1.Clinical Study of Low Back Pain

Low back pain has been recognized as a general symptom rather than a specific disease entity, especially when musculoskeletal lesions of the low back, hysteric conversion symptoms and visceral lesions are present. Today, the development of industry, the complexity of living methods and the execution of medical insurance have increased the number of the patients who have low back pain. In view of the suggestion that the patients who have attacks of low back pain are increasing in our society, and that, at the present time, there is little clinical study of it, the author considered that a study of his current experience would be of some general interest. In this thesis the records of 413 in-patients, whose chief complaint was low back pain and who were treated as such at Han Yang University Hospital, Department of Orthopedics between the periods from May 1972 to April 1978, were studied. The following results can be recognized by observing and analyzing their charts, physical findings and X-ray findings. 1. Of the patients suffering from lumbago, 33.2% were caused by the discogenic origin. Thus, the discogenic orgin was the most common cause of Iumbago, and male:55.4%, female: 4.6%. 2. According to simple L-S spine X-ray findings of the discogenic origin, 35.8%, of the decrease of the lumbar lordotic curve; 30.7% of the narrowing of the intervertebral space. Besides them, some cases of the lumbar scoliosis, osteoarthritic changes, soteoporosis, Knuttson's phenomenon and Schmorls node were appeared. And, 15.3% though they were thought to be the discogenic origin according to their symptoms and the physical findings, had no specific abnormal findings, suggesting that the patients are relatively young and their duration of lumbago is short. 3. Of the lumbar disc herniation, the herniation between the fifth lumbar and the first sacrum was most frequently appeared, 49.5% and the herniation between the fourth lumbar and fifth lumbar formed 41.3%, and the herniation between the third and the fourth lumbar was 2.8%. 4. Of spondylolistheses, the highest proportion, 66.7%, occured in the fifth lumbar level. And in the degree of slipping, 96.3% came under the grade 1 of Meyerding's method. 5. The frequency of anomaly, among the patients with lumbago, appeared as follows; spina bifida formed 12.6%: sacralization, 8.0%: lumbarization, 4.6%: tropism, 3.6%. 6. Of the patients with discogenic origin, 91.2% were under conservative treatment and 8.8% were under operative treatment. 7. Of the patients with Tbc spine, 35.9% were under conservative treatment, and 64.1% under operative treatment. Among the methods of operation, curettage and anterior interbody fusion was 80%: posterior interbody fusion, 12%: incision and drainage, 8%. 8. In the treatment of spondylolistheses, 66.7% were under conservative treatment, and 33.3% under operative treatment. In the operatlve methods, 6 cases were performed by posterior spinal fuslon by H-sahped bone graft: and 3 cases, postero-lateral spinal fusion.

2.Two cases of aquamous cell carcinoma of the lower lip treated with liquid nitrogen.

Squamous cell carcinoma of the lower lip can be developec by a number of underlying etiologic factors, the most outstanding of which is solar irracdiation, and is considerably more aggresive and has a much higher incidence of metastatic spread. Its early detection and effective treatment afford a high cure rate. Cryosurgery is a simple, efficient and effective method of treatment of skin cancer and is now a widely accepted modality for the treatment of these lesions. Two patients(67-year-old male and 57-year-old female) were presented with solitary or two masses on the lower lip without regional lymphadenopathy. These two cases showed clinical and histopathologic findings of squamous cell carcinoma. They were successfully treated by cryotherapy with liquid nitrogen without any significant side effects.

3.A Case of Granular Cell Tumor.

We report a case of granular cell tumor in a 62-year- old female. She had a 1. 5 cm sized, asymptomatic, firm nodule with central depression on the lateral aspect cf the left orbit. Histologically, the dermis had many large, pale tumor cells arranged in clusters and strands. The cells had a distinct cellular membrane and a pale cytoplasm filled with faintly eosiinophilic,coarse granules. The characteristic faint granules within the tumor cells are PAS-positive and diastase-resistant. The overlying epidermiis showed pseudocarcinomatous hyperplasia. We treated it with total excision.

4.A Case of Histoid Leproma with Mycobacterium leprae Detected in the Peripheral Blood by polymerase Chain Reaction.

Histoid leproma usually occur in pat.ients with lepromatous leproy who have had relapsed after being undcr treatment(usually sulfones) for a long time and who have shown variable improvement. A 56 year old female patient who had been suffering from lepromatous leprosy and had been treated which clapsone relatively regularly for approximately 28 years had a history of several shiny rice grain t.o peanut sized nodules on the face and right flank. Histopathologic examination of a specimen taken from a nodular lesian revealed a well circirmscrihed nodule composed mainly of spindle-shaped cells and epithe:ioi 1 cells on the upper and mid-levmis, and granuloma composed of epithelioid cells, multinucleated wait cells and foamy histiocytes on the lower dermis. In AFB stain, numerous acid-fast bacilli were scatt red dermis and epidermis, especially stratum corneum. This study opplied a polymease chain reaction(PCR) on sections of biopsy tissues and peripheral blood of pat.ient. Positive amplification of a 530 bp fragment was deteated not only in the lesion tissues but also in the per ipher al blood.

5.Immunohistochemical Identification of Keratin in Cutaneous Tumors.

Sixty cutaneous tumors of different types were investigated by means of immunoperoxidase technique(avidin-biotm-peroxidase complex method) with the use of antikeratin antibody. The results were as follows: 1. Squamous cell carcinomas, keratoacanthomas, and Bowen's disease exhibited strong reactivity with antikeratin antibody. 2. In squamous cell carcinomas and keratoacanthomas, the most heaviest staining patterns were detected in areas of keratinization and horn pearl formation. 3. Basal cell carcinomas and seborrheic keratosis were moderately stained, whereas Paget'a disease and metastatic carcinoma(oat cell carcinoma) did not contain keratin. 4. In some cases of basal cell carcinomas, the tumor cells were partially positive. 5. Malignant melanomas, nevi, malignant.lymphomas, neural tumors, hemangiomas, and dermatofibromas were uniformly negative.

6.A Cases of Cutaneous Metastasis Probably Originating From Stomach Carcinoma.

Cutaneous metastases from internal carcinornas are relatively rare. The frequency of metastases to the skin varies from 0.5-4.4%. We report a case of cutaneous metastasis suspected to originat from stomach cancer. A 44-year-old female, who had an episode of total gastrectony under diagnosis of stornach cancer three years ago, had multipe painless erythernatous nodules on the case and neck for one year. Histologic findings shows atypical anaplastic cell infiltration at the derr is and subcutaneous tissues. Cells with hyperchrornatic and erally displaced nuclei were recogniz d PAS staining shows positive reactions within the cells of glarid like structure. She died twelve month after the appearance of the cutaneous lesions.

7.A case of leukemia cutis.

Leukemia cutis is defined as infiltration of the skin by malignant leukocytes that have originated from the bone marrow. The cutaneous eruption of leukcmia may be divided into the specific and non-specific lesion (leukemid). These may occur separately or in combination. True leukemia infiltrations of the skin consist of nodular, diffuse, or plaque-like infiltrations with swelling, macules, papules, ecchymoses, palpable purpura, erythroderma, and ulceration. A 55-year-old man presented with multiple as mptomatic dark-brown nodules on the whole body with recurrent gingival bleeding. The patient developed seoticemia and died 6 months after the onset of the disease.

9.A case of xanthoma showing unusual clinical manifestation.

We report an atypieal case of xanthoma without evidence of underlying disease, A 50-year-old asymptomatic, yellowish, flat plaques on the upper eyelids, trunk and thighs, and huge, deep-seated tumors on anterior aspect of the neck, back and ankle areas of 3 years duration. Biopsies were performed at the lesions of the neck and hack. Two biopsy specimens showed the same findings of xanthoma, respectively. The appearance of refrigerated serum was clear. Paper electrophoresis didn't show elevation of any lipoprotein, suggesting normolipoproteinemia.

10.Detection of Mycobacterium leprae in Tissue and Blood by Polymerase Chain Reaction.

BACKGROUND: Methods to detect and quanitify Mycobacterium leprae(M. leprae)are needed for studies involving the epidemiology, pathogenesis, and chemotherapy of leprosy. Serological assays and skin tests lack the sensitivity and specificity to serve as diagnostic tool for M. leprae infection. The polymerase chain reaction(PCR) based on the selective amplification of an 530-bp frangment of the gene encoding the proline-rich antigen of M. leprae was performed with sections of fixed or frozen biopsy samples from leprosy patients. OBJECTIVE: This study was done to investigate the applicability of PCR for the detection of low numbers of M. leprae in tissues and peripheral blood. METHODS: The PCR was used to amplify a 530-base-pair M. leprae DNA with the thermoxtable Taq DNA polymerase. RESULTS: The In frozen skin tissues and peripheral blood of leprosy patients. relatively high detection rates of PCR products was achieved by using direct gel analysis as well as Southern blot hybridization. CONCLUSION: These results suggest that PCR amplification for the detection of M. leprae may be useful for the epidemiologic study of large papulations as well as coinical astudies on the individual patients.
