2.A Case of Malignant Melanoma on the Hypothenar Region of Right Hand: Case Report

Most of malignant melanoma of skin had occurred in transformation from benign nevi(Webster, 1944), especially most frequently from junctional nevi of them. which was characterized by flat surface, dark-brown color and hairlessness(Ackermen, 1948) Blumberg (1972) introduced five levels of hietologic invasion of malignant melanoma described by Clark et al. Morton(1970) reported various measures of treatment such as surgery combined with radiotherapy or with chemotherapy or with recent immunotherapy with good result. The authors recently experienced 24-yeas old male soldier patient with malignant melanoma on hypothenar region of right palm. The pathologic investigation including electronmicroscopical aspect was carried out and are presented. Treatment consisted of the resections of right axillary lymph nodes and the lesion in situ on the hypotherar region of right hand, pedicle skin graft to right hand from abdomen, and immediate postoperative immunotherapy with intradermal injection of 1.0 cc. of B.C.G. daily on extensor surface of both thighs in turn, which lasted for about 3 months duration to find good result.

3.The Reconstruction of Injured Hands by Microsurgery

The human hand is a highly sophisticated tool of sensibility and prehension, and the reconstruction of injured hands are essential to not only reconstruct of hand function but also reconstruct of body. Seven cases of tendocutaneous flap, a case of osteocutaneous flap, nine cases of thumb reconstruction with a free neurovascular wrap-around flap from the big toe, two cases of second toe-to-index trander, and a case of left ring finger to right index finger transfer were performed at the Department of Orthopedic Surgery of Hanyang University Hospital, and satisfactory results were obtained both comsmetically and functionally.

8.Effect of Exercise on Antioxidant Enzyme Activities of Skeletal Muscle and Liver in STZ-diabetic Rats.

BACKGROUND: The purpose of the present study was to investigate the effect of exercise on the activities of antioxidant enzymes, superoxide dismutase(SOD), glutathione peroxidase(GPX) and catalase(CAT) of skeletal muscle(gastrocnemius) and liver in streptozotocin(STZ) induced diabetic rats. The malondialdehyde(MDA) concentration was also measured as an index of lipid peroxidation of the tissues by exercise-induced oxidative stresses in the diabetic rats. MATERIAL AND METHODS: Male Sprague-Dawley rats were randomly divided into control and STZ-induced diabetic rats. The STZ in citrate buffer solution was injected twice at 5 days intervals intraperitoneally(50, 70 mg/kg respectively). On the 28th day after the first STZ injection, the diabetic animals were randomly divided into pre- and post-exercise groups. The exercise was introduced to the rats of post-exercise group by treadmill running till exhaution with moderate intensity (VO2max: 50-70%) of exercise. The duration of average running time was 2 hours and 19 minutes. RESULTS: The blood glucose concentration was increased(p<0.001) and plasma insulin concentration was decreased(p<0.001) in the diabetic rats. The glycogen concentration in the muscle and liver was decreased by exhaustive exercise in the diabetic rats(p<0.001). In the skeletal muscle, the activities of GPX was increased(p<0.05) and the activities of SOD and CAT were not changed in the diabetic rats compare to the control rats. The activities of GPX was not changed by exercise but the activities of SOD(p<0.01) and CAT (p<0.01) were decreased by exercise in the diabetic rats. The concentration of MDA was not changed by exercise in diabetic rats. and the values of pre-exercise and post-exercise diabetic rats were not different from the value of control rats. In the liver, the activities of SOD was decreased(p<0.01) and the activities of GPX and CAT were not changed in diabetic rats compared to the values of control rats. The activities of SOD, GPX and CAT were not changed by exercise in diabetic rats but the the slight decreasing tendency of the activity of SOD was observed. The MDA concentration was increased in the diabetic rats compared to the values of control rats(p<0.001) but there was no change of MDA concentration by exercise in diabetic rats. CONCLUSIONS: In summary, the exhaustive physical exercise has not been shown to impose oxidative stress on skeletal muscle due to oxygen free radicals regardless of decreament of SOD and CAT in the diabetic rats. In liver tissue, the tissue damage by oxidative stress was observed in diabetic rats but the additional tissue damage by the exhaustive physical exercise was not observed.

9.A Case of Bowen' s Disease Showing Transformation into Invasive Squanmous Cell Carcinoma.

Bowens disease is a precaicerous lesion and may transform into in asive squamous cell carcinoma. We present a case of a 67 year-old male with Bowens disease on the buttock from which squamous cell carcinoma developed. He had anerythematous, scaly and crusted slaque with a nodule on the left buttock. Skin biopsy specimen taken from the plaque and nodule revealed typical histopathological findings of Bowens disease and squlamous cell carcinoma, respectieels. The patient was treated with excision and skin graft under general anesthesia.

10.The Distribution of Fibronectin in the Experimental Keratitis of Rabbit.

The distribution of fibronection was investigated in the carrageenan-induced keratitis of rabbitis. Keratitis was induced by the injection of carrageenan into corneas of the rabbits, and specimens were taken at various intervals. Histological examinations were done with H and E, silver impregnation and PTAH stains. The distribution of fibronectin was observed by using avidin-biotin-peroxydase(ABC) technique. The results were follow. 1. The histological changes in early stages showed acute inflammatory exudate in stroma. After two weeks, granulation tissues were formed. And then after the fourth week, the granulation tissues began to mature with the apperance of fascicle of collagen fibers and thickened epithelium. 2. Fibronectin was demonstrated in a fibrillar appearance in the acute inflammatory exudate of the stroma, which was normally absent. With the development of chronic inflammatory lesions and granulation tissues, the amount of fibronectin was increased, relative to the increase and codistribution of reticular fibers, but with the appearance of mature collagen fibers, the amount was markedly reduced. 3. The fibronectin which was absent in the normal epithelium was prominent in the regenerating epithelium.
