1.A Case of Primary Leiomyosarcoma of the Vagina.

The vagina is an infrequent site of primary sarcomas, which are less than 2% of all malignant vaginal neoplasm. The most common malignant mesenchymal tumor of the vagina is rhabdomyosarcoma, but leiomyosarcoma is rare type tumor of the vagina. Microscopically, spinle cells with pleomorphic nuclei and mitotic figures were frequently noted, but cross-striations were not found in the tissue. Alpha-smooth muscle actin and desmin were positive, but sarcomeric actin was negative in the tumor cells. The authors report a case of primary leiomyosarcoma of the vagina with brief literature review.

3.An Analysis on the Patient Transported via 119 System.

An analysis was performed to evaluate the problems in Emegency Medical Service System(EMSS) through the review of the 119 transport chart of 1,229 patients who visited to emergency center of Chonnam University Hospital via 119 system during the period from January 1998 to December 1998. The results were obtained as follows: 1) 1,229(4.9%) of the 25,253 patients were transported to emergency center via 119 system. Among 1.229 patients, male were 691(56%) and female 538(44%), and nontraumatic patients were 946(77%) and traumatic patients 283(23%). The peak age was 5th to 6th decades. 2) Mean arrival time to scene(ambulance response time) was 4.8 +/-.8minutes and mean arrival time to hospital 19.6 +/-0.1minutes. In the distribution of the requested place for ambulance, 73% was house and 23% was field. 3) In the severity of patients, 154(12.5%) patients were classified into emergent, 442(36%) into urgent, and 633(51.5%) into non-emergent. Among 1,229 patients, number of patients admitted were 419cases(34.1%), operated 85(6.9%), discharged in emergency department 536(43.6%), dead 70(5.7%), and transferred 119(9.7%), respectively. 4) The assessment of prehospital treatment by Emergency Medical Technician(EMT) revealed that checking the vital sign was only 49.5%, and prehospital care was limited to airway management, O2 inhalation and immobilization. There was no case of the notification to medical institute or the consultation to doctor. These results show that over the half of patients transported to level III emergency center via 119 system were classified into non-emergent and the prehospital management of the patient by EMT was not adequate. This study suggests the need of more simple and objective triage guideline for patient transport, the improvement of prehospital care system, and reconstruction of the computerized communication system.

4.The Significance of the Antral Follicle Number Prior to Stimulation in Predicting the Outcome of Assisted Reproductive Technologies.

BACKGROUND: Recruitment and development of multiple follicles in response to gonadotropin stimulation are the key factors leading to successful treatment by assisted reproductive technologies. Prediction of ovarian responses prior to stimulation is useful in counselling and helpful in adjusting the dosage of gonadotropin to individual patients. OBJECTIVE: To evaluate the number of antral follicles of both ovaries as a predictor of outcome of assisted reproductive technologies. METHODS: Data were collected by a retrospective analysis. A total of 56 consecutive infertile women were undergoing the first cycle using a standard regimen of ovarian stimulation from July, 2000 to June, 2001. Basal FSH, LH, prolactin, and estradiol concentration were measured. The number of follicles (2-5 mm) were recorded. RESULTS: In women with fewer antral follicles, the number of retrieved eggs was significantly decreased and increased the duration and dosage of human menopausal gonadotropin, but pregnancy rate was similar to those with a higher number of antral follicles. CONCLUSION: It is suggested that the total antral follicle number is sensitive ultrasound parameter in predicting the ovarian response.

5.Efficacy of Biometry Using Swept-source Optical Coherence Tomography for Posterior Chamber Phakic Intraocular Lens Implantation

Purpose:
The proper implantable collamer lens (ICL) size affects ICL stability. This study compared device efficacy using white-to-white diameter (WTW) measurements with Orbscan II and IOL Master 700.
Methods:
We retrospectively studied 90 eyes (45 patients) who underwent toric ICL implantation from January 2019 to February 2020 and were followed for 1 year. The correlation between WTW and anterior chamber depth (ACD) for each measuring device was analyzed.
Results:
The mean WTW measured by IOL Master 700 and Orbscan II was 12.2 ± 0.3 and 11.6 ± 0.3 mm, respectively, while the mean ACD was 3.28 ± 0.16 and 3.20 ± 0.15 mm. The WTW and ACD measured with IOL Master 700 averaged 0.57 ± 0.12 and 0.08 ± 0.04 mm larger than with Orbscan II. The differences were significant and the regression analysis had high correlations (R2 = 0.875 and R2 = 0.913, respectively; both p < 0.001).
Conclusions
WTW measured by the IOL Master 700 can be used as a reference either alone or together with the Orbscan II value to determine ICL size.
6.Analysis of Cardiopulmonary Resuscitation In Ward of Tertiary Hospital.

BACKGROUND: Although cardiopulmonary resuscitation(CPR) is a very effective therapy in cardiac arrest, it is hard to prove the true effectiveness of CPR. Several studies about out-of-hospital and emergency department CPR exist, but only a few reports about in-hospital CPR are available. This study was designed to investigate in-hospital cardiac arrest, to analyze the result of CPR, and to evaluate the problems associated with in-hospital CPR. METHODS: A clinical analysis of 71 cases of in-hospital CPR announcement from January 2000 to August 2000 was performed. The initial rhythm on cardiac arrest, return of spontaneous circulation(ROSC), and the survivals were analyzed in the case of the 46 true cardiac arrest patients. RESULTS: During 8 months, there were 71 cases of in-hospital CPR announcement. Among them, there were 46 cases of true cardiac arrest and 25 cases of non-cardiac arrest. Of the 46 true cardiac-arrest cases, 27(58.7%) experienced ROSC, 15(32.6) survived for over 24 hours, and 7(15.2%) survived to be discharged. The initial rhythms on cardiac arrest were 30 cases(65.2%) of asystole, 14(30.4%) of PEA(pulseless electrical activity), and 2(4.3%) of ventricular fibrillation, with ROSC being 17 cases(56.7%), 9(64.3%) and 1(50.0%) cases and discharged survivors being 4 cases(13.3%), 3(21.4%) and 0(0.0%) cases, respectively. CONCLUSION: Extraordinarily high proportions of asystole and PEA were seen in the initial rhythm of cardiac arrest, and those were associated with high survival rates. Although further study is needed to evaluate the course leading to this high proportion of asystole and PEA, this result suggests that if the EMS system in the hospital is activated promptly and systematically, a better outcome will be achieved in case of cardiac arrest with asystole and PEA.

Tuberculomas of the lung are rare in children and one of the more common lesions presenting a solitary pulmonary nodule roetgenorgraphically. We are reporting of a 3-year-old child with a tuberculoma in left upper lobe. The patient was initially diagnosed as the benign mediastinal tumor but in the end as tuberculoma in left uper lobe. Wedge resection including the mass was done. The tumor had brown smooth external surfaces on sectioning including the mass was done. The tumor had brown smooth external surfaces on sectioning show pale gray and soft cut surface was shown. In light electromicroscopy chronic granulomas with multinucleated giant cells and central caseous necrosis were observed which are the characteristics of tuberculoma. The postopeative course was smooth and uneventful and patient has been well for 4 months postoperatively.

8.A Case of Papillary Serous Carcinoma of Endometrium.

Uterine papillary serous carcinoma (UPSC) has been recognized as an aggresive tumor with early and deep myometrial invasion, frequent lympho-vascular space involvement, and a high relapse rate. It has also been shown that deep myometrial invasion cannot predict the risk of extrauterine disease. UPSC accounts for 2-10% of all endometrial cancer and generally occurs in postmenopausal women (mean age 66 years) who usually present with abnormal vaginal bleeding. Obesity, diabetes, hypertension, or a history of previous hormone replacement treatment, known as risk factors of endometrial cancer, are not usually seen in women with UPSC. In this paper, we present a case of uterine papillary serous carcinoma with obesity, hypertension, diabetes. The initial endometrial biopsy showed an endometrioid adenocarcinoma but the result of postoperative pathologic finding revealed UPSC with extension to less than half thickness of the muscle layer and involvement of pelvic lymph node.

9.Altered Expression of beta3 Integrin on Sclerotic Aortic Valves in a Hypercholesterolemic Rabbit Model.

BACKGROUND: Although aortic valve sclerosis causes no significant hemodynamic alterations, it is associated with an increased risk of cardiovascular death and myocardial infarction. However, the role of beta3 integrin in aortic valve sclerosis remains unclear. MATERIAL AND METHOD: Twenty male New Zealand rabbits were divided into two groups. Group 1 rabbits (n=10) received a normal chow diet, while group 2 (n=10) rabbits received a diet containing 1% cholesterol for 12 weeks. After the rabbits were euthanized, their aortic valves and ascending aortas were excised for analysis. RESULT: Total serum cholesterol (2,148.3+/-1,012.5 mg/dL versus 53.7+/-31.8 mg/dL, p<0.05), triglyceride (240.4+/-218.3 mg/dL versus 31.6+/-6.4 mg/dL, p<0.05), and low density lipoprotein (LDL)-cholesterol (2,065.3+/-960.9 mg/dL versus 29.1+/-30.9 mg/dL, p<0.05) levels were significantly higher in the cholesterol diet group compared with the normal diet group. Myofibroblasts and macrophages were more highly expressed in the aortic valve leaflets of rabbits in the cholesterol diet group than of those in the normal diet group. A real-time polymerase chain reaction revealed decreased beta3 integrin mRNA levels in the hypercholesterolemic aortic valves and aortas. CONCLUSION: The present study shows that hypercholesterolemia induces aortic valve sclerosis. These findings suggest that alterations in beta3 integrin may play a role in the development of aortic valve sclerosis.

10.A case of Laparoscopic Removal of Perforated Intrauteirne device.

Intrauterine devices (IUDs) have a low complication rate and minimal side effects and are pervaded contraceptive method. Perforation of the uterus by an IUD is very rare. But perforation of the uterus is one of the most serious complications associated with insertion of IUD. During the puerperium when the uterus is small and the uterine wall is thin the risk of perforation increase. The frequency has been estimated between 0.05 and 13 per 1000 insertions. When the IUD strings are not visible during pelvic examination, physicians make efforts to locate the IUD. We experienced a laparosopic removal of perforated intrauterine device, which had been inserted on postpartum 5 th week, at posterior lower segment of uterus with intrauterine pregnancy. So we report a case with a brief review of the literature.
