2.A Case of Dandy-Walker Syndrome with Chromosomal Abnormality.

The Dandy-Walker syndrome is a developmental disorders of the brain characterized by cystic deformity of the 4th ventricle and agensis of the cerebellar vermis. Other systemic anomalies and chromosomal abnormalities are associated with this syndrome. We are experienced a case in a 9 months old male infant who presented initially with frequent vomiting, low birth weight, On the physical examination, a prominent occiput, palpable mass below the right upper quadrant, pulmonary valve stenosis, congenital dislocation of the hips, ventral flexion of fingers, clubfoots and the rocker-bottom deformities of feet were present. On the chromosomal study, there were chromosomal polymorphisms in a thickened C-band of chromosome No. 1 by C-banding method. The brain CT revealed a large, thin-walled, low density mass of CSF without enhancement in the posterior fossa, showing upward displacement of cerebellar hemisphere with absent inferior vermis(or associated with dysplastic cerebellar hemisphere). A brief review of the related literatures were included in this report.

3.Immunohistochemical Analysis of Cutaneous Pseudolymphoma.

BACKGROUND: In the skin, it is often difficult to differentiate lymphomas from reactive lymphoid lesions by light microscopic examination. OBJECTIVE: Our purpose was to determine whether immunologic data obtained from mutine-processed specimens could be used to further objective morphologic interpretations. METHODS: We conducted an immunohistochcmical staining in 44 cases of benign and malignant cutaneous lymphoproliferative lesions using nine antibodies, including anti-CD3, UCHL1, MT1, MT2, L26, MB2, BerH2, 123C3, and MIB1. RESULTS: 1. Immunophenotyping with anti-CD3, UCHL1, MT1, L26, and MB2 was useful for the diagnosis of T cell or B cell lymphoma. However, these antibodies showed a lack of specificity for neoplastic cells, 2. Antibody to CD56, 123C3 showed positivity in 4 cases of angiocentric lymphoma, but negativity in 8 cases showing angiocentric lymphoma-like pathology. 3. Antibody to CD30, BerH2 showed positivity in all 6 cases of CD30 positive large cell lymphoma, but negativity in 6 cases showing diffuse lymphoma-like pathology. 4. Antibody to Ki-67, MIB1 showed positivity in more than 30% of infiltrating cells in 6 cases of angiocentric lymphoma, 4 cases of diffuse B cell lymphoma, and in more than 60% of infiltrating cells in 6 cases of CD30 positive large cell lymphoma. CONCLUSION: These observations suggest that immunostaining may provide useful adjunctive information in distinguishing benign from malignant cutaneous lymphoproliferations in paraffin sections.

5.Orbital Venography.

Orbital venography was performed on four eyes which showed the unilateral exophthalmos and were suspected as orbital tumors. One eye was confirmed in the nature and location of tumor by the tophographic finding and tumor-staining figure of venography. One eye was confirmed in only the location of the tumor by surgery, which is consistent with the venographic picture. The other two eyes were not confirmed by surgery but we can speculate the some space occupying lesion by viewing the topogra phic findings of venography. Neither complication nor sequela was observed during or after the venographic procedures. We think orbital venography is preferably valuable for diagnosing the orbital lesion.

6.A Case of Atypical Hemolytic Uremic Syndrome Associated with Invasive Streptococcus pneumoniae Infection.

The recently proposed Bethesda system for cervical/vaginal cytology has made a standardization related to "atypia". In cellular changes due to inflammation or repair, the word "benign cellular change" has been suggested as a substitute for atypia. Terminology related to atypical cells may become standardized, but the cytologic criteria has not been well defined yet. We evaluated 160 cases of atypical squamous cells of undetermined significance(ASCUS) by the Bethesda System(TBS). Among 30,428 cases screened, a cytologic diagnosis of ASCUS was made in 498 cases(1.6%) and 160 cases were histologically verified. ASCUS was diagnosed based on nuclear enlargement and nonclassical signs of condyloma. The results are as follows: One hundred and twenty three cases(76.9%) revealed chronic cervicitis. Thirty seven cases(23.1%) demonstrated squamous intraepithelial lesion. Among intraepithelial lesions, condyloma and mild dysplasia were 28 cases(75.7%). Moderate and severe dysplasia were 5 cases(13.5%) and 4 cases(10.8%), respectively. It is concluded that patients with ASCUS should be colposcopically examined.

7.Congenital Anomaly of Urinary Tract in Children.

Giant cell tumor (GCT) occurs very unusually in the rib (less than 1% of GCT). We present the cytologic features of GCT of the rib. It showed multiple cellular clusters composed of characteristic, benign looking osteoclast-like multinucleated giant cells and fibroblast-like mononuclear cells. The multinucleated giant cells contained numerous nuclei (average, 30 to 40 per cell, which were closely packed. The nuclei in giant cells were remarkably uniform and round to oval. The mononuclear, neoplastic stromal cells were elongated and spindle-shaped, There was no cytologically malignant portion in the tumor.

10.Usefulness of Interdigital Nerve Conduction Studies of the Foot for the Detection of Early Polyneuropathy.

BACKGROUND AND PURPOSE: Frequently the conventional nerve conduction studies (NCS) with testing usual peripheral nerves did not reveal any abnormalities in patients with early polyneuropathy. Recently Lee and Oh demonstrated that the interdigital sensory nerve conduction studies (NCS) of the foot were very sensitive in detecting early changes of neuropathy. The authors performed the study to see abnormal features of interdigital NCS in early phase of polyneuropathy and to understand the sensitivity of interdigital NCS in making diagnosis of polyneuropathy. METHODS AND MATERIALS: The interdigital NCS of the foot were performed in 19 clinically suspected polyneuropathy with no abnormalities by the conventional NCS (group I), in 7 polyneuropathy with electrophysiological abnormalities by the conventional NCS (group II), and 10 normal controls without clinical or electrophysiological findings of polyneuropathy (group III). The interdigital NCS were done according to the original description of Oh et al ,using the near-nerve needle recording at the level of ankle with averaging technique. The maximal nerve conduction velocity (NCV), the largest negative NCV, the amplitude and the duration of compound nerve action potential (CNAP) were analysed in each recording of I,V digital and I-II, II-III, III-IV, IV-V interdigital nerves of the foot. RESULTS: The abnormal interdigital NCS findings were found in 9 out of 19 patients in group I (47.4% ) , 7 out of 7 patients in group II(100.0%) ,and none in group III.(0.0%). In group I, the most common abnormal findings were no recordable CNAP (77,7%) and the second were the decreased amplitude of CNAP (66.6%). The decreased NCV were found only in one case ( group I-19). The digital or interdigital nerves involvement were symmetrical and the number of involved branches ranged from one to twelve, being averaged to be 7. In group II, the most common abnormalities were no recordable or decreased amplitude of CNAP (85.7 %) and the decreased NCV were found in 1 out of 7. Three out of 7 showed abnormalities in all digital or interdigital nerves and the remaining four showed abnormal findings in at least 4 among 12 digital or interdigital nerves. CONCLUSION: The interdigital NCS with the near nerve needle and averaging technique increased the diagnostic sensitivity in those subjects with clinically suspected polyneuropathy (group I) and the most common abnormal features in the interdigital NCS were the amplitude changes of CNAP. Therefore the authors suggest that the interdigital nerves of the foot are involved earlier in the process of polyneuropathy and the interdigital NCS of the foot might provide enhanced sensitivity for detecting early polyneuropathy.
