4.A Case of Malignant Melanoma of Soft Parts with Unusual Histopathologic Findings.

We report a case of malignant melanoma of soft parts presented with a hard mass on the dorsum of the left second toe. Malignant melanoma of soft parts has also been known as clear cell sarcoma of tendons and aponeuroses, which indicates the histologic feature of clear cell predominance and the origin of tumor cells, tendinous or aponeurotic structures. Although our case presented with many characteristic clinicopathologic features of malignant melanoma of soft parts, diagnosing this case as malignant melanoma of soft parts was made with great difficulty because of a rather unusual finding that the major cell component of neoplasm was spindle shaped cells in stead of clear cells. Cases featuring this distinctive histopathologic finding have not been reported individually to our knowledge, but described briefly in the literature. We think this case deserves special attention because of its close resemblance to various sarcomas.

Mediastinal fibrosis is pathologically characterized by chronic inflammation and fibrosis of mediastinal soft tissue. Mediastinal fibrosis is local expression of a family of systemic fibrosing syndroms. This can result in compression of adjacent mediastinal structures. Idiopathic fibrosing syndromes include retroperitoneal fibrosis, sclerosing cholangitis of the orbit and fibrosis of the thyroid gland(Riedel's struma). The cause of these disorders is obscure, in some instance there is an underlying malignancy, infection, history of drug ingestion, or trauma with retoperitoneal bleeding. Treatment of mediastinal fibrosis depends on structures involved by the fibrotic process. The disease is self limited in most case or improved by steroids uses. We experienced a case of idopathic solerosing mediastinitis with orbital fibrous dysplasia of unknown cause, which was confirmed by open lung biopsy, so reported it with a review of literature.

Distant metastasis of a colon carcinoma in situ has not yet been reported. We experienced a case of a sigmoid colon carcinoma in situ with common hepatic lymph node metastasis. After the first operation, we diagnosed dual intramucosal adenocarcinomas of the sigmoid colon without any regional lymph node metastasis. After the second operation, a metastatic adenocarcinoma was found in the common hepatic lymph nodes. We suggest that metastasis in cases of a colonic carcinoma in situ is rare, but possible. The parallel progression model of tumors can explain this early metastasis.

7.Effects of alpha-Lipoic Acid on Apoptotic Cell Death in Rat Hippocampus Following Transient Forebrain Ischemia-reperfusion Injury.

PURPOSE: The purpose of this study was to evaluate effect of alpha-lipoic acid on apoptotic cell death in rat hippocampal neuron following transient forebrain ischemia-reperfusion (I/R) injury. METHODS: The four-vessel occlusion method was used to induce transient I/R injury in the forebrain of Sprague-Dawley rats. In the treatment group, alpha-Lipoic acid (LA) was administered subcutaneously at 50 mg/kg/day for 7 days before induction of I/R injury. RESULTS: Pretreatment with LA significantly reduced the number of TUNEL-positive neurons in the pyramidal cells layer of the hipocampal CA1 region 5 days after the ischemia, suggesting a marked reduction of apoptotic cell death. Pretreatment with LA also resulted in marked suppression at the transcript level of mRNA for caspase-3 at 24 hours, and decreased concentration of the active form of caspase-3 protein in the hippocampus at 1, 3, and 5 days after I/R injury. Furthermore, as indicated by western blot analysis, the concentration of apoptosis-inducing factor (AIF) in the hippocampus was reduced at 1 and 3 days after a transient I/R injury by pretreatment with LA. CONCLUSION: The results of this study suggest that LA has the potential to prevent neuronal cell death in the hippocampus by inhibiting intracellular signaling pathways responsible for apoptosis following transient I/R injury.

9.Accuracy comparison between subtractive and additive methods in fabricating working model

The purpose of this study was to compare the accuracy of the working models fabricated by the subtractive and additive processing methods based on the 3-dimensional reconstruction model. A total of 20 models were fabricated with subtractive processing method from polymethyl methacrylate (PMMA) blocks using the stereolithography (STL) file of master gypsum model and another 20 models were fabricated with additive processing method using 3D printer with 0.025 mm of a layer thickness. The CAD-reference-model (CRM) and CAD-test-model (CTM) were superimposed by a software for accuracy analysis (Geomagic Qualify 13), where the STL files were transformed to point cloud data and the surface data (CRM and CTM) were subjected to initial alignment and followed by re-alignmented according to best-fit superimposition. The distances between surface data and all points, in this process, were converted to the root mean square (RMS) and averaged. In the experimental results, It was shown that the accuracy is higher in work model fabricated by additive processing method compared to one fabricated by subtractive one (p < 0.05). In addition, it is considered that the working model fabricated by subtractive processing method is to be clinically applicable by improving the improper reproducibility of the tooth surface and depressed area.

Endobronchial lipoma is a rare, benign neoplasm of the large bronchi, which makes up about 0.1 percent of all lung tumors. It can produce irreversible parenchymal lung damage or bronchiectasis if diagnosis and treatment are delayed. The treatment of endobronchial lipoma is early removal and surgical procedures depend on the status of distal parenchymal lung damage. We experienced a case of endobronchial lipoma in 53 year-old female patient, which caused total collapse of left lung and save left lung by enucleation of tumor via bronchotomy.
