We evaluated Immunochromatographic assay kit to screen HBsAg in human serum. When the reference HBsAg was applyed to ICA, HA and EIA kits, the limit of detection for HBsAg were found out to be 4, 2 and 0.25 ng/ml respectively. But ICA kit required 5 minutes to read the result whereas HA and EIA kit more than one hour. The sensitivity was 97% (29 of 30 samples) and the specificity 100% (45 samples) compared with conventional EIA. The ICA kit needs no instrument or machine to perform the test contrary to the conventional methods. Therefore, this rapid and sensitive ICA kit can be used for HBsAg-screening, especially in the emergency room and in the scene of the accident.

Adenoid cystic carcinoma is a rare disease of which incidence is 10-15 % of bartholins gland carcinoma. The clinical presentation is characterized by a vulval mass that existed for a prolonged period to the onset of symptoms, usually infection, pain, and burningsensation Histologically, this tumor is characterized by cribriform pattern cell growing and perineural invasion. The obtaining of clear surgical margins is the most important aspect of treatment. Here we report a case of adenoid cystic carcinoma of the Bartholins gland with a brief review of literature.

5.A Case of Kimura's Disease with an Unusual Location and Clinical Manifestation.

Kimura's disease is a chronic inflammatory condition of unknown cause. It usually appears as a painless nodule in the head and neck region often associated with regional lymphadenopathy, but there have rarely been reports of penile involvement or clinical presentation as an ulcerative plaque. We present a case of Kimura's disease occurring on the penile shaft as an ulcerative plaque in a 44-year-old man. A vesicular eruption had occurred on his penile shaft followed by an ulcerative plaque for 2 months. He had suffered from recurrent regional lymphadenopathy for 15 years and he had also exhibited intermittent eosinophilia. Histopathologic findings revealed diffuse eosinophilic infiltration in the dermis of the skin and eosmophilic abscesses in the lymphoid follicles of the mguinal lymph node. After short-term systemic steroid therapy and local injection of triamcinolone acetonide, the ulcerative plaque was healed.

6.Retinal Hemorrhages in Newborn Infants.

In newoborn infants, we can find the retinal hemorrhage, hyphema, vitreous hemorrhage, cataract, iridodialysis, choroidal rupture, lens dislocation and the injuries to the conjunctiva and lid due to birth injury. We studied the frequency, influencing factors, shape and absorption period of retinal hemorrhages in newborn infants by the following method. The 152 newborn infants that were delivered at the obstetric department of S.C.H. college hospital from May 25th '80 to June 29th '80 are examined with direct ophthalmscope after instillation of 1% mydriacyl solution. Fundus eyamination was done within 24 hour after delivery, 3rd day, 2nd week, and 3rd week. 1) The retinal hemorrhage is seen in 19.1% of newborn infant. 2) Bilaterality is over 55%. 3) Frequency is higher in primipara than in multipara and higher in over 30 years old mother and vaccum deliveries. 4) The retinal hemorrhages are absorpted about 1/2 of cases within 3 days and almost within 3 weeks.

7.Rapid Tracheal Intubation with High-dose Vecuronium.

For the rapid endotracheal intubation, the ideal neuromuscular blocking drug with short onset time, short duration and few side effects has so far not been found. Succinylcholine is still, inspite of its side-effects and contraindications, the standard drug of choice for rapid intubation. But, high-dose vecuronium as an alternative to succinylcholine for rapid intubation may be recommended. To compare the intubating conditions with that in succinylcholine, the onset and cardiovascular effects of high-dose vecuronium(0.3 mg/kg) were evaluated clinically with a scoring system. The results are as follows ; 1) High-dose vecuronium may provide an alternative means of achieving a rapid onset of neuromuscular blockade. 2) High-dose vecuronium shows minimal effects on cardiovascular system. In concluson, high-dose vecuronium as an alternative method for rapid endotracheal intubation may be recommended without any significant cardiovascular change if succinylcholine is contraindicated.

9.Unusual Prolonged Recovery of Succinylcholine - induced Neuromuscular Block Following Neostigmin.

We report a case of slow recovery of the neuromuscular block induced by succinylcholine (SCC) immediately after administration of neostigmine for the reversal of pancuronium. Left hemithyroidectomy was performed in a 4l kg female patient under general endotracheal inhalation anesthesia with 50%-nitrous oxide and 2.0%-enflurane. Four mg of pancuronium was used for muscular relaxation to facilitate the controlled ventilaion on the anesthesia ventilator. At the end of the operation, one and a half hours from the injection of pancuronium, 10 mg of pyridostigmine was administered to reverse the neuromuscular blockade. Five minutes later, 25 mg of succinylcholine was given for reintubation. Following this event, train-of-four stimuli was applied under the uncalibrated mode with Datex Relaxograph because apnea followed by SCC appeared for more than 30 minutes. Neuromuscular monitoring revealed fade to less than 50% of the T, ratio. The ventilator was applied for the recovery of neuromuscular blockade for one hour. There was no more late complications after transfer to the ward. The literature relating to this iatrogenic complication and phase II block after SCC is reviewed and discussed.
