2.A Clinical study on Non-union of the Femoral Shaft Fractures in Adults

Femoral shaft fractures are usually accompanied with extensive soft tissue injury due to high energy force and are difficult to manage for functional stress with weight bearing. According to Boyd(1965), femoral shaft is the first of non-union among the long bone shafts. And because of inadequate internal fixation, poor immobilization and infection, non-union of femoral shaft is still of interest to all the orthopaedic surgeons. During past 10 years, 19 cases of non-union of the femoral shaft were treated at dept. of Orthopaedic Surgery, Ewha Womans University hospital and following results were obtained. l. Of 19 cases, male between 20-40th age group was predominent. Prevalent type and site was mid. 1/3 comminuted fractures(p > 0.05). 2. Initial treatment was 10 cases of intramedullary nailing; 8 of plate fixation; 1 of screw fixation. Bone graft was added to 10 cases. 3. The probable causes of non-union were complex; 11 of inadequate interal fixation, 4 of poor external immobilization and 4 of infection. 4. 4 cases of infection were managed with control of infection and fixation. And 11 cases were fixed with plate and the other 4, with intramedullary nailing. All were supplemented with autogenous bone graft. 5. Postoperative immobilization was done with 15 cases of hip spica cast and 4 of long leg cast for average 9.3 weeks. 6. All were united at average 8.9 months. Limitation of motion of knee was remained in 3 cases in spite of physiotherapy. Severe osteoporosis was noted in 3 cases and leg length shortening with limping was found in 1 case.

3.Gianturco Metallic Biliary Stent in Malignant Biliary Obstruction: Results of Follow-up in Dead Patients.

PURPOSE: In order to study the patency, restenosis, efficacy, and complications of the metallic stent in the course of treatment of malignant biliary obstruction,the results of follow up of the dead patients after stent insertion were reviewed. MATERIALS AND METHODS: Self-expandable Gianturco metallic stent with 10-mm diameter was successfully inserted in 33 patients :10 with Klatskin tumor, 7 with common bile duct cancer, 7 with gallbladder cancer, 5 with pancreatic cancer, 2 with recurred stomach cancer, one with periampullary cancer, one with hepatocellular carcinoma. RESULTS: The overall duration of survival and patency of the stents in 33 patients were 5.2 months(1-12 months) and 4.9 months(1.14 months), respectively. Restenosis of metallic stents was found in 9 cases(27%), after 6.1 months in average. Causes of stent occlusion were overgrowing of tumor in 5, overgrowing and ingrowing of tumor in 3, extraductal dislodgement in one case. Two cases of symptomatic cholangitis after stent placement were successfully treated with percutaneous cholecystostomy. Three cases of destruction and migration of metallic stents were found after 6 months. CONCLUSION: On the basis of our experience, insertion of Gianturco metallic biliary stent is an acceptable treatment method in the malignant biliary obstruction, especially for whom short term survival is expected.

4.Primary Malignant Melanoma of the Cervical Spinal Nerve Root.

The authors report on a case of primary malignant melanoma of the 7th cervical spinal nerve root in a 45-year-old woman. Neuro-radiological features of this extra-dural mass were suggestive of a nerve sheath tumor. The lesion underwent total gross resection through the anterolateral approach. The patient's postoperative course was uneventful. Histopathological investigation confirmed malignant melanoma. There was no evidence of tumor recurrence or other melanotic lesions on regular follow-up examinations until the postoperative eighth month. When treating a common, benign-looking lesion of the cervical spinal nerve root, surgeons should be aware of the potential to encounter such a malignant tumor.

6.Compensation for Occupational Diseases by Chemical Agents in Korea.

Investigation into the frequency of compensation for occupational diseases (ODs) caused by hazardous chemicals revealed an important opportunity for the improvement and further development of occupational health and safety systems in Korea. In response to concerns after outbreaks of disease due to chemical exposure, specific criteria for recognition of ODs were established and included in the Enforcement Decree of the Labor Standard Act (LSA) and the Industrial Accident Compensation Insurance Act (IACIA) on June 28, 2013. However, the original versions of the LSA and IACIA contain several limitations. First, the criteria was listed inconsistently according to the symptoms or signs of acute poisoning. Second, all newly recognized hazardous chemicals and chemicals recognized as hazardous by the International Labor Organization (ILO) were not included in the LSA and IACIA. Although recent amendments have addressed these shortcomings, future amendments should strive to include all chemicals listed by the ILO and continuously add newly discovered hazardous chemicals as they are introduced into the workplace.

7.Health status and related factors in farmers by SF-12.

OBJECTIVES: This study was performed to understand farmers' health status by general characteristic, and to find out the related factors. METHODS: All the 984 subjects were interviewed by means of a structured questionnaire and SF-12. Among them, only 812 were eligible for analysis. Statistical methods used included frequency, t-test, ANOVA, binary logistic regression with SPSS 19.0. RESULTS: In binary logistic regression, marital status, smoking, regular exercise and monthly day off were associated with physical component score. Marital status, smoking and score of pesticide protective device wearing were associated with mental component score. CONCLUSIONS: This study suggests that effort to develop health promotion programs for workers of agricultural industry considering these results can improve their perceived health status.

8.A Case of Asystolic Cardiac Arrest in the Sitting Position for Shoulder Arthroscopy using an Interscalene Block: A case report.

An interscalene block for shoulder surgery provides excellent surgical conditions and is well received by patients. However, a combination of the sitting position, regional anesthesia, an awake patient, and a surgical procedure can result in another side effect, vasovagal episodes, which may lead to cardiac arrest in extreme cases. We experienced a case of 32-year-old male patient with a severe vasovagal episode that led to asystolic cardiac arrest 15 minutes after the changing to the sitting position under an interscalene block for shoulder arthroscopic surgery.

9.The Sensitization Rates for Inhaled Allergens by Skin Prick Test among Some Farmers in Gyeonggi Province, South Korea.

OBJECTIVES: Farmers are known to be exposed to a variety of allergens related to the work environment. This study was conducted to determine the sensitization rates as well as South Korea that they are sensitized to certain allergens farmers through the skin prick test. METHODS: By targeting a total of 1143 people living in the rural town of Gyeonggi Province, it was conducted a questionnaire containing demographic and occupational risk factors and underwent skin prick tests with 15 types of allergens(including positive and negative controls). Multivariable logistic regression analysis was used to analyze the association between occupational risk factors and skin prick test positivity. RESULTS: Except for the 30 people whose result is invalid, positive rate of the skin prick test was 18.6% in 1,113 people. The species of house dust mite, Dermatophagoides pteronyssinus and Dermatophagoides farinae was the highest at 8.7% and 8.6%. After adjusted by age, gender, smoking and education level, odds ratio of flower plant farmers is 4.467(95% CI: 2.094-9.527) and fruit farmer is 2.275(95% CI: 1.096-4.721). In addition, the rate of sensitization to grass pollen mixture of the flower plant farmers is significantly higher(15.9%, p<0.001) than other allergens. CONCLUSIONS: Even farmers, the rate of sensitization to allergens related to the general environment, such as house dust mite is relatively dominant. However, given the presence of potential cross-reactivity between the allergens or distribution showed that the unique aspects of allergen sensitization in the flower growers, occupational cause is not be completely ruled out.

10.Use of Artificial Intelligence in Diagnosis and Treatment of Psychiatric Disorders

Artificial intelligence (AI) is widely used as an auxiliary device to diagnosis and treat mental illness, and the scope of its use is gradually expanding. It is widely used as a tool to assist diagnosis in various fields such as radiology and ophthalmology. Meanwhile, in psychiatry, the use of AI has been limited so far due to the lack of a specific objective test for diagnosis and the reliability of medical records for sensitive records such as suicide attempts. However, AI can detect people’s behavior and facial expressions changes more sensitively and more accurately find the correlation between quantitative neurophysiological test results and behavior and emotions. Nowadays, AI is useful as an auxiliary tool in diagnosing not only pediatric and adolescent mental disorders such as attention deficit hyperactivity disorder (ADHD) and autism spectrum disorder (ASD), but also affective disorders and psychotic disorders. AI is also actively used to treat insomnia, ASD, ADHD, cognitive dysfunction, depression, anxiety disorders, substance use disorders, and schizophrenia. Many psychiatrists are still cautious about the use of AI. There are concerns that AI cannot replace the doctorpatient relationship, which is the most important element of traditional psychotherapy. Nevertheless, AI can diagnose psychiatric disorders more accurately and can also be useful in improving patients’ symptoms and quality of life through AI or digital treatments. I hope that psychiatrists to contribute to the treatment of human mental illness by becoming users, supervisors, and monitors of AI.
