2.Clinical Observation on the Cryo-extraction of the Lens in Cataract.

This study is based on the results of cryo-extraction of the lens in 123 eyes(98 cases) which were operated in Chosun University Hospital between Jan. 1979 and Feb. 1981. The results are summarized as follow: 1. The cryo-extraction have been done for 41 males and 57 females of the 98 patients. 2. The incidences of complication during operation were capsule rupture (5.7%), vitreous loss (4.9%), iris prolapse (1.6%). 3. The incidences of early postoperative complication were hyphema (12.1%). updra wn pupil (8.9%), corneal edema (6.5%), flat anterior chamber (4.9%). 4. The incidences of late postoperative complication were vitreous bulging (5.7%), vitreous opacity (4%), glaucoma(2.4%), uveitis (0.8%), retinal detachment (0.8%). 5. The ccrrected visual acuities before discharge of hospital were: more than 0.5 ... 39.0% (48 eyes), 0.1-0.4 ... 48.8% (60 eyes), less than 0.1 ... 12.2% (15 eyes). 6. The major causes of visual acuity less than 0.1 were vitreous opacity (33.4%), secondary glaucoma (20%), proliferative diabetic retinopathy (20%), corneal opacity (13.4%), retinal degeneration (13.3%).

4.Effect of Tending Diancibo Pu on Burn Treatment.

PURPOSE: Recently, in an experiment using animals, radiation therapy using a laser or Tending Diancibo Pu (TDP) has been shown to be effective in treating scars on the skin by increasing the production of fibroblast cell and collagen and by accelerating the process of epithelization. The purpose of this study is to examine the effectiveness of TDP radiation therapy in treating human burn injuries in terms of the frequency of treatment, the timing of eschar separation, and the duration of treatment. METHODS: In the treatment group, the burn area of the patients was first sterilized with saline solution and potadin solution and covered with one vaseline gauze. Then, the burn area was radiated every other day by using TDP for 20 minutes at distance of 20-25 cm, at radiant plate temperature of 250-280oC. In the cases of control group, the burn area of the patients was first sterilized with saline solution and potadin solution and covered with one vaseline gauze. Then, the area was covered with one burn gauze and bandaged. The treatment was conducted every other day. RESULTS: In cases of superficial second degree burn injuries, the difference between the treatment and the control groups was 1.34 in terms of the frequency of treatment. In the cases of both superficial and deep second degree burns, the differences between the treatment and the control groups were 3.47 in terms of the frequency of treatment, 0.63 weeks in terms of the timing of eschar separation, and 6.03 days in terms of the duration of treatment. All these differences were statistically significant. CONCLUSION: From the experiment, it can be concluded that TDP radiation therapy is more effective in treating human burn injuries than conventional treatment in terms of the of the frequency of treatment, the timing of eschar separation, and the duration of treatment.

5.Two Cases of Malignant Melanoma in the Bulbar Conjunctiva.

Two cases of malignant melanoma which are located in the bulbar conjunctiva are presented. A case of 56 years old male revealed melanosis in his medial palpebral conjunctiva and wedge shaped resection was performed. The other case of 76 years old male had dark brownish strawberry like pedunclated mass in his lateral bulbar conjunctiva and limbal area with history of progressive enlargement. This tumor mass was excised radically. Above two cases were confirmed as malignant melanoma histopathologically.

6.A Case of Corrosive Gastritis Caused by Hydrochloric Acid.

Recently the use of hydrochloric acid as a toilet disinfectant has become more common, and the risk of its ingestion is though to be increasing. We experienced a case of 67 year-old man who accidentally ingested hydrochloric acid and as a result developed a postprandial epigastric fullness end pain 3 weeks thereafter, The patient underwent an UGI series, fiberoptic gastroscope and abdominal ultrasonography and was found to have a gastric midbody stricture. A total gastreetomy and Roux-en- Y esophagojejunostomy was performed. We report this case with brief review of the literature.

7.Polymorphonuclear CSF Pleocytosis during the Treatment of Tuberculous Meningitis.

Among 73 patients with possible and definite tuberculous meningitis, 14 cases showed a sudden unexpected polymorphonuclear (PMN) CSF pleocytosis during treatment. Patients with superimposed bacterial meningitis were excluded. Eleven patients(15. 1%) matched inclusion criteria. The intervals between the onset of the treatment and the onset of the PMN CSF pleocytosis were 7-54 days(mean 17.2+ 14.4 days). The mean duration of PMN CSF pleocytosis was 14.2+12.4 days. A PMN CSF pleocytosis may develop occasionally weeks or months after the start of the treatment for tuberculous meningitis. Though the cause is uncertain, we suggest that probably its cause is superimposed acute meningeal inflanunation by the release of Mycobacterium from tuberculomas or.delayed Jarisch-Herxheimer reaction.

8.A Case of Yellow Nail Syndrome.

We report a 58 year old man who complained that his nails didn't grow for a year and developed yellowish discoloration. On physical examination his nails showed yellow greenish discoloration, increased corvexity, loss of lunulae and cuticles. C]iest X ray revealed atelectasis on right lower lung field and bronchiectasis on left lower lung field. Mild restrictive pattern was observed in pulmoniry function test. We gave him 800IU of Vitamin E claily for 3 months, but rio remarkable changes have been observed as yet..

9.3 Cases of Interstitial Keratitis Occurred in Congenital Syphilitic Patients.

Since the time of Jonathan Hutchinson (1858-63), it has been known that over 90% of cases of diffuse interstitial keratitis occur in syphilitic patients. The syphilitic patients was sharply decreased throughout the world after the introduction of penicillin for syphilis therapy, but recently the incidence of syphilis began to increase throughout the world. The authors experienced 3 cases of interstitial keratitis due to congenital syphilis. All the cases showed interstitial keratitis, positive serologic test for syphilis (VDRL) and Hutchinson's teeth. A brief review and relating literatures are presented.
