1.Spectral Analysis of REM Sleep EEG in Narcolepsy and REM Sleep Behavior Disorder.

INTRODUCTION: It has been proposed that narcolepsy and REM sleep behavior disorder (RBD) have overlapped symptom profile and pathophysiology. This study was aimed at measuring and comparing changes in EEG frequency band of REM sleep in narcolepsy and RBD, applying EEG spectral analysis method. METHODS: Nine patients diagnosed as narcolepsy and the same number of RBD patients were studied. Spectral analysis of the REM sleep EEG was performed in each patient on 9 epochs selected evenly from the first, second, and third REM periods. Then, we compared frequency band percentages of REM sleep EEG in narcolepsy and RBD. RESULTS: Narcolepsy patients had significantly higher delta frequency ratio than RBD ones (p=0.00). In alpha and beta2 frequency bands, RBD patients showed higher percentage than narcolepsy ones. Slow wave sleep was more prevalent in narcolepsy patients. But, no difference of REM sleep percentage was found between the two groups (p=0.93). CONCLUSION: Higher delta frequency ratio in REM sleep of narcolepsy patients than RBD ones reflects that sleep-promoting mechanism is more dominant in narcolepsy than in RBD.

2.Cutaneous Bronchogenic Cyst.

A 45 year old male patient complained of mucous fluid drainage in the vicinity af the manubrium sterni for 5 years. Skin around the lesion had normal appearance except pinpoint sized sinus opening. H.istopathology showed a cyst in the upper and mid-dermis which consisted of pseudoetratified columnar epithelium with interspersed goblet cells. Goblet; cells were confirmed by positive stain to D-PAS and alcian blue and by metachromatic stain to toluidine blue. VFe present here a case of cutaneous bronchogenic cyst clinically and histopathologically which is thought to be the first cace in Korea.

3.Cervical Syringomyelia: A Case Report

Syringomyelia was described by Etienne et al in 1954 and was named by Ollivier in 1827 by reason of the pathologic cavitation of cord. Also it is considered a very chronic and irregulary progressive disease of the spinal cord of unknown cause which has predilection for the cervical and lumbar enlargements. The patient of 19 years old male had been suffered from the painless, severe swelling and instability of right elbow after trauma on right elbow, aud had complained of the sensory and temperature dissociation, muscle weakness with clawing hand in right upper extremity and scoliosis of mild degree. These symptoms and findings were same as those of the classic syringomyelia. Therefore the syningomyelia which is relatively rare in incidence will be reviewed by this report with the references.

5.Internal Fixation Using Clavicle Hook Plates for Distal Clavicle Fractures.

BACKGROUND: To report the radiological and clinical outcomes of internal fixation using distal clavicle hook plates for distal clavicle fractures. METHODS: From April 2008 to December 2012, 32 patients with distal clavicle fractures underwent surgery using an AO hook plate. The reduction was qualified and evaluated according to the radiological findings. The evaluation of the clinical outcomes was performed with the University of California at Los Angeles (UCLA) score, the Korean Shoulder score, and the visual analogue scale (VAS) pain score. RESULTS: By radiological evaluation, we found that 31 of 32 patients showed anatomical reduction and solid bone union. Although we obtained satisfactory UCLA scores, Korean Shoulder Scale scores, and VAS pain scores, 12 cases of complications were present. We found 4 cases of osteolysis of the acromion, 1 case of nonunion, 3 cases of periprosthetic fractures, 3 cases of subacromial pain, and 1 case of skin irritation. We performed re-operations in 2 patients. CONCLUSIONS: To avoid complications associated with clavicle hook plates, choosing the appropriate hook size and bending of the hook according to the slope of the acromion undersurface is critical. Also, we believe that early removal of clavicle plates may help reduce complications.

7.A Case of Renal Cell Carcinoma Metastatic to the Scalp.

Cutaneous metastasis from renal cell carcinoma is unusual and have been reported infrequently. We report a case of metastatic renal cell carcinoma in a 33-year-old man who presented with a 3.5X2.0 cm-sized round, brightly red colored nodule on the scalp for 1 month. A skin biopsy revealed the typical clear cell adenocarcinoma with clear cytoplasm and oval hyperchromatic nuclei arranged in glandular pattern consistent with metastatic renal cell carcinoma. In most instances, once cutaneous involvement is manifested the disease is widespread and has a poor prognosis. But, our case presented only a single cutaneous nodule, and workup showed no evidence of further metastasis for 1 year.

9.A case of persistent pustulosis and antecedent scabies.

In 1978, Bjornberg and Friis reported 32 patients with persistent pusi ulosis(PP) on distal extremities, of whom 13 patients were initially diagnosed as scabies. In 1984, Elpern described 7 patients with infantile acropustulosis(IA), of whom 4 patients had antecedant scabies. We report herein a case of PP, which was found in a 18 month-old mal who was treated for scabies at the age of 5 month and 12 month. The pruritic vesicopustules dev',lo ed 2 months after the second treatment for scabies and relapsed for 4 months. We also discuss the relationship between scabies and PP and also between PP and IA.

10.Application of Cumulus Cells as Factors to Predict the Outcome of IVF-ET.

OBJECTIVE: To establish the evaluation system of the quality of oocytes on the basis of the incidence of cumulus cells apoptosis, to investigate the relationships between the incidence of cumulus cells and the outcomes of IVF-ET. METHOD: Thirth-four cycles undergoing controlled ovarian hypersimulation for IVF-ET with tubal infertility (23 cycles) or unexplained infertility (11 cycles) were included in this study. Cumulus cell masses surrounding mature oocyte and co-culture of embryos with autologous cumulus cells during IVF-ET process. The incidence of apoptosis in cumulus cells was assessed by apoptosis detection kit fluorescein. The effect of co-culture using cumulus cells and the incidence of cumulus cells apoptosis. RESULTS: The results were as follows: 1. The incidence of apoptosis in cumulus cells markedly increased in patients aged 40 or over, while the fertilization rate was greatly decreased in those age group. 2. Apoptosis in cumulus cells was found in both the fertilized oocytes and unfertilized oocytes, but the incidence of apoptosis was higher in unfertilized oocytes. 3. There is no clear correlation between apoptosis in cumulus cells and the number of oocytes retrieved. However, the incidence of apoptosis was increased when the number of oocytes retrieved was 5 and fewer in comparison with 6~10. 4. Embryo grade was significantly affected by the incidence of apoptosis in cumulus cells. 5. Pregnancy rate of IVF-ET per cycle was 29.4%, and the pregnant group had the higher fertilization rate and a significantly lower incidence of apoptosis in cumulus cells compared with the nonpregnant group. 6. When cumulus cells were used as helper cells in the co-culture of the embryo, in vitro activity of cumulus cells based on morphological change and proliferation did not influence the quality of embryo, but was closely associated with the implantation rate and pregnancy rate, which was enhanced when morphological changes and proliferation of cumulus cells was more active. 7. This difference in the outcome of IVF-ET according to in vitro activity of cumulus cells used for co-cultue was not associated with the incidence of apoptosis in cumulus cells, but rather had likely relations with the different secretion pattern of protein, which may be an embryotrophic factor by cumulus cells. CONCLUSION: These results suggest that the incidence of apoptosis in cumulus cells can be used in predicting oocyte qualities and the outcomes of IVF-ET. And the effect of co-culture largely depends on the in vitro activity of cumulus cells as well.
