3.A Case of Malignant Lymphoma Associated with Herpes Zoster, Neurogenic Bladder and Acquired Ichthyosis.

Herpes zoster is a viral infection of the dorsal nerve root causing painful vesicular eruptions over the skin or mucous membrane of the affected nerve. It is less well known to the dermatologists that berpes zoster causcs bladder dysfunction when the zoster affects the lumbar and sacral spinal ganglia. The authors observed a case of herpes zoster involving the S 2~4 dermatomes, associated with bladder atony in a 58 year old male who had received rad.iotherapy and chemotherapy for his malignant Iymphoma of lymphocytic type. Conservative treatment with urinary catheterization and antibiotics resulted in complete recovery of the bladder function about 3 weeks later. He had also ichthyosiform skin lesions on his back and lower extremities which, by carefuI history and histopathological examination, proved to be acquired ichthyosis.

4.Staining Resistance of The Soft Denture Liners.

This study was investigated to compare the staining resistance of soft denture liners. Specimens wee made of Coe-soft. Coe-Comfort, Soft-liner, Visco-gel, and were stored in 1% methyleneblue solution for 24 hours. The amounts of color change before and after treatment with mono-poly and thermocycling were measured by colorimeter(TC-6FX, Tokyo Denshoku Co. Ltd, Japan) for evaluation of staining resistance. The following conclusions were drawn from this study. 1. The staining resistance of Visco-gel was increased, but there was no changer of staining resistance in Coe-soft, Coe-comfort, and Soft-liner after treatment with monopoly. 2. The staining resistance of the Coe-comfort was the least in all soft denture liners. 3. The staining resistance of Visco-gel and Soft-liner were decreased after thermocycling.

5.Research on the frequency of Terry's nail in the medical inpatients with chronic illnesses.

Terrys nail(TN) might be causally associated with cirrhosis or other disorders, but his criteria have been proved to be inadequate reflection of systcmic disorders. Modified criteria of Holzberg and Walker were introduced in TN examination in 1984. Using these criteria, fingernails were studied in 444 medical inpatients with chronic systemic diseases at Taegu Catholic Hospital from March 1991 to March 1992. There were 136(30.6%) patients with TN and 308(69.4%) patients without TN, respectively. Ninety-three patients with TN showed a signifiart association with cirrosis(49/86, 57%, p<0.01 by the x-test), congestive heart failure 18/35, 515, p<0.05), and diabetes mellitus(25/51, 49%, p<0.05), and otherwise not;ignificantly associated with chronic renal failure(19%), and cancer(18%). Male and female patients with TN were affected about equally. Age distribution in The patients with TN showed 35(25.8%) the age group from twenty to fortynine and 101(74.2%) in the age group from fifty and above with a mean age of 57.3+14.0, whereas the patients without TN had 178(57.8%), and 130(42.2%), with 47.4+16.9, respectively. The greater part was distributed in the older group(p<0.001 by student t-test), suggestive of an increased risk in older group. The average number of nails affected per patient was tending to be higher in frequency close to the thumb, with an equal frequency in six. Thirty-nine patients(28.7%) had all nails affected. The distal band with was 0.5 to 3.0mm averaging 1.0+0.5mm with no difference, in sex. The distal band was pink in 43(31.6%) of the 136 patients with TN, brown in 62(45.6%) and intermediate in color in 13(22,8%). The results of routine laboratory test such as hemogram, eiythrocyte sedimenation rate, liver enzyme, triglyceride, protein, albumin, blood urea nitrogen and creatinine were statistically insignificant between the patients with and without TN.

8.Effects of Interleukin 4 on the Production of Interleukin 6 in Human Keratinocytes.

BACKGROUND: The human keratinocyte can synthesize interleukin 6 (IL-6) under certain conditions, and the IL-6 synthesis is inhibited by interleukin 4 (IL-4) in the human monocyte. OBJECTIVE: To find out what kind of stimulating agents can induce the IL-6 production and whether IL-4 affects the production of IL-6 in the human cultured keratinocytes. METHODS: We stimulated the keratinocytes with either lipopolysaccharide (LPS), fetal bovine serum (FBS), human recombinant interferon-gamma(IFN-gamma) to induce the IL-6 production, and treated the keratinocytes, which stimulated either with 10% FBS or human recombinant IFN-gamma, with human recombinant IL-4. RESULTS: The LPS stimulation resulted in no increase of IL-6 levels in the keratinocyte supernatants. When the keratinocytes were stimulated either with 1%, 5%, 10% FBS with or without 5 microgram/ml LPS, significantly increased amounts of IL-6 were detected. The level of IL-6 in the keratinocytes treated with the human recombinant IFN-gamma increased, too. The human recombinant IL-4 downregulates the secretion of IL-6 by the keratinocytes which were activated either with 10% FBS or human recombinant IFN-gamma. CONCLUSION: We have shown that it is possible to induce the IL-6 synthesis by stimulating the keratinocytes and that human recombinant IL-4 profoundly inhibits the synthesis of IL-6. So we suggest that there may be a cytokine network which regulates the primary immune response in the skin.
