1.A Case of Premature Sebaceous Gland Hyperplasia with Zosteriform Distribution.

We experienced an atypical case of sebaceous gland hyperplssis in a 28-year old male. He had asymptomatic, multiple, globoid, yellowish papules in a zosteriform distribution on the right lower abdomen (T8 dermatome) that were of 5 years duration. Histologically, there are many hyperplastic lobules of sebaceous glands, some of which are connected with a central dilated duct.

2.The correlation between cranial base size, shape and head posture, and the position of maxillo-facial structures.

This study was done to evaluate the correlations berween the size, the form of the cranial base, head posture and the horizontal and vertical position of craniofacial structures. For this purpose, 100 cephalometric radiographs were taken from the sample composed of 51 male and 49 female, 12 measurement criteria and 37 reference points were established and digitized, then calculation was performed for the values of measurement variables and the horizontal and vertical position of reference points. The correlations between them were analyzed statistically and mean facial diagrams were constructed and compared with the selected groups which were composed of 10 Samples each as large and small group from the measurement value. The following results were obtained : 1. The angles n-s-ba and n-s-ar as variables for the form of cranial base correlated hight to the horizontal and vertical position of reference points in the cervical column with statistical significance(0.1% level). 2. The angles n-s-ba and n-s-ar as variables for the form of cranial base correlated to the horizontal position of the reference points in the facial stucture with statistical significance(1% level), but not to the vertical position of them(5% level). 3. The length n-s, s-ba, and n-ar as variables for the size of cranial base were correlated th the position of eranioracial structures in various ways, but in general, highly correlated to the horizontal and vertical position of midfacial structures around the teeth and alveolar area. 4. The angle NAL/CVT and NSL/OPT as postural variables for the inlination if cranial base and cervical column were correlated to the horizintal position of the craniofacial structures with statistical significance(1% level), but not to the vertical position of them(5% level). 5. The angle OPT/HOR and CVT/HOR as postural variables for the inclination of cranial base and ture horizontal line were not correlated to the horizintal and vertical position of the craniofacial structures with statistical significance(5% level). 6. The correlation between the measurement variables and horizontal and vertical positions of the reference poits in soft tissue were shown as similar to the related hard tissue points.

3.Treatment for Fracture of the Humeral Neck Using a Device of External Fixation

Fractures of the humeral neck are relatively common, but their anatomical complexities make closed reduction difficult sometimes, and in case of reduction it is tough to maintain the position. Even open reduction of the fracture also faces multiple problems including poor functional recovery of the shoulder joint. The Department of Orthopedic Surgery of Seoul Adventist Hospital developed a device of external fixation for 13 humeral neck fractures. We used this method for displaced and unstable fractures of humeral neck from December 1987 to March 1989. Satisfactory results were obtained in 11 cases and poor results in 2 cases. We think our method is a effective useful way of treatment for humeral neck fractures, and its main advantages are as follows:1. Using proximal and distal bars, fracture can be easily reduced and firmly maintained. 2. Active motion exercise can be started early and functional recovery of the shoulder joint can be highly predictable. 3. It is quite convenient for treating combined fracture or external wound of same extremity.

4.A Case of Metastatic Renal Cell Carcinoma to the Gallbladder.

Metastatic renal cell carcinoma is common, but dissemination to the gallbladder is extrem-ely rare. The patient was diagnosed as having distal CBD stone and renal cell carcinoma before she underwent operation, but postoperative pathologic examination revealed clear cell type renal cell carcinoma within the gallbladder. We report a case of renal cell carcinoma with synchronous metastasis to the gallbladder.

5.The Awareness and Countermeasures against Harmful Effect of Passive Smoking in Korean Adults.

OBJECTIVES: This study is to evaluate the awareness, attitude, practice and the countermeasures against passive smoking in Korean adults. METHODS: By self-administered questionnaires, we assessed the knowledge, attitude, behavior for passive smoking and the countermeasure for reduction of it's harmful effects in 289 men and 238 women. RESULTS: The subjects that have heard about passive smoking were 96.8% in total and well known were 26.4% of current smoker, 56.6% of ex-smoker, and 14.8% of non-smoker(p=0.001). The irritative symptom from passive smoking was the most frequent in non-smokers and the most common place where exposed to passive smoking was public place. For attitude against passive smoking in 'no smoking allowed area', ex-smokers were the most active to recommend to stop smoking. And for opinion about establishment of 'no smoking allowed area', the restriction by law was the best acceptable method in smokers, exsmokers, and nonsmokers. In marking of 'no smoking allowed area', 69.9% of smokers answered no smoking, but in non-marking area only 6.3% stop smoking. When smokers were recommended to stop smoking, the more subjects stop smoking with good feeling in marking area, but the less in non-marking area. The factor associated the high awareness of passive smoking were aged(OR=1.07, 1.03-1.12), men(OR=4.34, 2.32-8.46). The persons who have known well about passive smoking had good attitude and behavior to prevent of harmful effect of passive smoking. CONCLUSIONS: This study suggested that education program would be necessary to reduce the passive smoking.

7.Comparing the Use of Single and Double Interlocking Distal Screws on a Polarus Intramedullary Nail for Humeral Shaft Fractures.

BACKGROUND: Our aim was to make a comparative analysis of radiological and clinical outcomes of using either one or two interlocking distal screws on a Polarus intramedullary nail for the internal fixation of humeral shaft fractures. METHODS: From January 2008 to March 2014, we enrolled 26 patients with humeral shaft fractures who were operated on using intramedullary nails. The patients were divided into 2 groups according to how many interlocking distal screws were used to lock the Polarus nail: in group 1, a single interlocking distal screw was used in 12 patients; and in group 2, double interlocking distal screws, in 14 patients. We compared the degree of recovery of the displaced fracture fragments between the two groups. To compare the nonunion and shoulder function, we assessed each patient's modified American Shoulder and Elbow Surgerns (ASES) score. RESULTS: We found that 10 of 12 fractures achieved union in group 1, and 13 of 14 fractures, in group 2. We did not find a meaningful difference in the time to bone union between the two groups. The percentage of recovery of displaced fracture fragments until union was 66.9% for group 1 and 59.41% for group 2. At the final follow-up, we found that the scores for shoulder joint modified ASES was 78.7 for group 1 and 80.7 for group 2. CONCLUSIONS: Our results show that if locked appropriately, even a single screw on a Polarus nail can provide satisfactory radiological union and improved clinical outcome after intramedullary nailing of humeral shaft fractures.

9.Allopurinol Induced Abnormalities of Liver Function Test in Gout Patients.

Liver function tests before treatment showed abnormalities of liver function tests during treatment, while 15(68. 2%) of 22 patients who had abnormal liver function tests before treatment showed abnormalities during treatment. In 12 of the 25 patients who showed abnormalities of liver function tests during treatment with allopurinol, allopurinol was stopped and all patients showed improvement of liver function tests. In remaining 13 patients, 10 patients were improved and other 2 patients showed only mild abnormalities of liver function tests despite of continuing allopurinol and 1 patient was lost during follow-up. CONCLUSION: Abnormalities of liver function tests were common during treatment with allopurinol. Most patiensts who had mild abnormalities of liver functions tests during treatment with allopurinol were improved regardless of continuing allopurinol.

10.Endothelial Ca2+ signaling-dependent vasodilation through transient receptor potential channels

Ca2+ signaling of endothelial cells plays a critical role in controlling blood flow and pressure in small arteries and arterioles. As the impairment of endothelial function is closely associated with cardiovascular diseases (e.g., atherosclerosis, stroke, and hypertension), endothelial Ca2+ signaling mechanisms have received substantial attention. Increases in endothelial intracellular Ca2+ concentrations promote the synthesis and release of endothelial-derived hyperpolarizing factors (EDHFs, e.g., nitric oxide, prostacyclin, or K+ efflux) or directly result in endothelial-dependent hyperpolarization (EDH). These physiological alterations modulate vascular contractility and cause marked vasodilation in resistance arteries. Transient receptor potential (TRP) channels are nonselective cation channels that are present in the endothelium, vascular smooth muscle cells, or perivascular/sensory nerves. TRP channels are activated by diverse stimuli and are considered key biological apparatuses for the Ca2+ influx-dependent regulation of vasomotor reactivity in resistance arteries. Ca2+- permeable TRP channels, which are primarily found at spatially restricted microdomains in endothelial cells (e.g., myoendothelial projections), have a large unitary or binary conductance and contribute to EDHFs or EDH-induced vasodilation in concert with the activation of intermediate/small conductance Ca2+-sensitive K+ channels. It is likely that endothelial TRP channel dysfunction is related to the dysregulation of endothelial Ca2+ signaling and in turn gives rise to vascular-related diseases such as hypertension. Thus, investigations on the role of Ca2+ dynamics via TRP channels in endothelial cells are required to further comprehend how vascular tone or perfusion pressure are regulated in normal and pathophysiological conditions.
