BACKGROUND: Previous studies have shown that in vitro exposure to ultraviolet B(UVB) radiation resulted in a dose-dependent inhibition of natural killer activity(NK activity) of normal human peripheral blood mononuclear cella(PBMC), and that in vivo exposure to snlight also induced NK activity suppression. The precise meehanism of the UV-regulation on the riat iral killer system(NK system) is not established. Objective & METHOD: The purpose of this study is to examine whether the addition of interleukin-2(IL-2) and/or free oxygen radical scavengers, superoxide dismutas(SOD) or sodium azide(SA), is effective in reducing the UVJ3-induced suppression of NK activity of FBMC. RESULTS: The results are as follows 1. The suppressive effect of UVB radiat,ion on NK activity could successfully be prevented in the presence of SOD(100 and 1,000U/ml) during the radiation. 2. SA( LO and 10 M/ml) did not prevent the suppression of NK activity. 3. IL-2(100U/ml) markedly enhanced the NK activity of nonirradiated PBMC, but had no effect on irradiated PBMC. 4. Combination treatment with both IL-2 and free radical scavengers on UVB-irradiated PBMC resulted in no additive or synergistic effect on the prevention of the suppression of NK activity compared with a single treatment with either IL-2 or free radical scavengers. CONCLUSION: In the presserit study, we found that SOD providec a protective effect on NK activity during the UVB radiation and we suggest that superoxide anion(O ) might play a major role in the UV-regulatory mechanisms of the NK system.

Most of malignant melanoma of skin had occurred in transformation from benign nevi(Webster, 1944), especially most frequently from junctional nevi of them. which was characterized by flat surface, dark-brown color and hairlessness(Ackermen, 1948) Blumberg (1972) introduced five levels of hietologic invasion of malignant melanoma described by Clark et al. Morton(1970) reported various measures of treatment such as surgery combined with radiotherapy or with chemotherapy or with recent immunotherapy with good result. The authors recently experienced 24-yeas old male soldier patient with malignant melanoma on hypothenar region of right palm. The pathologic investigation including electronmicroscopical aspect was carried out and are presented. Treatment consisted of the resections of right axillary lymph nodes and the lesion in situ on the hypotherar region of right hand, pedicle skin graft to right hand from abdomen, and immediate postoperative immunotherapy with intradermal injection of 1.0 cc. of B.C.G. daily on extensor surface of both thighs in turn, which lasted for about 3 months duration to find good result.

The human hand is a highly sophisticated tool of sensibility and prehension, and the reconstruction of injured hands are essential to not only reconstruct of hand function but also reconstruct of body. Seven cases of tendocutaneous flap, a case of osteocutaneous flap, nine cases of thumb reconstruction with a free neurovascular wrap-around flap from the big toe, two cases of second toe-to-index trander, and a case of left ring finger to right index finger transfer were performed at the Department of Orthopedic Surgery of Hanyang University Hospital, and satisfactory results were obtained both comsmetically and functionally.

BACKGROUND: Multidrug-resistant tuberculosis(MDR-TB) in patients is mainly caused by acquired drug resistance, However, a small proportion of MDR-TB is caused by initial drug resistance(IDR), which may be somewhat different from acquired drug resistance. This study analyzed the clinical characteristics of IDR in MDR-TB patients to use the results as basic data in managing the disease. METHODS: A retrospective study of 30 IDR cases in MDR-TB patients from Jan. 1995 to Dec. 1998 was perormed. In order to analyzed the clinical charcteristics, the age, sex, family history, duration of negative conversion, number of resistant drugs, treatment regimens, duration of treatment, extent of disease and cavitary lesion on the chest X-ray was examined. In order too analyzed the level of improvement, the extent of the disease and cavitary lesion on the chest X-ray, tested by Wilcoxon signed rank sum test, and the disease free interval rate of 1-year and 4-year was examined using the Kaplan-Meier method. RESULTS: The mean age of the patients was 46.6 years and the sex ratio 1:1. Six(20%) patients had a family history. The mean negative conversin of the sputum AFB stain was 2.6 months. The number of resistant drugs was 7.6 and the number of used drugs 3.6. Twenty-three(67%) patients were treated for less than 12months and 28(93%) patients were treated with first-line drugs. The extent of the disease and the cavitary lesion on the chest X-ray improved after treatment(p<0.05). Among 13 patients who were followed up for 22.6 months, 2(15%) patients relapsed and the disease free interval rate of 1-year and 4-year was 85%. CONCLUSION: It is recommended that the duration of treatment of IDR in MDR-TB with first-line drugs be 9-12 months even if the extent of disease and cavitary lesion on the chest X-ray improves.

Idiopathic gingival fibromatosis is associated with other abnorma ities of which the most striking is hypertrichosis of variable degree. Gorlin and Pindborg described this condition as a distinct syndrome termed idiopathic gingival fibromatosis and hypertrichosis. We report a case of idiopathic gingival fibromatosis and hypertrichosis occuring in 34-year-old woman with enlarged gingival tissue, hypertrichosis, and enlargenment of the breasts, ears and nose. Laboratory tests including hormonal and chromosomal studies were all within normal limits. Histologically, the gingival hyperplastic tissue showed thick bundle of collagen fibers with few fibroblasts or capillaries and little or no inflammatory infiltrate.

We reviewed the cliniccipat.hologic findings of 11 cases of dermatophytic granulomas. Patients consisted of 8 male and 3 females with a mean age of 45 years (range 5-80 years). The symptom was neither painful nor pruritic except one case. Skin lesions mimicked tuberculosis (2 cases), sporotriehosis (2 cases), calcinosis cutis (1 case) or eczema (1 case). The commonest sites involved were upper extremities, followed by legs, cace and scalp. The duration between the onset of the diseases and final diagnosis ranged 1 month to 3 years. Possible predisposing factors causing dermatophytic granulomas appeared to be due to application of steroid ointment (6 cases), diabetes mellitus (2 cases) and trauma (1 case). Histopathologic featured revealed suppurative folliculitis with fo eign body granuloma in most cases and lobular ponniculitis in 2 cases. With PAS and methenamine silver stains, hyphae and spores could be identified on the horny layers, within hair follicles and/or on the dermis in biopsy specimen.', of all cases. Culture of the causativi; organisms using dermal fragment on abouraud media demon-strated T. rubrum in seven patients and M. canis in two. A test for recall antigen(Multitest CMI) showed suppression of cellular immunity in 3 cases. Treatment with griseofulvin or ketoconazole for 2-10 weeks was effective except one case.

The sheaths of tendons are an uncommon site of tuberculous invclvement as compared to other parts of the body. But tuberculou, tenosynovitis occurs often enough to varrant particular attention and study. We report a case of tuberculous tenosynovitis occuring in a 49-year-old woman who had multiple yellowish palpable nodules and swellings distributed along the tencon of flexor digitorum superficialis on the left palm for 3 month. Histopathologic findings showed multip e tuberculoid granulomas in the mid-and deep-dermis. Eosinophilic homogenous trabeculated structure(remnants of tendon sheath resistant to necrosis) was present within the area of caseation. Acid-fast stain showed a crowd of acid-fast bacilli in the deep dermis. The cultures from the skin biopsy spec mens on Ogawa media(3% KH PO4) at 24C, 32C and 37C were all negative. She was treated with surgical excision of granulation tissue and involved tendon sheath followed by systemic administration of INH, RIF and ethambutol for 1 year. The lesions were markedly improved and showed no relaps, during last 5 months of follow up period.
