2.Comparing the efficacy of a high pressure spray oral hygiene appliance and a sonic vibration toothbrush in reducing dental plaque.

OBJECTIVES: This study aims to compare home care dental devices for their dental plaque removal ability. METHODS: A single blinded randomized crossover clinical study design was used to measure plaque index (Turesky Modification Quigley-Hein Plaque Index). RESULTS: All 3 groups showed a statistically significant decrease in the plaque index after usage (P<0.001). In manual tooth-brushing group, the teeth on the right dental arch showed a greater decrease in the plaque index than on the left arch (P=0.041). All appliances showed greater plaque reduction in the upper right posterior teeth than in the lower right posterior teeth (P=0.009, 0.004, 0.007). CONCLUSIONS: The results of this study support development of more effective oral hygiene appliances and emphasis on oral hygienic education.

3.Traumatic Asphyxia: 3 Case Report.

Traumatic asphyxia is a clinical symptom complex characterized by craniofacial cyanosis, subconjunctival hemorrhage, and head and neck vascular engorgement due to sudden compressive injury on the thoracic cage. It is occasionally combined with mental deterioration, lung contusion, and edema. It is considered due to increased intrathoracic pressure in state of closed epiglottis. Recently, we had experienced three cases of traumatic asphyxia of which clinical courses were somewhat different. The first case developed by a compression between a elevator and the ground while the victim did not realized the accident happening, and the patient showed nonspecific facial edema and ecchymosis but mental deterioration and ultimate respiratory failure. The second case by a compression between cars, while the impending accident was noticed by the victim, showed full-blown asphyxia without mental or respiratory symptoms. The last case by forceful coughing and vomiting showed facial edema and ecchymosis without any other symptoms. Hospital courses of all cases were uneventful. We believe that 'fear response' or 'closure of the epiglottis' might be an important mechanism on developing symptom of traumatic asphyxia.

4.Clinical Type and Treatment Response in Patients with Digital Mucous Cyst: A Retrospective Study

Background:
Digital mucous cysts (DMCs) are benign, occurring mainly on the fingers but rarely on the toes.Although the exact cause of DMC has not been identified, it is classified into two different types based on its pathogenesis and location. Surgical and nonsurgical procedures, including aspiration, sclerotherapy, steroid injection, cryotherapy, and CO 2 laser, can be used to treat DMC.
Objective:
This study aimed to compare treatment response based on DMC type and evaluate its efficacy in surgical and nonsurgical (sclerotherapy) treatments.
Methods:
We retrospectively reviewed the electronic medical records of patients with DMC who visited our dermatology department between January 2010 and November 2020.
Results:
This study enrolled 39 patients having a mean age of 59.2 years. The proportion of female patients (21/39, 53.8%) was more than male (18/39, 46.2%). The DMC occurrence was more frequent in the fingers (32/39, 82.1%), especially in the index and middle fingers than in the toes (7/39, 17.9%). In sclerotherapy, DMC in the distal interphalangeal joint (DIPJ) required more treatment than that in the proximal nail fold (PNF) (p<0.05). The number of treatments using sclerotherapy was higher compared to that of surgical treatments (p<0.05).
Conclusion
The DIPJ type of DMC patients was more difficult to treat using sclerotherapy than the PNF type.Moreover, surgical treatment has a better response than sclerotherapy.
5.Surgical Treatment for Riedel's Thyroiditis: a Case Report.

Riedel's thyroiditis is a rare fibrotic condition that results in the destruction of the thyroid and infiltration into surrounding tissues. The exact etiology is not yet clear, although systemic fibrosing disorder, a variant of Hashimoto's thyroiditis, a primary inflammatory disorder of the thyroid, and even a manifestation of end-stage subacute thyroiditis has been suggested. Although various treatments have been applied, no definitive treatment has yet been established. We report a case of Riedel's thyroiditis treated without complications using microscopic surgery. A 54-year-old man visited our clinic presenting with neck tightness and a left neck mass. A gun biopsy revealed a benign thyroid mass, although the radiologic findings showed a malignant thyroid tumor with invasion into the trachea and strap muscles. The patient underwent a left hemi-thyroidectomy and shaving of the trachea, esophagus and recurrent laryngeal nerve under microscopy. The final pathology revealed Riedel's thyroiditis combined with Hashimoto's thyroiditis. The patient had symptomatic relief without vocal fold paralysis and hypocalcemia. Surgical treatment using microscopic dissection can be considered to be one of treatment option for Riedel's thyroiditis.

6.Measurement of thermal expansion characteristic of root canal filling materials: Gutta-percha and Resilon.

The purpose of this study was to evaluate the thermal expansion characteristics of injectable thermoplasticized gutta-perchas and a Resilon. The materials investigated are Obtura gutta-percha, Diadent gutta-percha, E&Q Gutta-percha Bar and Epiphany (Resilon). The temperature at the heating chamber orifice of an Obtura II syringe and the extruded gutta-percha from the tip of both 23- and 20-gauge needle was determined using a Digital thermometer. A cylindrical ceramic mold was fabricated for thermal expansion test, which was 27 mm long, with an internal bore diameter of 3 mm and an outer diameter of 10 mm. The mold was filled with each experimental material and barrel ends were closed with two ceramic plunger. The samples in ceramic molds were heated in a dilatometer over the temperature range from 25degrees C to 75degrees C. From the change of specimen length as a function of temperature, the coefficients of thermal expansion were determined. There was no statistical difference between four materials in the thermal expansion in the range from 35degrees C to 55degrees C (p > 0.05). However, Obtura Gutta-percha showed smaller thermal expansion than Diadent and Metadent ones from 35degrees C to 75degrees C (p < 0.05). The thermal expansion of Epiphany was similar to those of the other gutta-percha groups.

7.The relationship of pit and fissure sealant in children and mother's socioeconomic status, mother's oral-health screening.

OBJECTIVES: This study examines the relationship between the implementation of pit and fissure sealant in children, and the prevention level of teeth based on household income by a cross-sectional study. METHODS: The study population consisted of a representative community sample of 1,701 subjects, ranging from 6-18 years in children and over 27 years in mothers, from the 4th Korea National Health and Nutrition Examination Surveys in 2009. Mother's indicators included age, occupation, education, household income, smoking, brushing frequency, oral health screening, self-related oral health, and mother's concern. We used a multiple logistic regression to identify and quantify the risk factors for the pit and fissure sealant. RESULTS: In multiple analyses, age of children, household income level, and oral health screening were significant factors for the implementation of pit and fissure sealant for the children. The multiple-adjusted odds ratio (95% confidence interval) for the implementation of pit and fissure sealant, comparing the lowest to 101-200, 201-300, 301-400, 401-500 and 501 ten thousand Korean Won or more were 2.2 (1.15-4.10), 2.5 (1.38-4.61), 2.0 (1.04-3.90), 2.8 (1.46-5.53), and 3.6 (1.82-6.94), respectively. CONCLUSIONS: It was concluded that the implementations of the pit and fissure sealant were positively related with the household income level and mother's oral health screening.

9.A Case of Propionic Acidemia.

Propionic acidemia is an autosomal-recessive inborn error of branched-chain amino acid metabolism. It is caused by deficient activity of propionyl-coenzyme A carboxylase and is characterized by a spectrum of clinical and biochemical findings. It usually manifests in the neonatal period or early infancy. Since Childs et al first described the propionic acidemia of infants in 1961, it has rarely been reported. There have been no previous report of this organic acidemia in Korea. We present a case of propionic acidemia in a 4-day old male, who had poor feeding, dehydration, and hyperammonemia and died at 12 days of age. Diagnosis was established by gas chromatography and mass spectrometry, and this case is the first reported propionic acidemia in literature in Korea. A review of the related literature was included.

10.Moderately Differentiated Neuroendocrine Carcinoma of the Larynx.

Neuroendocrine neoplasm of the larynx is a morphologically heterogeneous group of tumors. This unusual neoplasm includes the four different type of tumors. Paraganglioma is the best-known neural origin tumor. The epithelial origin tumor is further divided based on the degree of differentiation into well, moderately, and poorly differentiated carcinoma. The diverse biological behavior of neuroendocrine neoplasm of larynx makes an accurate diagnosis of paramount importance, since treatment depends on diagnostic accuracy. The diagnosis is based primarily on light microscopy, and immunohistochemical and/or ultrastructural investigations are needed to diagnose correctly. The mainstay of treatment of moderately differentiated neuroendocrine carcinoma of the larynx is surgery. Elective neck treatment should be performed considering the high likelihood of cervical lymph node metastases. The 5-yr survival rate approximates 50%, and 10-yr survival rate is only 30%.
