2.Effect of Pancuronium into the Lateral Ventricle on the Repiratory and Circulatory Functions in Rabbit.

In this study the action of pancuronium given directly into the lateral ventricle on the mean blood pressure, heart rate, respiratory frequency and skeletal muscular contractility of the rabbit were investigated. Pancuronium, which a dose of 10ug/kg and 30kg/kg were given into the lateral ventricle of rabbit. 1) Intraventricular injection of 10ug/kg of pancuronium increased pulse rate, respiratory frequency and muscle contraction and the differences were not statiscally significant. 2) Intraventricular injection of 30ug/kg of pancuronium produced a gradual increase of respiratory frequency which began at 5 minutes after drug injection and continued over a half hour. The difference between control and experimental value was statistically significant (p<0.005) while pulse rate and muscular contractility were increased but not significant. 3) Mean blood pressure was not changed after intraventricular injection of the pancuronium.

3.Quality of Life in Patients with Hematopoietic Stem Cell Transplant.

PURPOSE: This descriptive study was to investigate the quality of life in patients with hematopoietic stem cell transplantation (HSCT) from June 1 to October 13, 2007. METHOD: The survey was conducted in 6 different university hospitals which located in Seoul and Jellanamdo province using the Functional Assessment of Cancer Therapy-BMT Scale (FACT-BMT) version 4. We collected a total of 155 questionnaires and analyzed 149 among them. RESULTS: The average score of quality of life was 2.53 out of 5. Physical well being score was highest among sub-domains, followed by emotional well-being, additional concerns, social/family well-being, and functional well-being. Study subjects worried that their conditions would get worse. However study subjects didn't regret having been received HSCT. Age, duration from HSCT, age at diagnosis, income, readmission, HSCT type, educational background, marital status, and the level of activities of daily living were related to quality of life. CONCLUSIONS: The findings of this study indicates that the HSCT survivor's quality of life issue is still important and have to be investigated repeatedly in the future. That is necessary for generalizing QOL outcomes for clinical use. We also suggest to develop interventions to improve QOL.

5.The differences of opinion on Boramae incident between physiscians and common people.

BACKGROUND: Without available medical policy, up until now discharges against medical advice have occurred in th field of medicine in Korea. Seoul district court had convicted two physicians of crime because they had allowed a patient to discharge against medical advice, referred to as the Boramae incident in May of 1998. This shocked people in medical field as well as people and appeared in almost all newspapers and created a debate especially to physicians. The purpose of this study was to evaluate the differences of opinion of this incident between physicians and common people. METHODS: Ninety-nine physicians and ninety-nine common people were questioned from September 1 to September 30, 1998 about sociodemographic characteristics and opinion of this event. RESULTS: The mean age of the physician group, the non-physician group was 38.12+/-11.2, 38.5+/-11.1, respectively. Each group consisted of 87.9 percent, men and 12.1 percent, women. There were no differences between physicians and common people in experiences of discharges against medical advice in their families, decisions on Boramae-like incidents that could occur in the near future, and causes of discharges against medical advice. The physician group had more awareness rate of this incident than the non-physician group, and insisted more on the needs of methods to prevent recurrence. In opinion of the judgement, 5.1 percent of non-physician group agreed to conviction of Seoul district court. 22.2 percent of the non-physician group was of the opinion that this incident will affect patient care positively, about patient wifes decision, 21.2 precent of non-physician group considered that she made an imprudent decision. CONCLUSION: There were significant, differences between physicians and common people in the awareness of this incident, opinion on th judgement, needs of methods to prevent recurrence, the influence of this incident on patient care in the future, opinion of patient wifes decision, exceptional experiences of discharges against medical advice in a family, decisions on Boramae-like incidents that can occur in the near future, and causes of discharges against medical advice.

6.Congenital Chylothorax.

Chyle is the digestive product absorbed from intestinal lymphatics which the component have high concentration of triglyceride and protein. The various pathological circumstances which give rise to intrathoracic effusion of chyle are well documented and so the etiology is clearly defined in most instances. However, the occurrence of chylothorax in early infancy, in the absence of other demonstrable diseases and without apparent birth trauma, suggests the existence of congenital malformation of the thoracic ducts. We lately experienced a 20 day old new born who was proved to have congenital chylothorax by clinical history and laboratory findings. So we report the clinical findings and laboratory findings with the review of the literatures on congenital chylothorax.

7.Right Ventricular Myxoma Obstructing Right Ventricular Outflow Tract.

Cardiac myxoma is the most common primary tumor of the heart, but right ventricular myxoma causing outflow obstruction is relatively rare. A 15 years old girl developed dyspnea on exertion and intermittent syncope caused by a right ventricular mass obstructing the right ventricle outflow tract. Transthoracic echocardiography revealed 3.6 x 3.0 cm sized pedunculated subpumonic mass originating from the right ventricular anterior free wall. The patient underwent an emergency operation, consisting of the removal of the mass by wide excision of the tumor base and PTFE (polytetrafluoroethylene) patching of the right ventricular anterior free wall defect. Pathological findings of the mass were compatible with myxoma, and the patient was discharged uneventfully 7 days after the operation.

9.Tension Pneomothorax Related to General Anesthesia - 3 cases.

Pneumothorax was recognized as a potential hazard of mechanical ventilation after tracheal intubation. Excessive pressure the trachea during general anesthesia could produce lung rupture and pneumothorax. The incidence of pneumothorax has increased as a result of increased application of invasive diagnostic and therapeutic procedures. Emphysematous bleb rupture afterlaryngospasm, bronchospasm, or cough may be causes of pneumothorax. Tension pneumothorax during anesthesia is often difficult to diagnosis, but it needs prompt dignose and treatment. We experienced 3 cases of tension pneumothorax during and after general anesthesia.

10.TENSILE STRENGTH OF LASER WELDED-TITANIUM AND GOLD ALLOYS.

Lasers have given dentistry a new rapid, economic, and accurate technique for metal joining. Although laser welding has been recommended as an accurate technique, there are some limitations with this technique. For example, the two joining surfaces must have a tight-fitting contact, which may be difficult to achieve in some situations. The tensile samples used for this study were made from a custom-made pure titanium and type III gold alloy plates. 27 of 33 specimens were sectioned perpendicular to their long axis with a carborundum disk and water coolant. Six specimens remained and served as the control group. A group of 6 specimens was posed as butt joints in custom parallel positioning device with a feeler gauge at each of three gaps : 0.00, 0.25, and 0.50mm. All specimens were then machined to produce a uniform cross-sectional dimension, none of the specimens was subjected to any subsequent form of heat treatment. Scanning electron microscopy was performed on representative tested specimens at fractured surfaces in both the parent metal and the weld. Vickers hardness was measured at the center of the welds with a micropenetrometer using a force of 300gm for 15 seconds. Measurement was made at approximately 200 ,am and 500 pm deep from each surface. One-way analysis of variance (ANOVA) and Scheffe' s test was calculated to detect differences between groups. The purpose of this study is to compare the strength and properties of the joint achieved at various butt joint gaps by the laser welding of type III gold alloy and pure titanium tensile specimens in an argon atmosphere. The results of this study were as follows 1. When indexing and welding pure titanium, there was no decrease in ultimate tensile strength as compared with the unsectioned alloys for indexing gaps of 0.00 to 0.50mm, although with increasing gap size may come increased distortion (p>0.05). 2. When indexing and welding type III gold alloy, there were significant differences in ultimate tensile strength among groups with weld gaps of 0.00mm, 0.25 and 0.50mm, and the control group. Group with butt contact without weld gap demonstrated a significant higher ultimate tensile strength than groups with weld gaps of 0.25 and 0.50mm (p<0.05) . 3. When indexing and welding the different metal combination of type III gold alloy and pure titanium, there were significant differences in ultimate tensile strength between groups with weld gaps of 0.00, 0.25, and 0.50mm. However, the mechanical properties of the welded joint would become too brittle to be acceptable clinically (p<0.05) . 4. The presence of large pores in the laser welded joint appears to be the most important factor in controlling the tensile strength of the weld in both pure titanium and type III gold alloy.
