Melorheostosis is a rare from of hyperostosis characterized by candle greasing down along the major axis of long bones. A case of melorheostosis combined with exostosis in the fifth toe was presented here because of its rarity.

2.A Clinical Study of Panoxyl 2.5(R) in Acne Vulgaris.

Bezoyl peroxide, a powerful oxidizing agent, has been uaed topically in the treatment of acne vulgaris as a cream or lotion and has been shown to he effective for this codition. In 1972 Fulton emphaaized the importance of the vehicles used to obtain. maximum potential of benzoyl peroxide and for this, the gel formulation has been examined in many clinical trials. The present study was undertaken to evaluate the therapeutic effect of Fanoxyl 2.5, a 2.5% . benzayl peraxide gel, in patients with acne vulgaris. A total of 31 patients entered this study at the Departrnent of Dermatology, Jeoul National University Hospital rluring 3 rnanths period, from Decemher, 1978 through February, 1979, All patienta were instructed to apply Fanoxyl 2.5' on their affected areas once or twice a day and the nurnber of lesions on two or three t fixed areas were counted before and onee weekly after initation of the treatment. The reaults observed in the 3I patients were as follaws; 1. The percentage decrease in total nurnber of lesions in 4 weeks after treatment was 56.9%. 2. The authors could follow up for 6 weeks in 6 cases. In 6 case, the mean percentage in total number of lesions in 6 weeks after treatment was 81.7%. 3. Clesed and open eomedones were gradually eliminated; and their numbers were decreased in 4 weeks after treatment by 46.5% and 56.8% respeetiveIy. The papules were rapidly eliminated and their numbers were decreased in 4 weeka, after treatment by 79.8%. 4. Although most patients felt a mild burning sensation and tightness at the begining of the treatment, only a minority of the patients complained of discomfort due to pain(l case), erytherna(l case), tightness(2 cases),burning sensation (1 case), scaling(1 case). But all were tolerable without taking any specific measures or discontinuance of application. In one case, contact dermatitis developed after 4 weeks treatment. The authors concluded through this experiment that Panoxyl 2.5' (2.5% benzoyl peroxide gel) is a very effective local therapeutic agent in the treatment of acne vulgaris patients.

5.Treatment of the Tibial Plafond Fractures Using the Ilizarov Method.

An intra-articular fracture of the distal tibia or a fracture of the tibial plafond is a relatively uncommon but the most difficult fracture to manage. There are many treatment options and many authors have reported good results after open reduction and rigid internal fixation to restore the length with articular surface and bone-graft followed by early motion and prolonged non-weight bearing. But in the fractures with severe comminution or with severe soft tissue injury, tries for internal fixation with plate and screws in the distal tibia will result in extensive soft tissue stripping of the wound with precarious blood supply, high rates of wound breakdown, superficial and deep infection, and osteomyelitis. The purpose of this study is to evaluate the results of treatment of severely open (open type III-A and more by Gustilo and Anderson) or comminuted plafond fractures (fracture type III and more by Ovadia and Beals) with an Ilizarov method with a minimum follow up of one year (average: 27.9 months) and we have reviewed 12 cases from December 1990 to October 1995. The results were as follows. 1. The average time of bone union were 17.5 weeks (range: 6-42weeks). 2. The radiological results of the treatment were excellent in 4 patients (33.3%), good in 4 patients (33.3%) and poor in 4 patients (33.3%) by radiological assessment of Crutchfield and the clinical results of the treatment were excellent in 4 patients (33.3%), good in 5 patients (41.7%) and poor in 3 patients (25.0%) by functional rating scaie of Crutchfield. 3. It seems that prognosis depends on severity of comminution at the time of injury. 4. The complications were bony collapse of the distal tibia in 2 patients, nonunion in 1 patient, degenerative arthritis of the ankle joint in 2 patients and chronic osteomyelitis in 2 patients. 5. Ilizarov technique is an useful method in the management of severely open or comminuted plafond fracture of the tibia and it would be preferable to apply buttress plate or bone graft after improvement of skin condition to prevent the late collapse.

7.The Transition of Internal Diseases of the aged in Korea.

BACKGROUND: Recent times, elderly population increased due to development of medical treatment and extension of the average span of a human life. Hereupon, variable studies and measures for elderly population were made an attempt in the world at large. In medical treatment, an aged person related an institutes begin to vivacious activity for reflection of this national concern. So we survey the transition of internal diseases of the aged in Korea for 10 years lo support the policy of medical treatment. METHODS: In order to access this study, we use data of korea medical insurance coorporation and national statistical office republic of Korea. We classified internal diseases into several diseases by age, and sex. RESULTS: 1. The transition of out-Patients Digestive troubles have the most high prevalence in both men and women, and cardiovascular diseases are become increasing. Endocrine diseases are become increasing also. 2. The transition of in-patients 1) 65-69 years old Neoplastic diseases and digestive diseases are high in men, but cardiovascular disease are become increasing. Digestive diseases are high in women but, cardiovascular disease and endocrine diseases are become increasing. 2) 70-74 years old Cardiovascular diseases are decreased in men and especially in women. 3) Over 75 years old Digestive diseases are decreased in both men and women, endocrine diseases and cardiovascular diseases are increased in women. 3. The transition of the cause of death. The cardiovascular diseases are high in both men and women, and showed decreasing tendency gradually, Neoplastic diseases and endocrine diseases are increasing in steady. CONCLUSION: In our data, the cardiovascular diseases are high especially, in women according to age in the transition of internal diseases of an aged man in Korea, recent 10 years and endocrine diseases are increasing in steady. So, in medical treatment, we must consider many-sided methods to access this population

9.Comparison of the Effect of Femoro-Femoral Extracorporeal Lung Assist(ECLA) and Jugulo-Carotid ECLA on the Systemic Oxygenation in Dogs.

The extracorporeal respiratory support has (ECMO) or extracorporeal lung assist (ECLA). Jugulocarotid(JC) ECLA, which drains venous blood via the internal jugular vein and perfuses via the common carotid artery, is recognized as a therapeutic modality for acute respiratory failure of neonates. But recent follow-up studies report the possible neurologic complications after the ligation of the carotid artery. Femoro-femoral (FF) bypass is reported to be effective in cardiac support during cardiopulmonary resuscitation. Surgical approach of FF bypass is easier because of the superficial location of great vessels. To evaluate the cardiorespiratory support of FF bypass, twelve mongrel dogs were divided into JC and FF groups(n6 in each group) and ventilated artificially at F1O2 of 0.3 and 0.15 in a dog. While ventilating the oxygenator with N2 gas, control hemodynamic and blood gas values were obtained at FO1O2 0.3 and at F1O2 0.15. The values during VA ECLA in both groups were measured while flushing O2 gas into the oxygenator. We compared bypass flows and oxygenation effects between two groups. l) The comparison between JC ECLA and FF ECLA in F1O2 0.3 Mean arterial pressure (MAP) and cardiac output (CO) increased in both groups and mean pulmonary arterial pressure (MPAP) decreased in JC group. Bypass flow rates were 86+/-6ml/kg/min in JC group and 62+/-8 ml/kg/min in FF group. 2) The comparison between JC ECLA and FF ECLA in FO 0.15 MAP and CO increased in both groups and MPAP decreased in JC group. VA ECLA increased the arterial oxygen tension from 49+/-12 mmHg to 93+/-20 mmHg in JC group and from 54+/-14 mmHg to 77+/-12 mmHg in FF group. Bypass flow rates were 103+/-21 ml/kg/min in JC group and 67+/-12 ml/kg/min in FF group. The amount of changes of deltaPaO2(y) related to bypass ratio(x) was same between two groups The systemic oxygenation effect of FF ECLA in hypoxia was as efficient as that of JC ECLA. In conclusion, FF ECLA can be adopted for acute respiratory failure in which PaO2 is maintained about 50mmHg.

10.A Clinical Study of Ribavirin (Virazole) in Herpes Zoster.

Ribavirin(l-b-D-ribofuranosyl-l, 2, 4-triazole-3-carboximide, Virazole) is a. synthetic nucleoside derivative with reported broad spectrum activity in vitro and in vivo against a wide variety of DNA and RNA viruses. This drug works by selective inhibition of viral protein in the early stage of viral replication, but does not influence on host cell protein. The present study was undertaken to evaluate the therapeutic effect of Ribavirin in patients with herpes zoster. A total of 15 patients entered this study at the Department of Dermatology, Seoul National University Hospital during 5 months' period, from May, 1980 to September, 1980, Ribavirin(10mg/kg/d) was given to 10 patients. After initiation of therapy, we observed duration of pain and vesicles and onset of crust formation in each patient clinically. The results observed in the 15 patients were as follows. 1. The average duration of pain was 6.0 days in Ribavirin group and 8.6 days in. control group (p<0. 05). 2. Postherpetic neuralgia was not found in any patients treated with Ribavirin. 3. The average duzation of all vesicles and onset of crust formation were 7.0 days and 3.8 days in Ribavirin group and 7.2 days and 4.2 days in control group respectively. 4. No side effects were observed during the course of Ribavirin treatment.
