1.Clinical and Histopathologic Study of the patients with Lichen Striatus.

Nineteen patients with lichen striatus were investigated by clinical data, clinical photographs and histopathologic study by the pattern analysis method of Ackermsn. Results were as follows '. 1. The male to female ratio was 1: 1.1, and average age was 7.8 years. Seven- teen patients (about 90%) visited hospitals during summer season. 2. Nine cases showed superficial perivascular dermatitis, 9 cases interface dermatitis (vacuolar type : 7, lichenoid type, 2), 1 case nodular dermatitis and 2 rases interface dermatitis with nodular dermatitis. 3. In the epidermis, 9 cases showed variable nurnber of dyskeratotic cells. One case showed finding of perforating lichen striatus. 4. Two cases showed inflammation around acrosyringium, 4 cases around hair follicle, 6 cases around eccrine sweat gland and 1 case showed perineural inflamrnation. Summarizing above results, histopathology of lichen striatus characteristically shows superficial perivascular dermatitis, interface dermatitis, and infiltration of inflammatory cells around the sweat gland, which helps to diagnose the disease.

2.B lunted dyspnea perception in severe asthmatics.

BACKGROUND: Blunted perception of dyspnea, which may be related to the increased mortality, has been demonstrated in patients with a history of near-fatal asthma and in the elderly. OBJECTIVE: The purpose of this study was to evaluate the relationship between the perception of dyspnea and the severity of asthma. METHOD: Baseline spirometry and Borg score change(A Borg score) during breathing through an inspiratory muscle trainer were measured in 27 consecutive asthma patients and 11 normal subjects. RESULTS: The baseline Borg score was negatively related to FEV1 A Borg score was significantly lower in asthmatics than in controls at high level of loads. A Borg score was lower in severe asthma than in mild asthma. A Borg score was positively related to the baseline FEV1. Seventy-five percent of severe asthma, 62.5% of moderate one, and 9.1% of mild one showed impaired dyspnea perception. Dyspnea perception was related to age(r, = -0.49, p<0.001). CONCLUSION: Three quarters of patients with severe asthma showed impaired dyspnea perception to inspiratory resistive load. Dyspnea perception was related to asthma severity and age.

3.A Case of Multiple Basal Cell Carcinoma.

We reoport a case of multiple basal cell carcinoma in a 51-year-old male who have taken a herb medication for right hemiplegia during 30 years. The patient had a 15-year history of the black colored, ulcerated nodules on left forehead and erythematous, protruded masses on back. Also, he had a 6-year history of black colored, ulcerated nodules on right temporal area of scalp and brownish, flat plaques on anterior chest, Biopsy specimens from lesions revealed typical histopathologic findings of the basal cell carcinoma. Wide excision with graft was done. One year later, he was expired due to lung carcinoma.

4.Pain after Use of the Central Third of the Patellar Tendon for Anterior Cruciate Ligament Reconstruction: Ipsilateral vs . Contralateral side Harvesting Group.

Complications reported include arthro5brosis, patellofemoral joint pain, patellar tendinitis, patellar tendon rupture, recurrent laxity, posterolateral instabi5ty, medial subluxation of the patella, chronic knee pain and painful neuroma. Among them, anterior knee pain was the rnost frequent eomplication. Authors performed arthroscopy assisted one tunnel technique for reconsction of the ACL using autogenous Bone Patellar tendon-Rone (BPTB) in 87 patients. Patients were divid@d into two groups based on the harvested side (knee) of the BPTB. Group 1 consisted of 45 cases of ipsilateral side and group 2 consisted of 42 cases of contralsteral side in harvesting the BPTB. The patients were foll4wed up for at least 12 months to determine whether using the BPTB harvested from the contralateral knee wguld reduce the anterior knee pain at the ACL reconstructed knee. Althaugh there were no statistically signifiqant differences between group 1 and 2 with regard to anterior knee pain, the prevalence of the anterior knee pain was less frequent in group 2 than group 1. This study showed that using the contralateral side harvested autogenous patellar tendon would be a possible protector for anterior knee pain and assists for early rehabilitation.

5.Osteochondritis Dissecans of the Knee

Osteochondritis dissecans is a condition in which part of the articular surface of a joint separates, due to a plane of cleavage through the subchondral bone. It is found most commonly in the knee although other joint can be affected, notably the elbow, ankle and hip. The etiology remains unclear, and treatment still provokes controversy in the orthopedic community. This review is confined to osteochondritis dissecans of the knee and reports the result of different methods of treatment including arthroscopic drilling. The authors experienced 7 cases of osteochondritis dissecans with received treatment at C.A.F.G.H. from May 1982 to November 1984. The results were obtained as follows: 1. Common symptoms and signs were pain, limitation of motion, quadriceps atrophy and weakness. 2. Five of six cases had histories of trauma. 3. 6.Alveolar soft part Sarcoma with Metastasis to Bone: A Case Report

Alveolar soft part sarcoma is a clinically and morphologically distinct soft tissue tumor that was first defined and named by Christopherson et al in 1952. Since 1953, alveolar soft part sarcoma invading bone have been reported sporadically. We experienced a case of alveolar soft part sarcoma with metastasis to femoral shaft, which was treated by wide resection & vascularized fibular strut graft.

8.Ozon-induced airway hyperresponsiveness of rat tracheal smooth muscle.

BACKGROUND: Ozone (03) induces airway inflammation and hyperresponsiveness which are characteristic features of asthma. There have been few studies observing O3-induced increase in responsiveness of rat airway muscle. Objectives: The aims of this study were to develop an O3-induced nonallergic asthma model using rat tracheal smooth muscle (TSM) and to evaluate the role of airway epithelium on the modulation of muscle responsiveness. METHOD: Five groups of 20 male Sprague-Dawley(SD) rats were exposed to filtered air including 0.12, 0.5, 1.0, or 2.0 ppm 03 for 1 hour. Thirty minutes after the exposure, bronchoalveolar lavage (BAL) and isometric contractile responses of the isolated tracheal ring segments to KCI, acetylcholine (ACh), and electrical field stimulation (EFS) were measured. RESULTS: The percent age of neutrophils was significantly higher and that of alveolar macro-phages in BAL fluid was significantly lower in 2.0 ppm O3-exposed rats than in the control. There were no significant differences in the maximal contractile responses of TSM to KC1, ACh, EFS and in the sensitivity to ACh (ACh-EC50) and EFS (EFS-EC50) between the control group and the ozone exposed group. ACh-EC50 and EFS-EC50 were correlated positively with the percent age of neutrophils and inversely with that of macrophages. Removal of epithelium significantly increased the sensitivity to ACh in O3-exposed group, but not in the control group. CONCLUSION: These findings indicate that O3 induces neutrophilic airway inflammation, but not an increased sensitivity of TSM to ACh or EFS in SD rats. However, O3-induced epithelial damage may be associated with increased muscle response.

10.The Change of Total Ascorbic Acid Level in the Experimental Spinal Cord Injury.

The changes of total ascorbic acid level in the spinal cord tissues following experimental spinal cord injury were observed in the 20 cats. Three small areas (2x1.5cm sized) in the thoracic cord following laminectomy were made and the specimens were removed in one control and the other two areas after 5 & 30 minutes following impact injury respectively in each cat. The significant decrease of the total ascorbic acid level after 5 & 30 minutes following experimental spinal cord injury was found.
