgery.

Tangential vitreoretinal traction around fovea is known to be the principal cause in the pathogenesis of idiopathic macular holes. According to the recent study, the internal limiting membrane(ILM) may provide a scaffold for fibroblast proliferation and contraction, therefore play an important role in the enlargement of macular hole. We removed the ILM during macular hole surgery and assessed the anatomical and functional outcome of the macular hole surgery. Twelve consecutive patients with full-thickness macular hole were enrolled in this study. They were examed for best-corrected vision, fundus photography, fluorescein angiography, Humphrey automated perimetry, focal eletroretinogram, before and after surgery. After surgery, vision was improved in 8 eyes(67%), hole was closed in 10 eyes(83%). Macular thresholds on visual field was increased in 8 eyes and peripheral scotomas developed in 3 eyes postoperatively. The amplitude increased in 7 eyes and the implicit time decreased in 8 eyes on focal electroretinograms postoperatively. Out results suggested that the complete removal of ILM should be attempted to improve vision and to close holes during the full-thickness macular hole surgery.
