Transplants

Although the primary repair has been recognized so far, the tendon graft is reported as a primary treatment method for tendon injury in no man's land of Bunnel, judging from the state of injured hand, the ability of operator and the environment of medical operation. The 27 cases of tendon graft that were operated for 11 years from June, 1973 to June, 1984 in the Department of Orthopedic Surgery, Hanyang University College of Medicine have been followed up for clinical study. The results were as follows: 1. The Material of this study was composed of 13 males (21 cases) and 5 females (6 cases) whose age were from 8 to 39 years old. 2. The injured fingers were classified with 3 thumbs, 6 index fingers, 7 middle fingers, 8 ring fingers and and 3 little fingers out of 27 cases and all injured parts of fingers were Zone II except that of thumb which was Zone IV of Bunnel. 6 patients were injured more than 2 fingers. 3. The duration from tendon injury to tendon graft which scattered from 1 month to 9 years and averaged out to 28.2 months is supposed to have little relation to post-operative results. 4. The post-operative results showed up 9 excellent, 7 good and 1 fair out of 17 cases in good gmup, 3 good and 6 fair out of 9 cases in cicatrix group, 1 fair out of 1 case in nerve damage group at pre-operative state. 5. The post-operative results of thumb which show up 2 excellent, 1 good out of 3 cases were most successful among those of all fingers. 6. The pre-operative state is supposed to have great relation to post-operative state judging from 94% satisfactory results of good group at pre-operative state. 7. As multiple injury makes little difference from single injury, the injured state of each hand is supposed to an important effect on post-operative results. 8. As all electric bumed injury show up fair except that of thumb, they are supposed to have unsatisfactory results.

The paroxysmal kinesigenic choreoathetosis is characterized by choreathetoic, dystonic and/or tonic movements induced by sudden voluntary movements, without loss of consciousness, urinary incontinence and clonic movement. These attacks begin in childhood, and are occasionally familial. The response to anticonvulsants is usually excellent. We present 4 cases of paroxysmal kinesigenic choreoathetosis with a brief review of literature, which is believed to be the first report in Korea.

Pagets disease is a chronic skeletal disease of middle and later life, beginning insidiously and characterized by progressive structural changes and typical deformities occuring in the long bones, pelvis, and cranium. The occurence of Pagets disease is extremely rare in Korea. We have experienced a case of Pagets disease of bone in 67 year-old female affecting mainly the ilium and skull, and we report the case with brief review of the literatures.

Pain, premature degenerative arthritis, stiffness and persisting subluxation are common sequalae of fracture dislocations of the proximal interphalangeal joints of the fingers. With disruption of greater than 40% of the volar articular surface of the middle phalanx, it is un- likely that congrous stable reduction can be achieved. And the most severe injuries are caused by longitudinal compression force, which drives the middle phalanx where the middle phalanx eventually comes to rest. Many authors had reported a variety of method for the treatment of the unstable fracture dislocation with no satisfactory results except Eaton(1980). In his ten years experience the volar plate arthroplasty for acute and chronic fracture dislocations got a good results with satisfactory range of motion and no pain on motion. Authors had been treated two cases of chronic fracture dislocations of the proximal interphalangeal joint with Eatons volar plate arthroplasty followed by good results in one year followed-up study.

Development of acetabulum and epiphysis of the femoral head following reduction in seventy-four patients with ninty-one congenitally dislocated hips were assessed radiologically. Acetabular index, CE angle of Wiberg, neck-shaft angle of the femur and overgrowth of the pelvis and femur were measured according to the various treatment. To evaluate results, the method of treatment was divided into four groups; closed reduction, open reduction, open reduction combined with pelvic osteotomy and modified Klisic operation (open reduction, pelvic osteotomy, femoral osteotomy and so on). Each treatment was compared with three subgroups according to ages; first subgroup was belong to patients under one year of age, second from one year to three years, third over three years. This paper agrees to Harris (1976) and Lindstrom (1979) about improvement of acetabular index and CE angle of Wiberg that showed a rapid rate of repair in the first 6 months and thereafter slowly progressed to normal range of degree. Overgrowth of pelvis and femur was also noticed in cases of hips treated pelvic and/or femoral osteotomy, moreover even in cases of open reduction only. The goal of this preliminary investigation may be supposed to propose the idea which method of treatment to the first visiting patient will approach the most satisfactory final result.

BACKGROUND: Tracheal tubes are different on the flexibility, material and bevel angle according to the manufacturers. These may affect the success rate of blind tracheal intubation through laryngeal mask airway (LMA). In addition, using straight-tip exchanger or curved-tip introducer, we tried to compare the success rate of tracheal tube insertion through LMA. METHODS: After receiving informed consents, 30 patients were enrolled. Blind tracheal intubation was tried with reinforced tracheal tube (M group), or one of two kinds of conventional tracheal tube (P group and B group). Tracheal tube was selected in random order and advanced into the trachea through LMA one by one. If all the three attempts was unsuccessful, we considered the blind tracheal intubation through LMA a failure. We also compared the success rate of blind tracheal insertion through LMA using straight-tip exchanger or curved-tip introducer. The first trial was performed on the neutral head position. The exchanger or introducer was rotated during the second trial. If the second attempt was unsuccessful, the exchanger- or intubator-guided intubation was performed on the sniffing position and, as a last and fourth trial, while mandible was being elevated with hands. RESULTS: Success rate of blind tracheal intubation through LMA was 50% (15/50) in group P and 36.7% (11/30) in group M, 33.3% (10/30) in group B (P=0.05). By using curved-tip introducer, intubation through LMA was successful in 80% (16/20), while it was successful only in 35% (7/20) via straight-tip exchanger. CONCLUSION: Blind tracheal intubation through LMA was dependent on the types of tracheal tube. Also, curved tip introducer may be a more valuable aid in performing blind tracheal intubation through LMA. Its use seems to be better technique than directly inserting tracheal tube through LMA.
