2.Efficacy of Cefadroxil Monohydrate (Duricef.

Effect of Cefadroxil monohydrate was evaluated on 37 patients with urinary tract infection in our department during past 6 months periods. Cefadroxil was given Orally at a dose of 1.0 gm - 3.0 gm bid per day for 3 - 15 days and following results were obtained. 1. Excellent or good results was observed in 31 out of 34 patients in whom complete follow up study were done, giving success rate of 91.2%. 2. Therapeutic effects were obtained in 10 out of 11 patients with urethritis and all patients with cystitis and pyelonephritis, but not in 1 with urethritis, 1 with prostatitis and 1 out of 9 urinary tract infections with predisposing factors. 3. In this clinical settings, Cefadroxil was effective against E. coli infection in 10 patients, mixed infection in 2 and Staphylococcus aureus infection in 5. However it was ineffective against Serratia infection in 1 patient and Staphylococcus infection in 1. 4. As to the side effects, only mild dizziness was observed in 2 patient.

3.The Incidence of parasites found of vegetables.

The high incidence of intestinal parasites among the Korean people almost same as 40 years ago might be partly interpreted by the fact that they are all fond of raw vegetables cultivated on the farms fertilized with non-treated night soil. The cabbages, onions and radishes were collected from the five areas of Seoul vicinity and the number of parasites attached to those vegetables were investigated to compare with the results obtained 10 years ago by the same methods of sampling and examination as the present survey. The resultant findings were summarized as follows: The eggs of Ascaris, hookworm, Trichostrongylus and whipworm, rhabditoid larvae and filariform larvae of hookworms were detected from those vegetables. Additionally, a number of larvae of free living nematodes and morphologically undetermined eggs were also observed. The maximum number of parasites found from one vegetable were as follows: 16 Ascaris eggs on a cabbage, 27 hookworm eggs on an onion, 8 whipworm eggs on a cabbage, 68 filariform larvae of hookworm on an onion. The onions were highly contaminated by the number of parasites as much as 177.6 including the larvae of free living nematodes, the next being the cabbages with 25.3 and the radishes with 4.1 in average. No notable relationship was observed between the size of vegetables and the number of parasites attached to them. Ascaris eggs were found from 56 percent of all cabbages, 28 percent of the onions and 40 percent of the radishes; whipworm eggs were detected on 40 percent of radishes and the filariform larvae of hookworms were observed from 40 percent of onions examined. It may be concluded that there is no notable change in the number of parasites attached to the vegetables collected from the Seoul vicinity compared to the results investigated by the same method 10 years ago.

5.The Effect of Photodynamic Therapy in BALB/-c Mice Adenocarcinoma in Homograft Model.

Among the efficient cancer treatments, photodynamic therapy (PDT) is one of the therapies inducing rapid apoptosis of cancer cells while causing minimal damage to surrounding normal tissue. We studied the effect of PDT on the adenocarcinoma in BALB/-c mice of homograft model, and the following results were obtained. Apoptosis occurred up to 3 mm in depth from the surface in the first 1 hour after PDT applied, and subsequently the counts were increased in the deeper portion. A remarkable apoptosis observed up to 6 mm in depth shows that the light in use could not penetrate more than 6 mm of tissue. Tissue necrosis was identified in the deeper area of the tumor 6 hours later or thereafter. This necrosis seemed to occur as an indirect effect of vascular obstruction resulting from the damage of endothelial cells which was induced by selective collection of photosensitizer in the endothelial cells of newly forming vessels as well as in the cancer cells. These results indicate that the effective depth of PDT is greater than the depth of light penetration.

6.Expression of Surfactant-D Protein and TNF-alpha in the Interaction of Pneumocystis Carinii and Alveolar Macrophages in Pneumocystis Carinii Pneumonia.

Alveolar macrophages participate in the host defense against P. carinii, but the mechanisms in degradation and clearance of the organism from lung has not been well established. We observed the transmission and scanning electron microscopic features and evaluated the expression of TNF-alpha and Surfactant-D in the interaction of P. carinii with alveolar macrophages. Expression of TNF-alpha and Surfactant-D in the experimentally induced P. carinii pneumonia in rat was examined by immunohistochemistry and immunoelectron microscopy. Electron microscopically, the alveolar macrophages phagocytized trophozoites and cysts of P. carinii micro-organisms. Immunohistochemically TNF-alpha was strongly expressed in the cytoplasms of alveolar macrophages. Postembedding immunogold labeling for Surfactant-D protein was expressed on the pellicles of trophozoites and cysts, P. carinii micro-organisms in the cytoplasms of macrophages, free floating surfactant materials and multilamellar bodies of type II epithelial cells. We conclude that alveolar macrophages interacted with P. carinii micro-organisms respond with increased expression of TNF-alpha. TNF-alpha may bind to P. carinii and exert a direct toxic effect upon the micro-organisms. Surfactant-D protein may augment binding of P. carinii to the alveolar macrophages and enhance the clearance of the micro-organisms.

7.Ventilatory Dynamics in Hypertensive Heart Disease.

Small and large airways functions were studied in patients with hypertensive heart disease in slightly ro moderately compromised state functionally. In this study, the forced vital capacity and various flow paramaeters reflecting expiratory flow rate were determined from simultaneously recorded forced expiratory volume and maximal expiretory flow volume curves in 86 cases. The closing volume was measured by a single breath nitrogen mrthod in 57 cases and airway resistance with its related parameters by a body plethysmograph in 11 cases. These results were compared with those obtained from the same numbers of healthy controls matched for sex, age and height. In the patient group, the forced vital capadity and all the observed values of flow parameters, execpt for the ratio of the first second vital capacity to the forced vital capacity, were significantly reduced than those in the controls. When the remainder of flow parameters was volume-adjusted to the forced vital capacity, however, the mean of the peak expiratory flow rate and the maximal expiratory flow rate at the 75 percent of the vital capacity were not significantly different from that of controls. In contrast, the volume-adjusted values of maximal expiratory flow were remained significantly smaller than those in the controls. The closing volume and its ratio to the vital capacity were significantly larger in the patient group. Airway resistance and its related parameters revealed no significant differences between two groups. These findings suggest that the patients with hypertensive heart disease in a mild to moderate failure are associated with restrictive ventilatory impairment and a small airways obstruction, but with little or no large airway dysfunction.

8.ANALYSIS OF THE FIT IN THE IMPLANT PROSTHESIS USING LASER DISPLACEMENT METER AND THREE-DIMENSIONAL FINITE ELEMENT METHOD.

A precise fit of the implant prosthesis is one of the most important factors in preventing mechanical complications. To analyze the degree of the misfit of implant prosthesis, a modal testing experiment was accomplished. And to interpret the modal testing analysis mathematically, three-dimensional finite element models were established. In the experimental modal testing analysis, with a laser displacement meter, FFT analyzer, impact hammer, etc., natural frequencies of the models with various degree of prosthesis fit were determined after the frequency response function were calculated. In the finite element analysis, the natural frequencies and mode shapes of the models which simulated those of experimental modal testing were computed. The results were as follows : 1. Natural frequencies of the prosthesis-abutment were related to the contact state between components. 2. In the modal testing experiment, the natural frequencies increased from 50micrometer to 200micrometer gap and reached a plateau. 3. In the finite element analysis, the natural frequencies decreased gradually according to the increase of the gap size. 4. In the finite element analysis, the mode shapes of model 1 with misfitting prosthesis showed different patterns from those without misfitting prosthesis. 5. The devices including a laser displacement meter used in this study were useful for measuring the natural frequencies of an implant prosthesis which had various degrees of fit.

9.An Ultrastructural Study on the Small Intestinal Absorptive Cells of Rat after Administration of Lead Acetate.

This study was carried out to visualize lead by Timm sulphide silver method and to define lead-induced change of duodenal absorptive cells of rat after administration of 0.01% lead acetate with drinking water. Sprague-Dawley rats, weighing 250g, were used, and they were orally administered with 0.01% sodium acetate and sacrificed at 0.5, 1, 1.5 hours and 2, 7, 15, 30, 45, 60 days after administration. A portion of duodenal tissue was observed under light microscope, scanning and transmission electron microscopes after development with Danscher method. The blood lead concentration in experimental group began to increase from the 2nd day after administration, and it increased gradually until the 45th day and decreased at the 60th day. On light microscope, many brown lead granules were observed at the villi tip at the 2nd day. There is mild blunting of villi tip at the 45th day. At the 60th day, most of the villi were mildly shortened and showed lymphangiectasia. On scanning electron microscope, the villi tip was mildly blunted and the extrusion zone became irregular at the 45th day. The depth of creases did not change. At the 60th day, the villi tip was moderately blunted and the extrusion zone was markedly irregular. The depth of creases increased. On transmission electron microscope, at 0.5 hour after oral administration, numerous lead granules were diffusely scattered and were not confined to any specific microorganelles. The lead granules decreased with time. At the 7th 15th day, the intercellular spaces were widened and several vacuoles appeared and the condensation of mitochondrial matrix. There was also ribosomal detachment from RER, and there was neither secondary lysosomes or post-lysosomes. At the 30th and 45th day, secondary lysosomes appeared and the condensation of mitochondrial matrix with early formation of myelin figures was noted. At the 60th day, the intercellular space widening extended to the upper most portion of the cells, and nonspeciqic degenerative changes became severe. In view of above mentioned findings, it can be concluded that passive diffusion as well as active transport was partly involved in the absorption of lead. Most of the changes of microorganelle are compatible with nonspecific degenerative changes which could occur due to impairment of oxidative phosphorylation.

10.Adenocarcinoma of Urinary Bladder: 2 cases report.

Two cases of adenocarcinoma of the urinary bladder with clinical and pathological features, and brief review of the literatureare presented. Case 1: The patient, a 52 year-old man, was admitted to this hospital because of intermittent painless total gross hematuria for 15 years. Cystoscopy was done, and showing a cauliflower mass with broad based diffuse infiltrating lesion at the right anten or portion of bladder. TUR-B was performed. Microscopically, the lesion consisted of colonic metaplastic epithelium with atypical glands and cystic dilatation and adenocarcinoma. Case 2: The patient, a 52-year-old woman, was admitted to this hospital because of total painless gorss hematuria for 1 year. Cystoscopy was done showing a sessile diffuse mass with ulceration on the dome area. Total cystectomy was performed. Grossly, the tumor showed an ulcerative tumor mass with elevated nodular margin at the dome of the bladder. Microscopically, the lesion consisted of anaplastic glands with back to back arrangement and branching glands through the entire thickness of the bladder wall.
